Characterization of a muscle-associated antigen from Wuchereria bancrofti.

A recombinant clone, WbN1, isolated from a genomic expression library of Wuchereria bancrofti and showing restricted specificity at the DNA level (Southern and PCR analyses) for Wuchereria bancrofti and Brugia malayi has been previously described. Sequence analysis of WbN1 indicated that it had notable similarity to myosin. Further characterization using in situ hybridization has localized the mRNA in the muscle of the adult parasite and in the microfilariae. Rabbit polyclonal antiserum, raised against the recombinant WbN1 fused to the maltose-binding protein, recognized a 200-kDa polypeptide

Mental health conditions in people affected by filarial lymphoedema in Malawi: prevalence, associated risk factors and the impact of an enhanced self-care intervention.

BACKGROUND: This study aimed to determine the key mental health indicators affecting people affected by lymphatic filariasis (LF) lymphoedema by assessing the prevalence of depressive symptoms and quality of life (QOL), identifying associated sociodemographic and clinical risk factors, and evaluating the impact of an enhanced self-care intervention for lymphoedema management. METHODS: A prospective cohort study of adults with filarial lymphoedema from two regions of Malawi was conducted over six months in 2021. Depressive symptoms and QOL were assessed using Patient Health Questionnaire (PHQ-

Characteristics and advances in signaling pathways, cellular communication, cell junctions, and oxidative stress in lymphedema.

Lymphedema is a chronic inflammatory disease without an effective treatment method, and it results in a high disease burden and psychological distress in patients. Although there have been significant advances in targeted therapies, there are still no effective options to treat this refractory disease. In recent years, new advances and breakthroughs have been made in signaling pathways, including RAS/MAPK, PI3K/AKT, VEGF-C/VEGFR-3, HGF/MET, and TGF-β1, which are important for understanding the pathogenesis and disease progression of lymphedema. Mutations in genes encoding cell junctions affect

Alternative activation of macrophages by filarial nematodes is MyD88-independent.

Alternative macrophage activation is largely defined by IL-4Rα stimulation but the contribution of Toll-like receptor (TLR) signaling to this phenotype is not currently known. We have investigated macrophage activation status under Th2 conditions in the absence of the core TLR adaptor molecule, MyD88. No impairment was observed in the ability of MyD88-deficient bone marrow derived macrophages to produce or express alternative activation markers, including arginase, RELM-α or Ym1, in response to IL-4 treatment in vitro. Further, we observed no difference in the ability of peritoneal exudate cel

Up-regulation of lipophorin (Lp) and lipophorin receptor (LpR) gene in the mosquito, Culex quinquefasciatus (Diptera: Culicidae), infected with the filarial parasite, Wuchereria bancrofti (Spirurida: Onchocercidae).

In mosquitoes, including Culex quinquefasciatus, immune molecules are known to be upregulated or produced de novo upon exposure to parasites or pathogens. These molecules are regulatory in nature acting against parasite or pathogen infection and development. Similarly, there are molecules that are upregulated to facilitate parasite development in the vector mosquitoes. Lipophorin, a major lipid transporting lipoprotein in the hemolymph of insects, is implicated as a helper molecule in the clotting mechanism and facilitator of parasite and pathogen development in mosquitoes. In the present stud

Granuloma formation around filarial larvae triggered by host responses to an excretory/secretory antigen.

In previous studies using a murine model of filarial infection, granuloma formation was found to be a most important host-protective mechanism. We have also shown that in vitro cytoadherence is a surrogate for the formation of antifilarial granulomas in vivo and that it requires "alternatively activated" host cells and a source of antifilarial antibody. We show here that antibodies against L3 excretory/secretory (E/S) products can facilitate in vitro cytoadherence. We generated a set of hybridomas reactive with filarial E/S products and screened them for their ability to mediate in vitro cytoa

Impaired clearance of primary but not secondary Brugia infections in IL-5 deficient mice.

Eosinophilia in blood and tissues has been strongly associated with helminth infections for over a century. In vivo depletion of IL-5, a cytokine crucially involved in eosinophilopoiesis with an antibody or through genetic manipulation, reproducibly abrogates helminth-induced eosinophilia, but renders mice permissive only in some models of parasite infection. In the current study, we compared the ability of IL-5(-/-) and B6(+/+) mice to clear intraperitoneal infections with Brugia pahangi L3. IL-5(-/-) mice had statistically significantly higher worm burdens than B6(+/+). This was true for pri

Interleukin-12 suppresses filaria-induced pulmonary eosinophilia, deposition of major basic protein and airway hyperresponsiveness.

Tropical Pulmonary Eosinophilia (TPE) is a severe form of allergic asthma caused by the host inflammatory response to filarial helminths in the lung microvasculature, and is characterized by pulmonary eosinophilia, increased filarial-specific IgG and IgE antibodies, and airway hyperresponsiveness. The current study examined the effect of IL-12 on pulmonary eosinophilia, deposition of eosinophil major basic protein and airway hyperresponsiveness in mice inoculated i.v. with Brugia malayi microfilariae. Injection of recombinant murine IL-12 modulated the T helper (Th) response in the lungs from

Strengthening access to people-centred mental healthcare for people affected by skin neglected tropical diseases in Nigeria: a pilot study.

