[Erythrocytosis due to a high-affinity hemoglobulin: mutant hemoglobin Saint-Jacques beta 140 (H18) Ala----Thr with a change in the 2,3-diphosphoglycerate binding site].

All the high oxygen affinity variants have substitutions in regions that are crucial to hemoglobin function: mainly the alpha 1 beta 2 interface, the C-terminal end of the beta chain and the aminoacid residues involved in the 2,3 disphophoglycerate (2,3 DPG) binding site. In this report we describe a new variant with familial erythrocytosis: hemoglobin Saint-Jacques beta 140 (H18) Ala----Thr, whose main abnormality is a defect in organic phosphate binding in the central cavity between to two beta chains. This variant could not be separated from hemoglobin A by standard electrophoretic methods

Variation induced by a three-days application of the artificial beta-cell on glucagon, growth hormone and catecholamine secretion, glycosylated hemoglobin, and erythrocyte 2,3-diphosphoglycerate concentration.

Five insulin-dependent diabetics with poor metabolic control were examined at the beginning and after a three-day application of artificial endocrine pancreas (AEP). Pancreatic alpha cell function evaluated by arginine infusion (0.5 g/Kg b.w. over 30 minutes) showed no significant differences between the beginning and during artificial beta cell aplication, but the increment in plasma glucagon level over basal values observed in both tests appeared significantly higher at 30 and 60 min in comparison with a control group. Growth hormone response to arginine infusion was clearly reduced in the s

Phosphorus-31 nuclear magnetic resonance studies of human red blood cells.

Phosphorus-31 nuclear magnetic resonance spectroscopy is used to monitor the changes in the concentrations of 2,3-diphosphoglycerate and adenosine-5' triphosphate as a function of time and to measure the intracellular pH of normal and abnormal red blood cells in the presence of 25% oxygen, 5.6% carbon dioxide, and 69.4% nitrogen at 37 degrees C. Under these conditions, the intracellular pH values of normal AA, sickle SS, AS, SC, AC, and CC red cells are 7.24 +/- 0.07, 7.13 +/- 0.04, 7.15 +/- 0.03, 7.16 +/- 0.03, 7.24 +/- 0.05, and 7.14 +/- 0.03, respectively. The intracellular pH of SS red cel

Red blood cell alkalosis and decreased oxyhemoglobin affinity.

The behavior of the oxyhemoglobin curve was studied in ten patients with respiratory alkalosis (arterial [H+] less than 37 nM, pCO2 less than 32 mmHg and HCO-3 less than 22.0 mEq/L) and ten patients with metabolic alkalosis ([H+] less than 34 nM, pCO2 greater than 37 mmHg and HCO-3 greater than 28.0 mEq/L) to determine whether different alkalotic states similarly affect the red blood cell [H+] and 2,3-diphosphoglycerate interaction and thus the oxygen affinity of hemoglobin. The findings were statistically indistinguishable in respiratory alkalosis and metabolic alkalosis: a) low plasma [H+],

In vitro effects of pentoxifylline on hemoglobin affinity for oxygen and electrolytic equilibrium of human blood.

The in vitro effects of a pentoxifylline solution on hemoglobin-oxygen affinity and electrolytic distribution between plasma and red blood cells were evaluated. Pentoxifylline increased intracellular pH and decreased extracellular pH. There were no exchanges of Na+, K+, Ca++ and Cl- between plasma and red blood cells. Red blood cell content of 2,3-diphosphoglycerate remained unchanged, while P50 was reduced as a consequence of the intracellular alkalosis. The possible implications of the metabolic effects of pentoxifylline on red blood cell deformability and 2,3-diphosphoglycerate content in v

The first case of a complete deficiency of diphosphoglycerate mutase in human erythrocytes.

An inherited and complete deficiency of diphosphoglycerate mutase was discovered in the erythrocytes of a 42-yr-old man of French origin whose blood hemoglobin concentration was 19.0 g/dl. Upon physical examination he was normal with the exception of a ruddy cyanosis. The morphology of his erythrocytes was also normal and there was no evidence of hemolysis. The erythrocyte 2,3-diphosphoglycerate level was below 3% of normal values and, as a consequence, the affinity of the cells for oxygen was increased. Diphosphoglycerate mutase activity was undetectable in erythrocytes as was that of diphosp

Fetal oxygen affinity and its parameters in a random obstetric population.

Fetal oxygenation studies have provided the obstetrician with some basic information regarding oxygen supply to the fetus. Although standard values for normal fetuses at term have been made, little has been done to evaluate any changes occurring with fetal and neonatal abnormality. Twenty-two sets of analyses were performed to measure oxygen affinity (P50) 2,3-diphosphoglycerate concentration and full blood gas analysis on umbilical arterial and venous samples. The samples were taken from a random obstetric population. All babies were at term or post term by antenatal data and paediatric asses

[Adaptation to hypoxia in liver cirrhosis: the role of 2,3-DPG (author's transl)].

By observing a group of 20 patients with liver cirrhosis, we have clarified some features concerning the tissue hypoxia, which is often present in such a disease. By determining the levels of the haemoglobin and of the intraerythrocytic 2,3-DPG, and by evaluating the acid-base state of such patients, we have emphasized the increased output of the 2,3-DPG as mechanism of adaptation to hypoxia associated with hepatic cirrhosis, both in subjects with anemia and alkalosis and in subjects without anemia but with alkalosis.

Phosphate compounds in the erythrocytes of newborns with ABO systems haemolytic disease.

