Avoidance and management of perioperative complications of endoscopic third ventriculostomy: the Dhaka experience.

OBJECT: Although endoscopic third ventriculostomy (ETV) is a minimally invasive procedure, serious perioperative complications may occur due to the unique surgical maneuvers involved. In this paper the authors report the complications of elective and emergency ETV and their surgical management in 412 patients from July 2006 to October 2012 at Dhaka Medical College Hospital (a government hospital) and other private hospitals in Dhaka, Bangladesh. The authors attempted some previously undescribed simple maneuvers that may help to overcome the difficulties of managing complications. METHODS: The

Spontaneous regression of germinoma in the pineal region before endoscopic surgery: a pitfall of modern strategy for pineal germ cell tumors.

Since its introduction in the neurosurgical field, neuroendoscopy has played an important role in the treatment of pineal germ cell tumors. The authors report a case of pineal germinoma revealing spontaneous regression before neuroendoscopic surgery. A 15-year-old boy presented with chronic headache, deterioration of his visual acuity, and diplopia. Head magnetic resonance imaging (MRI) revealed a large tumor in the pineal region and triventricular hydrocephalus due to occlusion of the aqueduct. As levels of tumor markers in serum and cerebrospinal fluid, including those of alpha-fetoprotein,

Novel application of computer-assisted cisternal endoscopy for the biopsy of pineal region tumors: cadaveric study.

BACKGROUND: Long-standing debate continues about the management and biopsy of pineal tumors because of their complex microsurgical anatomy and deep location. Inspired by the concept of biopsy under direct visualization in the absence of hydrocephalus, we explored the effectiveness of neuroendoscope outside of its traditional territory using a new minimally invasive technique, computer-assisted cisternal endoscopy (CACE), for the biopsy of pineal tumors. METHOD: Five cadaver heads were dissected to expose the pineal region through the posterior fossa. In the other 5 heads, a rigid endoscope-wa

[Endoscopic third ventriculostomy--a physiological alternative to shunts as treatment for obstructive hydrocephalus in children].

BACKGROUND: The standard treatment for hydrocephalus is a ventriculoperitoneal shunt which is associated with high rate long-term morbidity. Seeking alternative treatments, neuroendoscopic third ventriculostomies (ETV) have been performed in patients with obstructive hydrocephalus. During this procedure, the third ventricular floor is perforated thus bypassing the pathological obstruction. In this procedure, no hardware is left and fluid pressures are more physiologically balanced compared to shunted cases. STUDY GOALS: To present our experience with endoscopic third ventriculostomy (ETV) in

We report two cases of intraventricular hematoma with obstructive hydrocephalus. We perform endoscopic treatment of the hydrocephalus: aspiration of the hematoma associated with a third ventriculostomy. Both patients respectively aged 59 and 74 years had an obstructive hydrocephalus due to intraventricular hemorrage. The patient neurological status worsen at day 6 for patient No. 1 and at day 4 for patient No. 2. Endoscopic ventriculoscopy was performed respectively at day 6 and at day 5. Operatively, immediate vision was poor but was progressively improved by repetitive irrigation with Ringer

Endoscopically assisted microneurosurgery.

Technological developments in neuroendoscopy are leading to an expansion of applications into the realm of microneurosurgical procedures. The new dimension that using an endoscope provides requires insight into different neuroanatomical aspects and a new kind of strategy in planning a microneurosurgical procedure. To gain some new insights into these exciting aspects of neurosurgery we have explored the sellar, parasellar, and posterior fossa regions in 50 fresh anatomical specimens and used various types of endoscopes to observe the surgically relevant neurotopographical details. We then util

Comparative effectiveness of flexible versus rigid neuroendoscopy for endoscopic third ventriculostomy and choroid plexus cauterization: a propensity score-matched cohort and survival analysis.

OBJECTIVE Endoscopic third ventriculostomy (ETV)/choroid plexus cauterization (CPC) has become an increasingly common technique for the treatment of infant hydrocephalus. Both flexible and rigid neuroendoscopy can be used, with little empirical evidence directly comparing the two. Therefore, the authors used a propensity score-matched cohort and survival analysis to assess the comparative efficacy of flexible and rigid neuroendoscopy. METHODS Individual data were collected through retrospective review of infants younger than 2 years of age, treated at 1 of 2 hospitals: 1) Boston Children's Hos

Frameless neuronavigation-guided endoscopic total en-bloc removal of a third ventricular colloid cyst: a case report on surgical technique.

A 26-year-old man was referred to our department with recurrent episodes of loss of consciousness. The radiological evaluation of the patient's cranium showed a third ventricular colloid cyst with only a slight degree of obstructive hydrocephalus. The complete, en-bloc removal of the cyst was achieved by a frameless neuronavigation-guided endoscopic resection technique. The patient had an uneventful post-operative period and was discharged home on the fourth post-operative day without any neurological or psychological deficit. The surgical technique and pertinent literature are discussed with

Anatomical Surgical Trajectory for Endoscopic Transcortical Third Ventriculostomy: A Neuronavigation-Guided Cadaveric Study With Pediatric Clinical Correlation.

BACKGROUND AND OBJECTIVES: Endoscopic third ventriculostomy (ETV) is a widely used surgical treatment for obstructive hydrocephalus. While the anatomical landmarks of the third ventricular floor are well described, the organization of white matter (WM) tracts along the transcortical ETV corridor and their correlation with neuronavigation and endoscopy remain insufficiently clarified. This study aimed to anatomically map the transcortical ETV trajectory from cortical entry to the third ventricular floor using stepwise cadaveric dissections integrated with neuronavigation and clinical correlatio

Successful endoscopic evacuation of traumatic intracerebral hemorrhage: A report of two cases.

BACKGROUND: Traumatic intracerebral hemorrhage (TICH) often coexists with other traumatic intracranial lesions and presents complex pathophysiology, making surgical decision-making challenging. Although craniotomy has long been the standard treatment for space-occupying traumatic hematomas, endoscopic hematoma evacuation has emerged as a minimally invasive alternative. However, evidence supporting its application to TICH remains limited. CASE DESCRIPTION: We report two patients with TICH who underwent endoscopic hematoma evacuation for acute traumatic management. In both cases, neurological d

The role of neuroendoscopy in treatment of pediatric brain abscesses: case series and systematic review of the literature.

Treatment of brain abscesses in children requires a multimodal approach, consisting of neurosurgical evacuation and intravenous antibiotics. Endoscopic aspiration has been used for extensive removal of the purulent collection, but few pediatric cases have been described in literature. We report the management of endoscopically treated brain abscesses in children in our department and provide a systematic review of the literature. We retrospectively collected data on a series of children with intracerebral abscesses treated endoscopically. The literature search was performed and studies between

Treatment of Pyogenic Ventriculitis with Retroauricular External Ventricular Drainage.

BACKGROUND: Pyogenic ventriculitis (PV) and intraventricular empyema (IVE) are severe intracranial infections with high morbidity and mortality rates. Currently, there is no standardized treatment strategy for these conditions. Existing therapeutic approaches, including neuroendoscopic lavage, repeated external ventricular drainage (EVD), and long-tunneled EVD, each have their respective limitations. Building on the theories of long-tunneled EVD, we developed the retroauricular external ventricular drainage (RaEVD) technique. This study aims to assess the clinical effectiveness and safety of R
