Has the incidence of perforated peptic ulcer decreased over the last decade?

BACKGROUND: The therapeutic use of new drugs for inhibiting gastric secretion together with Helicobacter pylori antimicrobe therapy has given rise to controversy over the current incidence of perforated peptic ulcer (gastroduodenal peptic ulcer, GDPU). The aim of this study is to analyze the incidence of ulcer perforation over the last 12 years in our health area and the influence of new medical treatments. MATERIAL AND METHODS: Our series includes 246 patients operated on for perforated peptic ulcer during a 12-year period (January 1987 to December 1998) in our health area. The mean patient

Nonesophageal side-effects after antireflux surgery plus acid-suppression duodenal diversion surgery in patients with long-segment Barrett's esophagus*.

During the last years we have employed acid-suppression duodenal diversion procedures (truncal vagotomy-partial gastrectomy plus Roux-en-Y gastrojejunostomy) in addition to antireflux surgery in order to treat all the pathophysiological factors involved in the genesis of Barrett's esophagus. We have observed very good results concerning the clinical and objective control of GERD at the long-term follow up after this procedure. However, it could be associated with other nonesophageal symptoms or side-effects. This study was conducted to evaluate the presence of gastrointestinal symptoms (diarrh

[Treatment of alkaline gastritis and recurrent ulcer following Roux-en-Y gastrectomy and construction of a Rumpf antireflux valve].

Two-third gastrectomy or supplementary gastrectomy was performed on 13 patients, between July 2, 1986 and September 30, 1988. Continuity of the gastro-intestinal tract was restored by end-to-side gastrojejunostomy according to Roux-en-Y. An antireflux valve according to Rumpf was inserted 15 cm below the gastroenteroanastomosis, and end-to-side jejunostomy was performed about six centimetres below the valve. This surgical method was applied to six patients for treatment for alkaline gastritis. The same method was used on another seven patients to prevent development of alkaline gastritis. All

Case Report: Recurrent peptic ulcer perforations-a possible role of median arcuate ligament syndrome.

INTRODUCTION: Median arcuate ligament syndrome (MALS), also known as Dunbar syndrome, is a rare vascular compression disorder caused by extrinsic compression of the celiac artery by the median arcuate ligament, affecting 2/100,000 people. Clinical presentation is often non-specific and may include postprandial abdominal pain, nausea, vomiting, diarrhea, and weight loss. Impaired splanchnic perfusion may contribute to gastrointestinal ischemia and, in rare cases, recurrent peptic ulcer disease. We report a patient with multiple episodes of perforated peptic ulcer disease associated with previou

Laparoscopic cholecystectomy and choledocholithotomy in patients with a previous gastrectomy.

BACKGROUND: An increased incidence of cholelithiasis has been widely reported after truncal vagotomy and after gastric resection. In the early phase of patient selection, previous gastrectomy has been considered a relative contraindication to laparoscopic cholecystectomy (LC). In this study, we examined the management of LC in patients with previous gastrectomy. STUDY DESIGN: LC was attempted on 1,260 consecutive patients. Of these patients, 29 had a previous gastrectomy. Surgical procedures that had been performed included Billroth I gastrectomies (15), Billroth II gastrectomies (10), and to

Metabolic problems after gastric surgery.

The findings on dumping syndrome (DS) are not consistent considering its relations with age, sex, weight/height, smoking habits, race, dose of oral glucose, the time elapsed since surgery, the function of exocrine pancreas nor the duration of ulcer symptoms. The patients after total gastrectomy (TG) may present relative postprandial lack of insulin. As a sign of long-term hyperglycemia elevated HbA1 has been measured in DS patients. Oral galactose test may reveal new features of DS. Abnormalities in splanchnic blood circulation as well as release of intestinal hormones are involved with DS. Di

Neuroendocrine regulation of canine gastric secretion, emptying and blood flow.

Abstract Several peptides have been implicated as central nervous system transmitters regulating various peripheral organ systems. This study examined the central nervous system effects of rat corticotrophin-releasing factor (CRF), salmon calcitonin (CT), beta-endorphin (beta-End), neurotensin (NT), rat calcitonin gene-related peptide (CGRP) and bombesin (BOM) on gastric acid secretion, gastric emptying and left gastric artery flow in conscious dogs. All of these peptides, injected into the third cerebral ventricle, significantly inhibited gastric acid secretion but not plasma gastrin concentr

Subdiaphragmatic vagal deafferentation fails to block the anorectic effect of hydroxycitrate.

We investigated the neural mediation of the feeding suppression through orally administered hydroxycitrate (HCA) in male rats that were fed a high-glucose diet (about 48% glucose). Ten-day ad libitum food intake and body weight regain after previous body weight loss (13% of initial body weight) due to restrictive feeding were measured in rats with sham deafferentation (SHAM; n = 6), subdiaphragmatic vagal deafferentation (SDA; n = 7), and SDA plus celiac-superior mesenteric ganglionectomy (SDA/CGX; n = 9). HCA suppressed the 10-day cumulative food intake in all surgical groups and body weight

Multiple gastric Dieulafoy lesions in an adolescent female with massive recurrent upper gastrointestinal bleeding: diagnostic and therapeutic challenge - a rare case report.

