[Comparison between digoxin and atenolol in chronic atrial fibrillation].

The benefits of digoxin in patients with atrial fibrillation may be reduced due to its limited effect on atrioventricular conduction. The aim of this work was to compare digoxin and atenolol on functional class, resting and exercise heart rate and exercise capacity in patients with atrial fibrillation. Thirteen subjects with this condition, normal echocardiographic left ventricular function and size, a resting heart rate less than 80 beats/min and with no contraindication for beta blocker or digoxin use were studied. Patients were randomly assigned to receive initially digoxin 0.25 mg o.d. or

Pedal cadence differentially impacts cerebral blood flow but not a postexercise executive function benefit: evidence from active and passive exercise.

An exercise-induced increase in cerebral blood flow (CBF) serves as a candidate mechanism underlying a postexercise executive function (EF) benefit. Here, healthy young adults completed four 20-min interventions of active and passive (via mechanical flywheel) cycle ergometry at pedal cadences of 55 and 85 revolutions per minute (rpm). Active and passive exercise were used because each modality increases CBF-albeit via distinct mechanisms-and different pedal cadences were used because an increase in passive exercise cadence is thought to increase CBF. Hence, we sought to determine whether the m

Upfront Exercise Hemodynamic Phenotyping of Patients with Pulmonary Hypertension and Left Heart Disease Risk Factors: A Prospective Experience.

BACKGROUND: Resting hemodynamic assessment of suspected pulmonary hypertension (PH) in older adults with cardiovascular risk factors may not effectively discriminate pre-capillary from post-capillary disease. Exercise right-heart catheterization (RHC) can improve the ability to make this distinction, yet its clinical value remains unclear. We aimed to describe the challenges encountered in hemodynamic exercise phenotyping in this population and evaluate the relationships of exercise classifications to worsening clinical events, including mortality. METHODS: We prospectively recruited patients

Heart Rate Index-An Alternative Exercise-Based Equation for Estimating Peak VO 2.

PURPOSE: Heart rate (HR)-derived variables (HR reserve [HR peak  - HR rest ], chronotropic indices [attenuated HR response to exercise], HR recovery [attenuated HR response to exercise recovery], and peak HR index [HR peak /HR rest ]) together with peak oxygen uptake (VO 2peak ) are potential prognostic variables for cardiovascular and all-cause mortality. However, heart rate index (HRI) has not been established as a surrogate for VO 2peak , whether measured (Meas) or estimated (Est), during cycle ergometry (CE) and/or treadmill testing (TT). METHODS: HR-derived prognostic variables to assess

Effects of Exercise Interventions on Cardiac Structure, Function, and Mechanics in Individuals with Chronic Motor-Complete Spinal Cord Injury: An Exploratory Randomized Clinical Trial.

BACKGROUND: Individuals with spinal cord injury (SCI) at or above T6 face increased cardiovascular disease (CVD) risks due to altered autonomic control and physical inactivity. Arm cycle ergometry training (ACET) or body weight-supported treadmill training (BWSTT) may improve cardiovascular health, but the impact on cardiac structure and function remains unclear. OBJECTIVES: The study aimed to compare the impact of two exercise interventions on cardiac measures in individuals with chronic SCI. METHODS: Participants with motor-complete SCI (C4-T6, American Spinal Injury Association Impairment

Haemodynamic and metabolic adaptations in coronary microvascular disease.

OBJECTIVE: We aimed to evaluate the microcirculatory resistance (MR) and myocardial metabolic adaptations at rest and in response to increased cardiac workload in patients with suspected coronary microvascular dysfunction (CMD). METHODS: Patients with objective ischaemia and/or myocardial injury and non-obstructive coronary artery disease underwent thermodilution-derived microcirculatory assessment and transcardiac blood sampling during graded exercise with adenosine-mediated hyperaemia. We measured MR at rest and following supine cycle ergometry. Patients (n=24) were stratified by the restin

Surgical ablation of the right greater splanchnic nerve for the treatment of heart failure with preserved ejection fraction: first-in-human clinical trial.

