Management of the low-flow head and neck vascular malformations in children: the sclerotherapy protocol.

AIM: Image-guided sclerotherapy is becoming the preferred treatment for low-flow vascular malformations in head and neck region. The authors review the management protocol for this condition and evaluate its clinical outcomes. METHODS: Children with low-flow vascular malformations in head and neck region undergoing sclerotherapy from 2010 to 2013 were reviewed. All patients were assessed by pediatric surgeons and interventional radiologists in the multidisciplinary vascular anomalies clinic. Ultrasonography and intravenous contrast enhanced magnetic resonance imaging were performed preoperati

AMPAKINE enhancement of social interaction in the BTBR mouse model of autism.

Autism is a neurodevelopmental disorder in which the first diagnostic symptom is unusual reciprocal social interactions. Approximately half of the children diagnosed with an autism spectrum disorder also have intellectual impairments. General cognitive abilities may be fundamental to many aspects of social cognition. Cognitive enhancers could conceivably be of significant benefit to children and adults with autism. AMPAKINE compounds are a novel class of pharmacological agents that act as positive modulators of AMPA receptors to enhance excitatory glutamatergic neurotransmission. This class of

Effects of pregnanolone and dehydroepiandrosterone on ethanol intake in rats administered ethanol or saline during adolescence.

BACKGROUND: Adolescent alcohol use may contribute to long-term changes in the receptors and neuroactive steroids that may mediate its effects and to subsequent alcohol abuse and dependence as an adult. Therefore, in this study, ethanol preference and intake as an adult were examined after adolescent ethanol or saline administration. In addition, ethanol intake in the same groups was examined after administration of 2 neuroactive steroids with modulatory effects at GABA(A) receptors. METHODS: Two groups of male Long-Evans rats were administered 15 intraperitoneal (i.p.) injections of either et

Early closure of fistula after hypospadias surgery using N-butyl cyanoacrylate: preliminary results.

PURPOSE: We determine the effectiveness of the tissue adhesive n-butyl cyanoacrylate for early repair of fistula after hypospadias surgery in children. MATERIALS AND METHODS: Between February 1999 and December 2000 we evaluated prospectively 8 patients who underwent closure of early urethrocutaneous fistula after hypospadias repair using cyanoacrylate based tissue adhesive at our institutions. After discovery of the fistula parents were instructed to apply triamcinolone cream over the failed repair for 2 to 3 days before fistula closure. Multiple layers of tissue adhesive were applied after r

Elective delayed reduction and no anesthesia: 'minimal intervention management' for gastrochisis.

PURPOSE: In a pilot study of 14 children, born when the authors were on a 1:5 "on take" for neonatal referrals, a policy evolved of elective delayed midgut reduction without anaesthesia or sedation in the incubator on the neonatal surgical unit. There was no other form of selection, and it was fortunate that the authors did not encountered any adverse criteria in this small series. METHODS: Bowel reduction, which was pain free, was undertaken conventionally with the same attention and with no greater difficulty than under general anesthesia. Delaying midgut reduction for more than 4 hours led

Treatment of affective illness in the elderly with drugs and electroconvulsive therapy.

Affective illness is common, frequently debilitating, and sometimes life-threatening in the elderly. Considerations pertaining to treatment with heterocyclic drugs, MAOIs, lithium, psychostimulants and thyroid hormone, as well as ECT, have been reviewed. Amitriptyline and imipramine cause significant orthostatic hypotension and probably should be avoided in the elderly. In addition, amitriptyline is extremely anticholinergic. Amoxapine is essentially a neuroleptic sequelae, including tardive dyskinesia. If a patient has had a prior positive response or has a relative who had a good outcome fro

Phenobarbital during pregnancy in mouse and man.

Phenobarbital appears to produce similar behavioral effects on mice and humans with excitation at low and sedation or depression at higher doses. If plasma concentrations of phenobarbital reflect levels in other tissue, then brain concentrations producing excitation (near 10 micrograms/g) and depression (near 20 micrograms/g) are not substantially different for the two species. The doses needed to produce these levels are much higher in mice than man. Plasma concentrations of phenobarbital decline during pregnancy in humans. Whether this decline is accompanied by increased seizure frequency ha

Effects of surgical repair of congenital diaphragmatic hernia on cerebral hemodynamics evaluated by near-infrared spectroscopy.

BACKGROUND: Cardiorespiratory stabilization is recommended before surgical repair of congenital diaphragmatic hernia (CDH) because surgery may induce a transitory deterioration of chest compliance and gas exchange. It is not known if surgical intervention can affect cerebral circulation and oxygenation. AIM: The aim of the study was to assess noninvasively, by near-infrared spectroscopy, the possible changes in cerebral hemodynamics and oxygenation associated with surgical repair of CDH. SUBJECTS: Twenty-five newborns with severe CDH (birth weight, 3057 +/- 354 g; gestational age, 37.8 +/- 1

Diagnostic and therapeutic ERCP in the pediatric age group.

The role and value of endoscopic retrograde cholangiopancreatography (ERCP) in the pediatric age group is not well established, because pancreatic and biliary diseases are less common in children. This however is not the case in areas like the Eastern Province of Saudi Arabia where sickle cell disease (SCD) and other hemoglobinopathies are common, with increased frequency of cholelithiasis and choledocholithiasis. The purpose of this study was to evaluate the indications, findings, safety and therapies of ERCP in children. One hundred and twenty five children had diagnostic and/or therapeutic

Unsedated transnasal EGD in daily practice: results with 1100 consecutive patients.

BACKGROUND: EGD can be performed transnasally in adults by using small-diameter endoscopes. A large prospective study was conducted to evaluate the feasibility and tolerance of diagnostic transnasal EGD in daily practice. METHODS: Unsedated transnasal EGD was attempted in 1100 consecutive patients, in 3 different institutions, by using a 5.9-mm or a 5.3-mm diameter endoscope. The operator determined whether the procedure was successful or unsuccessful, the reason for failures, and any side effects. The influence of gender, age, endoscope diameter, and type of topical anesthesia on the success

Iliac-femoral venous stenting for lower extremity venous stasis symptoms.

BACKGROUND: Venous outflow obstruction may play a role in patients with chronic venous stasis symptoms who fail to improve despite conventional modalities of treatment that focus on the reflux component of the disease with little attention to the possibility of an obstructive component. The introduction of minimally invasive venous stenting using venography and intravenous ultrasonography (IVUS) provides the ability to treat the "obstructive" component of the disease. METHODS: We undertook a retrospective review of 56 limbs in 53 patients with chronic venous stasis symptoms. Initial transcuta