The following phosphate compounds of the erythrocyte acid-soluble fraction were subjected to chromatographic separation: ADP, ATP, adenylo-diphosphoglycerate, 2,3-diphosphoglycerate, hexose monophosphate, hexose diphosphate. In each of the fractions total phosphorus, and in fraction II inorganic phosphorus, were estimated. The material was derived from ten newborns with haemolytic disease as a result of ABO incompatability and from ten full-term healthy newborns, just after birth. The concentration of the compounds assayed, except for 2,3-DPG (the values in both groups were similar) was higher

The lack of effects of phototherapy on neonatal oxygen dissociation curves and hemoglobin concentration in vivo.

Phototherapy has been shown to cause hemolysis of fetal red cells and a shift to the right in the neonatal oxygen dissociation curve (deltaP50) in vitro. To determine if these parameters act similarly in vivo, we have studied 16 icteric infants before and after phototherapy and compared them with eight control infants studied at birth and at 3 days of age, measuring the change in P50, 2,3-diphosphoglycerate (2,3-DPG), serum bilirubin, the percent of bilirubin/albumin saturation, and the hemoglobin concentration in the two groups. Following phototherapy, in the icteric infants there was a shift

Oxygen affinity of erythrocytes and pulmonary gas exchange in patients on continuous ambulatory peritoneal dialysis.

In 11 patients on continuous ambulatory peritoneal dialysis 2,3-diphosphoglycerate, oxygen (O2) affinity of red cells (P50), blood gases, ventilation, and O2 consumption were studied. Contrary to patients on maintenance hemodialysis, 2,3-diphosphoglycerate and P50 were normal in patients on continuous ambulatory peritoneal dialysis; they were correlated with each other. Arterial O2 pressure was normal; under the conditions of dialysis with 35 mmol lactate per liter dialysis fluid a slight metabolic acidosis persisted; it was combined with a moderate respiratory alkalosis. Hemolytic anemias and erythrocyte enzymopathies.

The human erythrocyte generates high-energy adenosine triphosphate by anaerobic glycolysis and cycles oxidized and reduced nicotinamide adenine dinucleotide phosphate by the aerobic pentose phosphate shunt pathway. Certain enzymopathies of the pentose phosphate shunt are associated with hemolysis resulting from oxidative denaturation of hemoglobin. Glucose-6-phosphate dehydrogenase deficiency, an X-chromosome-linked disorder, is the prototype of these diseases and is genetically and clinically polymorphic. Six enzymopathies of anaerobic glycolysis cause hemolytic anemia; lactate dehydrogenase

Hypophosphatemia. Causes and clinical consequences.

Severe hypophosphatemia (i.e., serum phosphorus concentration below 1 mg/dl) occurs infrequently in veterinary patients. It is most often associated with diabetic ketoacidosis in small animals. Phosphate is necessary for the production of 2,3 diphosphoglycerate (2,3-DPG) and adenosine triphosphate (ATP); both are important for normal cellular metabolism. Consequences of severe hypophosphatemia may include hemolytic anemia, seizures, altered mentation, cardiomyopathy, and skeletal muscle weakness. Parenteral phosphate therapy is necessary in most cases of severe hypophosphatemia.

A procedure for decreasing the level of 2,3-bisphosphoglycerate in red cells in vitro.

The physiological adaptation to anemia and other hypoxic states includes an increase in the level of 2,3-bisphosphoglycerate (2,3-DPG) in the red cell. We suggest that the high level of 2,3-DPG may have adverse effects in vivo. It has been found that red cells incubated with glycolate lose 2,3-DPG at a rapid rate relative to controls. ATP is stable. Net 2,3-DPG synthesis is observed after the glycolate is removed from the cells suggesting that they are not harmed. The effect appears to be specific for glycolate since lactate, glyoxylate, glycerate, acetate, and citrate were without effect. Thi

Effects of 4000 rad irradiation on the in vitro storage properties of packed red cells.

Immunosuppressed patients who require red cell transfusions receive irradiated (1500-3000 rad) packed red cells. These cells are irradiated immediately before infusion. If a large group of patients become immunosuppressed due to exposure to radiation or chemicals, the ability to supply large volumes of irradiated blood at the time of use might not be possible. An alternate solution to providing quantities of irradiated blood is to irradiate the units prior to storage. This study presents in vitro data comparing storage of paired packed red cell units either irradiated or not irradiated. Five u

Erythrocyte organic phosphates in the anemia of renal failure in childhood.

Erythrocyte 2,3-diphosphoglycerate (2,3-DPG ) and adenosinetriphosphate (ATP) levels were determined in 43 children with chronic renal failure on conservative treatment (CT), and 12 children on regular hemodialysis (HD) immediately before and after a HD session. The results were compared to non-anemic and anemic controls. In spite of anemia, erythrocyte 2,3-DPG in renal failure was similar to non-anemic controls at normal blood pH, but rose during dialysis as a result of alkalosis. In contrast, ATP levels were high already at a normal blood pH. 2,3-DPG correlated with packed cell volume (PCV)

"Benign erythrocytosis" and other familial and congenital polycythemias.

The term familial and congenital polycythemia encompasses a heterogeneous group of disorders with the common characteristic of an absolute increased red cell mass since birth and/or similar phenotype also present in relatives. In the last 2 decades the differential diagnosis between primary and secondary familial polycythemias became more physiologically relevant as new sensitive techniques, such as accurate measurements of serum erythropoietin (S-EPO) concentration by radioimmunoassay (RIA) or ELISA, and assessment of growth of erythroid progenitor cells in vitro became available. Consequentl

[Oxygen transport function of the blood in acute disorders of gas metabolism and of the acid-base state].

The oxygen-transport function of blood in hyperoxia and acute disturbances of the acid-alkali condition was studied in "oxygenator--desoxygenator" as a model. It has been established that in acidosis there occurs decreased intensity of glycolysis in erythrocytes, accumulation of 2,3 diphosphoglyceric acid which decreases affinity of hemoglobin for oxygen. It does not occur in alcalosis.