AIMS: Inappropriate control of blood volume redistribution may be a mechanism responsible for exercise intolerance in heart failure with preserved ejection fraction (HFpEF). We propose to address this underlying pathophysiology with selective blockade of sympathetic signalling to the splanchnic circulation by surgical ablation of the right greater splanchnic nerve (GSN). METHODS AND RESULTS: In a single-arm, prospective, two-centre trial, 10 patients with HFpEF (50% male, mean age 70 ± 3 years) all with New York Heart Association (NYHA) class III, left ventricular ejection fraction >40%, pulm

Meeting international aerobic physical activity guidelines is associated with enhanced cardiovagal baroreflex sensitivity in healthy older adults.

PURPOSE: Cardiovagal baroreflex sensitivity (cvBRS) reflects the efficiency of modulating heart rate in response to changes in systolic blood pressure. International guidelines recommend that older adults achieve at least 150 min of moderate-vigorous physical activity per week. We tested the hypothesis that older adults who achieve these guidelines will exhibit greater cardiovagal baroreflex sensitivity versus those who do not. METHODS: A cross-sectional comparison of older adults who did (active, 66 ± 5 years, 251 ± 79 min/week; n = 19) and who did not (inactive, 68 ± 7 years, 89 ± 32 min/we

Physical fitness interventions for nonambulatory stroke survivors: A mixed-methods systematic review and meta-analysis.

INTRODUCTION: Physical fitness training after stroke is recommended in guidelines across the world, but evidence pertains mainly to ambulatory stroke survivors. Nonambulatory stroke survivors (FAC score ≤2) are at increased risk of recurrent stroke due to limited physical activity. This systematic review aimed to synthesize evidence regarding case fatality, effects, experiences, and feasibility of fitness training for nonambulatory stroke survivors. METHODS: Eight major databases were searched for any type of study design. Two independent reviewers selected studies, extracted data, and assess

The unnatural history of the ventricular septal defect: outcome up to 40 years after surgical closure.

BACKGROUND: Few prospective data are available regarding long-term outcomes after surgical closure of a ventricular septal defect (VSD). OBJECTIVES: The objective of this study was to investigate clinical outcomes>30 years after surgical VSD closure. METHODS: Patients who underwent surgical VSD closure during childhood between 1968 and 1980 were reexamined every 10 years. In 2012, we invited eligible patients to undergo another examination, which included electrocardiography, Holter monitoring, echocardiography, bicycle ergometry, measurement of N-terminal pro-B-type natriuretic peptide, and

Impaired right and left ventricular diastolic myocardial mechanics and filling in asymptomatic children and adolescents after repair of tetralogy of Fallot.

AIMS: After tetralogy of Fallot (TOF) repair patients have right ventricular (RV) dysfunction and reduced exercise tolerance. Diastolic dysfunction may be important but is as yet poorly characterized. The early diastolic strain rate (SR) is a measure of ventricular relaxation, and may be useful to assess diastolic mechanics in TOF. We hypothesized that children after TOF repair have diastolic dysfunction and dyssynchrony by this measure, and sought to determine their relationship with pulmonary regurgitation (PR), RV enlargement, and aerobic exercise capacity. METHODS AND RESULTS: We prospect

Varying the heart rate response to dynamic exercise in pacemaker-dependent subjects: effects on cardiac output and cerebral blood velocity.

Cerebral blood flow increases upon the transition from rest to moderate exercise, but becomes affected when the ability to raise CO (cardiac output) is limited. HR (heart rate) is considered to contribute significantly to the increase in CO in the early stages of dynamic exercise. The aim of the present study was to test whether manipulation of the HR response in patients dependent on permanent rate-responsive ventricular pacing contributes to the increase in CO, MCA V(mean) [mean MCA (middle cerebral artery) velocity] and work capacity during exercise. The effect of setting the pacemaker to D

A governing relationship for repetitive muscular contraction.

During repetitive contractions, muscular work has been shown to exhibit complex relationships with muscle strain length, cycle frequency, and muscle shortening velocity. Those complex relationships make it difficult to predict muscular performance for any specific set of movement parameters. We hypothesized that the relationship of impulse with cyclic velocity (the product of shortening velocity and cycle frequency) would be independent of strain length and that impulse-cyclic velocity relationships for maximal cycling would be similar to those of in situ muscle performing repetitive contracti
