A new treatment strategy for mid-low rectal cancer patients exhibiting a clinical complete or near-complete response to neoadjuvant chemoradiotherapy: Transanal endoscopic microsurgery --A multicenter prospective case-control clinical trial by MONT-R.

BACKGROUND: Total mesorectal excision is the standard surgery for locally advanced rectal cancer (LARC) after neoadjuvant chemoradiotherapy (nCRT), but it may lead to high complication rates and poor quality of life. This study evaluates whether transanal endoscopic microsurgery (TEM), as a partial resection procedure, can enhance quality of life for clinical complete response (cCR) or near-cCR patients without compromising survival. METHODS: Between May 2017 to September 2021, 80 patients with T3-4N0M0 or TanyN+M0 mid-low rectal cancer achieving cCR or near-cCR post-nCRT were prospectively i

Comparison of transanal total mesorectal excision and robotic total mesorectal excision for rectal cancer: an inverse probability of treatment weighting analysis.

BACKGROUND: Both transanal total mesorectal excision (TaTME) and robotic total mesorectal excision (robotic TME) have been developed to optimize mesorectal dissection quality in rectal cancer surgery. However, current evidence remains inconclusive regarding the comparative advantages between these two minimally invasive approaches. MATERIALS AND METHODS: Patients who underwent curative TaTME or robotic TME for rectal cancer at our institution from 2015 to 2021 were retrospectively reviewed. The primary outcomes were circumferential resection margin (CRM) involvement and oncological outcomes (l

Is Transanal total mesorectal excision (TME) as safe and effective as laparoscopic TME for low rectal cancer? A propensity score-matched analysis.

PURPOSE: Minimally invasive surgery for low rectal cancer is technically demanding because of the restricted pelvic access and visualization. Transanal total mesorectal excision (TaTME) was developed to address these challenges, but concerns remain regarding its safety and oncological validity. We conducted this study to compare the short-term surgical outcomes and long-term oncological results of TaTME with those of conventional laparoscopic TME (LapTME). METHODS: The subjects of this retrospective review were 154 patients who underwent radical resection for low rectal cancer between 2009 an

Potential sexual function improvement by using transanal mesorectal approach for laparoscopic low rectal cancer excision.

OBJECTIVE: Preliminary results of the transanal approach for low rectal cancer suggest better oncological outcomes than the conventional laparoscopic approach. We currently report the functional results. METHODS: From 2008 to 2012, 100 patients with low rectal cancer and suitable for sphincter-saving resection were randomized between transanal and laparoscopic low rectal dissection. Patients derived from this randomized trial were enrolled for functional assessment. End points were bowel function (LARS bowel and Wexner continence scores) and urogenital function (IPSS, IIEF-5 and FSFI-6 scores

Cap polyposis: further experience and review.

PURPOSE: Cap polyposis is characterized by the presence of inflammatory polyps with a "cap" of granulation tissue. It may represent one end of a spectrum of conditions caused by chronic straining. This experience represents the second largest reported series of cap polyposis. METHODS: The case notes of all patients with histologically proven cap polyposis were reviewed retrospectively and clinicopathologic features identified. A MEDLINE search was performed from 1985 to 2002 using cap polyps, polyposis, and inflammatory polyp as key words and further hand search was undertaken of key referenc

Rectal Surgery Evaluation Trial: protocol for a parallel cohort trial of outcomes using surgical techniques for total mesorectal excision with low anterior resection in high-risk rectal cancer patients.

AIM: Total mesorectal excision (TME) is the standard of care for rectal cancer, which can be combined with low anterior resection (LAR) in patients with mid-to-low rectal cancer. The narrow pelvic space and difficulties in obtaining adequate exposure make surgery technically challenging. Four techniques are used to perform the surgery: open laparotomy, laparoscopy, robot-assisted surgery and transanal surgery. Comparative data for these techniques are required to provide clinical data on the surgical management of rectal cancers. METHODS: The Rectal Surgery Evaluation Trial will be a prospect

Postoperative Intra-Pouch Mucosal Bridge Formation in a Child with Ulcerative Colitis.

INTRODUCTION: Restorative proctocolectomy with construction of an ileal J-pouch anal anastomosis is an established gold standard procedure for managing ulcerative colitis. One of the reported complications is a residual mucosal bridge as a result of leaving an apical bridge remnant when constructing the ileal J-pouch. However, now that the surgical procedure is well established, such complications rarely occur. CASE PRESENTATION: A 12-year-old girl presented to our hospital because of anal pain. She had undergone three-stage surgery for ulcerative colitis refractory to medical therapy, the th

A new grading of rectal internal mucosal prolapse and its correlation with diagnosis and treatment.

The management of rectal internal mucosal prolapse (RIMP) is not based on an accepted classification of the lesion which helps to choose the appropriate treatment. The aim of this prospective study was to report a new endoscopic grading of RIMP and to evaluate its clinical value. Thirty-two patients (7 men, 25 women; mean age 56 years, range 28-72) affected by symptomatic RIMP were prospectively classified as follows: RIMP was defined as first degree when detectable below the anorectal ring on straining, as second degree when it reached the dentate line, and as third degree when it reached the