OBJECTIVES: Neglected tropical diseases (NTDs) contribute to the global burden of mental health conditions, largely mediated through mechanisms of stigma and social exclusion. Within this group, skin NTDs are particularly prominent, as visible deformities often drive stigma reinforced by cultural myths, fear of contagion, social isolation, and associated poverty. The WHO recommends a collaborative cross-sectoral approach in its NTD Road Map 2021-30, integrating care for people with NTDs. This study was conducted as a pilot to assess the feasibility, acceptability, and impact of a person-centre

Establishing a filarial clinical research platform in a resource-limited setting: Lessons and experiences from Tanzania.

This paper examines the establishment and lessons learned from the LeDoxy Trial, a multinational, double-blind, randomized, placebo-controlled study that evaluated doxycycline (200 mg/d and 100 mg/d) over six weeks for the treatment of filarial lymphedema in Tanzania's Lindi and Pwani regions. Conducted from 2018 to 2021 across four sites, the trial enrolled 420 participants aged 14-65 years, addressing a critical gap in lymphatic filariasis (LF) research in geographically remote and historically under-researched areas. The trial faced significant challenges, including limited pre-existing res

Declining Onchocerca volvulus transmission despite limited ivermectin delivery in the Kakoi-Koda focus, Ituri, Democratic Republic of the Congo: An epidemiological, entomological and landscape evidence synthesis.

BACKGROUND: The Kakoi-Koda focus in Ituri Province was historically hyperendemic for onchocerciasis, yet screening for a recent trial suggested a marked decline, including in Logo Health Zone, which had never received routine ivermectin. We explored whether this decline extended across the focus and how infection indicators corresponded spatially with ivermectin delivery, entomological observations and deforestation. METHODOLOGY: We conducted a scoping evidence synthesis of epidemiological, programmatic, entomological and geospatial sources. Against a 2003 nodule-mapping baseline, change was

Assessment of mosquito species communities biting humans and their livestock in the forest hills of Karen state, Myanmar: a cross-sectional survey in six villages.

BACKGROUND: Mosquito-borne diseases cause significant burdens in rural areas of Southeast Asia. The lack of data on vector bionomics hinders disease control and elimination. The objectives of this study were to assess the diversity and biting behaviours of mosquito species biting humans and their livestock in the forest hills of Karen state, Myanmar, and to assess the patterns of species co-occurrence and the effects of the environment on vector abundance. METHODS: Mosquitoes were captured over 24-h diel cycles in six villages in September 2019 using the human landing catch and cow-baited tra

Chemical composition and mosquitocidal activity of methanolic leaf extracts from Ziziphus mauritiana L. against Aedes aegypti L. and Culex quinquefasciatus Say.

BACKGROUND OBJECTIVES: Mosquito vectors are a significant cause of pathogen transmission to both animals and humans. Mosquitoes are responsible for transmitting various diseases, including chikungunya, filariasis, West Nile fever, dengue, malaria, and Zika fever, placing approximately 80% of the world's population at risk of infection at any given time. Controlling mosquitoes has become a multifaceted challenge, influenced by diverse environmental factors such as deforestation, urbanization, and climate change on a global scale. For centuries, medicinal plants have been utilized as bioinsectic

The psychosocial impacts of skin-neglected tropical diseases (SNTDs) as perceived by the affected persons: A systematic review.

BACKGROUND: Neglected Tropical Diseases (NTDs) disproportionately affect marginalised groups within impoverished communities, conferring devastating physical, financial and psychosocial effects. Skin-NTDs (SNTDs) are uniquely stigmatising due to their visible nature, rendering affected individuals vulnerable to psychosocial risk and the associated decline in social participation, quality of life and mental health. In response to knowledge gaps identified by current global efforts for integrated control of SNTDs this review gathers existing evidence on the psychosocial effects of SNTDs, with co

Factors influencing mass drug administration adherence and community drug distributor opportunity costs in Liberia: a mixed-methods approach.

BACKGROUND: Preventive chemotherapy delivered via mass drug administration (MDA) is essential for the control of neglected tropical diseases (NTDs), including lymphatic filariasis (LF), schistosomiasis and onchocerciasis. Successful MDA relies heavily on community drug distributor (CDD) volunteers as the interface between households and the health system. This study sought to document and analyse demand-side (households) and supply-side (health system) factors that affect MDA delivery in Liberia. METHODS: Working in two purposively selected counties, we conducted a household MDA access and ad

Complex emergencies and the control and elimination of neglected tropical diseases in Africa: developing a practical approach for implementing safe and effective mapping and intervention strategies.

BACKGROUND: Complex emergencies resulting from conflict and political instability are a major challenge for national neglected tropical diseases (NTDs) control and elimination programmes, especially in sub-Saharan Africa. Currently, there are no formal guidelines for national programmes to use and plan activities in these humanitarian situations, therefore the aim of this study was to develop a new methodological approach for making decisions about the implementation of safe and effective mapping and mass drug administration (MDA) intervention strategies. METHODS: The study focussed on the 47

Pharmacokinetics, safety, and efficacy of a single co-administered dose of diethylcarbamazine, albendazole and ivermectin in adults with and without Wuchereria bancrofti infection in Côte d'Ivoire.

BACKGROUND: A single co-administered dose of ivermectin (IVM) plus diethylcarbamazine (DEC) plus albendazole (ALB), or triple-drug therapy, was recently found to be more effective for clearing microfilariae (Mf) than standard DEC plus ALB currently used for mass drug administration programs for lymphatic filariasis (LF) outside of sub-Saharan Africa. Triple-drug therapy has not been previously tested in LF-uninfected individuals from Africa. This study evaluated the pharmacokinetics (PK), safety, and efficacy of triple-drug therapy in people with and without Wuchereria bancrofti infection in W
