The different sonographic features of the twisted testis, which are time-related, are reported and stressed. Spermatic cord torsion happens rather frequently in young men. It must be discovered early on, to allow for the possibility of removal by surgery. It appears from recently written medical articles that the endocrine function of the testis is protected following several hours of torsion and the risk of immunological damage to the contralateral testis is subsequently reduced.

The incidence of renal cell carcinoma with a vena caval tumour thrombus has been reported in the literature, form 4% to 19%. Vena caval involvement causes serious diagnostic and therapeutic problems. High frequency probes for transrectal sonography are well tolerated and permit us to study the cervico-urethral unit carefully. We are allowed to single out some rare causes of obstruction, otherwise difficult to diagnose. Bladder neck obstruction secondary to a cyst is one of these. Our case report is about a young patient with complete retention secondary to bladder cyst discovered by transrectal sonography.

Diverticula of the urethral wall are not common. Generally, in the male, congenital urethral diverticulum is located in the anterior urethra. We report a twenty-one year old man who referred to our department for dysuria, micturition in two times, small stream urination. Sometimes he also presented a dry ejaculation. The exams showed, at the bulbous portion of the urethra, an urethral diverticulum. The diverticulum was probably congenital because the patient had no history of urinary infection, catheterization or trauma. A surgery procedure was performed. 9 months later, the result is good.

A bloodless study with a Photoplethysmographic test of the limb's blood circulation among haemodialyzed patients with arteriovenous fistula and the influence of Flunarizine treatment was performed. The Authors illustrate a case of tubercular stricture of the pelvic ureter, successfully treated, with the insertion of endo-ureteral prosthesis (double J) and with anti-mycobacterial therapy. They underline the increasing of incidence of the urinary t.b.c. (compared with the pulmonary) particularly at ureteral level. They confirm the efficiency of the ureteral catheterisation as therapy of the strictures, especially in the initial stages of the disease. They explain that this treatment can have only a temporary effect but can give time for a successive restoring surgical therapy.

The choice of the tissue to build up the neovagina is an important step in the surgical treatment of transsexualism. Both the penile skin and an intestinal segment are possible solutions to the problem. However intestinal segments have some defects like continuous secretions and painful intercourse. For these reasons the authors prefer the penile skin with the neuro-vascular bundle preserved. One hundred cases of this surgery are presented.

The surgery for both male and female transsexuals causes great difference in the behavior of operated patients. Then it is very important to analyze the variations of three parameters before and after operation: 1) the presence and the character of sexual feeling and the modality of sexual intercourse; 2) possibility of orgasm; 3) working behavior. The authors present the results of their study on 16 male transsexuals and 4 female transsexuals.

The authors analyze some medico-legal aspects concerning the surgical therapy of transsexualism, as it is regulated by the Law N. 442/1982. At first they examine the problems related to the verification of the indications for the surgical treatment, with particular reference to nosography, differential diagnosis and clinical criterions suitable to evaluate the usefulness of this intervention. Afterward they analyze the problem of the validity of the consent, considering the particular kind of treatment and the psychological connotations of these patients.