INTRODUCTION: Dieulafoy's lesion is a rare cause of upper gastrointestinal bleeding, accounting for approximately 1-2% of cases. It is characterized by the erosion of an abnormally large submucosal artery through intact mucosa, resulting in sudden and life-threatening hemorrhage. Diagnosis is often difficult due to intermittent bleeding and non-diagnostic initial endoscopy. CASE PRESENTATION: We report a 17-year-old female with no significant medical history who presented with massive recurrent hematemesis and hemorrhagic shock, with a hemoglobin level of 6.5 g/dL. Upper gastrointestinal endo

Endoscopic pyloric balloon dilatation obviates the need for pyloroplasty at esophagectomy.

BACKGROUND: Because the rate of acquired pyloric stenosis (APS) from truncal vagotomy is 15%, many surgeons perform pyloroplasty or pyloromyotomy at the time of esophagectomy. Endoscopic pyloric balloon dilatation (EPBD) is another method to manage APS. This study evaluated a cohort treated with preoperative EPBD. METHODS: This is a retrospective review of all patients treated with preoperative EPBD and esophagectomy for cancer from 2002 to 2009 at Brigham and Women's Hospital, a tertiary care center. Outcome measures included need for subsequent surgery for gastric outlet obstruction, rate o

Three computer models for the calculation of prevalence of peptic ulcer disease during long-term treatment.

At present the effects of maintenance treatment for peptic ulcer disease are usually calculated by using 'life-table' analyses. Whilst these accurately demonstrate the speed with which an initial relapse occurs they make no allowance for the fact that, in clinical practice, a relapse often responds to a further course of full-dose treatment and the patient then returns to maintenance therapy. A further compounding factor is that, in any long-term study, patients will be lost to follow-up for a variety of reasons not all related to failure of the treatment. In this paper we describe the use of

Surgical Treatment of Bleeding Ulcer in Cases When the Gastroenterological and Radiological Approach is Insufficient or Disabled - Single Center Expirience.

BACKGROUND: Urgent surgical treatment of bleeding gastric and duodenal ulcer is indicated in cases where there is no treatment by an interventional gastroenterologist and radiologist readily available, or there is no satisfactory response to the applied interventional procedure. OBJECTIVE: The aim of our study is to show that there is the still large number of patients with massive bleeding gastric or duodenal ulcers who had to undergo emergency surgery, in order to achieve hemostasis, provide survival analisys, and to present a methode of the surgical procedure which we perfomed. METHODS: T

What is the place of antrectomy with Roux-en-Y in the treatment of reflux disease? Experience with 83 total duodenal diversions.

Antrectomy with Roux-en-Y gastrojejunostomy was performed in 83 patients with "complicated" forms of peptic esophagitis. The esophagitis was considered complicated either because of the severity of the lesions (stricture, brachyesophagus, or endobrachyesophagus) or because of postoperative conditions after one or more previous operations (Heller's myotomy, esophagogastric resections, or hiatal hernia repair). A standard procedure was performed in 56 patients while technical adjustments were required for 27 patients who had previously undergone surgery. Two patients died from pulmonary embolism

Abdominal complications following cardiac surgery.

Between 1978 and 1991, 116 of 19,246 patients (0.6%) undergoing cardiac surgery developed abdominal complications (renal/hepatic failure excluded) within 30 days of their cardiac operation. Comparison with a randomly selected control group of 217 patients operated upon over the same period of time was also undertaken. Compared to the control group, the study patients were older (mean age, 63.3 +/- 12.5 years vs 57.5 +/- 21.5 years; P = 0.03), more likely to have a history of alcohol abuse (10% vs 4%; P = 0.03), and more likely to have a previous history of gastrointestinal problems (43% vs 17%

Experience with vagotomy--antrectomy and Roux-en-Y gastrojejunostomy in surgical treatment of duodenal, gastric, and stomal ulcers.

Gastroduodenostomy (Billroth I) is our reconstruction of choice following gastric resection for gastroduodenal ulcer. Dissatisfaction with a Billroth II anastomosis has led us in recent years to employ a Roux-en-Y diversion in selected cases, particularly those in which the pathologic state of the pyloroduodenal canal would render a Billroth I anastomosis unsafe. During the past 7 years, truncal vagotomy-antrectomy and Roux-en-Y (VARY) has been carried out in 50 selected patients: duodenal ulcer (DU) 13 patients, gastric ulcer (GU) 11 patients, and stomal ulcer (SU) 26 patients. Laparoscopic Surgery for Perforated Duodenal Ulcer Disease: Analysis of 70 Consecutive Cases From a Single Surgeon.

BACKGROUND: In the surgical treatment of perforated duodenal ulcer disease, laparoscopic surgery has become a key technique. However, difficult cases are not eligible for laparoscopic surgery. The purpose of this study was to evaluate the risk factors for postoperative complications and to examine the safety and effectiveness of laparoscopic surgery. MATERIALS AND METHODS: Seventy duodenal ulcer perforation patients who underwent laparoscopic surgery between July 2010 and March 2014 were reviewed. The type of surgery was chosen on the basis of the size of the perforation. All patients were cl

Corticotropin-releasing factor. Mechanisms to inhibit gastric acid secretion in conscious dogs.

Immunoreactivity similar to that of corticotropin-releasing factor (CRF) is found in regions of the central nervous system that modulate autonomic responses, including gastrointestinal functions. We examined the central nervous system effects of ovine CRF on gastric acid secretion in conscious dogs. Male beagle dogs (11-13 kg) were fitted with chronic intracerebroventricular cannulae and gastric fistulae. Gastric acid secretion in response to intravenously administered gastric secretory stimuli was measured by in vitro titration of gastric juice to pH 7.0 and in response to an intragastric mea
