Detailed layer-by-layer autopsy of the head and neck was performed on a series of seventy-three fatally injured motorcyclists in order to identify occult soft tissue injuries such as subluxation of the cervical spine or hemorrhage of vertebral or internal carotid arteries. The fatal cases were gathered as part of a larger study of 1082 on-scene in-depth motorcycle crash investigations in Thailand. Injuries were coded using the 1990 Abbreviated Injury Scale (AIS 90) and an Injury Severity Score (ISS) was determined for each case. Twenty-eight motorcyclists had been wearing a helmet at the start

OBJECTIVE: This is a descriptive study of the frequency and risk for brainstem injury by crash type, belt use, and crash severity (delta-V). NASS-CDS electronic cases were reviewed to see whether the transition from vehicles without advanced airbags and seat belts and side airbags and curtains to vehicles with the safety technologies has influenced the risk for brainstem injury.
METHODS: 1994-2013 NASS-CDS was analyzed to determine the number of brainstem injuries in nonejected adults (15+ years old) in vehicle crashes. Crashes were grouped by front, side, rear, and rollover. The effect of be

BACKGROUND: Despite the use of prehospital triage algorithms and other trauma scoring methods, a substantial proportion of older patients with life-threatening injuries are undertriaged, increasing the risk of preventable death and disability.
STUDY OBJECTIVE: The primary objective of this study was to describe the injury types, injury severity, and short-term survival outcomes of undertriaged older adults injured in motor vehicle collisions, compared to a group of correctly triaged older adults.
METHODS: This secondary analysis of records extracted from the National Automotive Sampling Syst

OBJECTIVE: Several studies have documented the relative risk or odds of injury and fatality for females versus males in motor vehicle crashes (Parenteau et al. 2013, Forman et al. 2019, Brumbelow and Jermakian, 2022; Noh et al. 2022). Though, none combined National Automotive Sampling System-Crashworthiness Data System (NASS-CDS) and Crash Investigation Sampling System (CISS). The aim of this study was to document the relative odds of various injury outcomes for females versus males while considering a broad range of crash types, pre-crash and crash variables, and occupant characteristics.

INTRODUCTION: Management of hemorrhage from pelvic fractures is complex and requires multidisciplinary attention. Pelvic angioembolization (AE) has become a key intervention to aid in obtaining definitive hemorrhage control. We hypothesized that pelvic AE would be associated with an increased risk of venous thromboembolism (VTE).
METHODS: All adults (age >16) with a severe pelvic fracture (Abbreviated Injury Scale ≥ 4) secondary to a blunt traumatic mechanism in the 2017-2019 American College of Surgeons Trauma Quality Improvement Program database were included. Patients who did not receive V

The prognosis of fracture healing depends upon the severity of injury sustained to the soft tissues and the blood supply left to the bone. The soft tissue injury cannot be precisely graded until after the fracture has been stabilized and radical debridement performed. Subsequent steps in the therapeutic strategy of compound fractures are determined mainly by the type and severity of soft tissue injury. Therefore fractures with soft tissue lesions need to be classified again after bone fixation and wound revision. Later treatment steps for restoration of the soft tissues are easily derived from

OBJECTIVES: To describe fatal pedestrian injury patterns in adults 25-64 years old and correlate them with motor vehicle collision (MVC) dynamics and pedestrian kinematics using medicolegal death investigations data of MVCs occurring in the current Canadian MV fleet.
METHODS: MVC-pedestrian injuries were collated in an Injury Data Collection Form (IDCF) and coded using the Abbreviated Injury Scale (AIS) 2015 revision. The AIS of the most frequent severe injury was noted for individual body regions. The Maximum AIS (MAIS) was used to define the most severe injury to the body overall and by bod

Lower extremity injuries are among the most common injuries sustained by motorcycle riders in crashes and often lead to extended and costly medical treatment and permanent disability. This study characterizes lower extremity injuries in a group of 700 motorcycle riders in crashes in Los Angeles County from July 1, 1988, through October 31, 1989. Motorcycle crash fatalities (n = 163) were identified through the Los Angeles County Coroner's office, and nonfatally injured riders (n = 537) were identified at four of the ten level I and level II trauma centers in the county. Lower extremity injurie

BACKGROUND: Optimized conservative treatment of rib fractures has long been practiced, but surgical fixation has not been promising until recently. We aimed to examine and analyze immediate postoperative outcomes and 6-month quality of life after injury in patients with moderately severe traumatic rib fractures.
METHODS: We conducted a prospective cohort study between July 2017 and June 2019 at the National Taiwan University Hospital. Seventy-two patients with moderately severe thoracic trauma were enrolled; 38 received conservative treatment and 34 underwent surgical fixation. Quality of lif

Lower extremity injuries resulting from motor vehicle crashes are common and have become relatively more important as more drivers with newer occupant restraints survive high-energy crashes. CIREN data provide a greater level of clinical detail based on coding guidelines from the Orthopedic Trauma Association. These detailed data, in conjunction with long-term follow-up data obtained from patient interviews, reveal that the most costly and disabling injuries are those involving articular (joint) surfaces, especially those of the ankle/foot. Patients with such injuries exhibit residual physical

PURPOSE: Severe traumatic brain injury (TBI) is a leading cause of death and disability. Approximately one-third to one-half of cases of TBI are associated with severe extracranial injuries, particularly abdominal trauma. This combination is associated with high mortality rates. However, studies describing the outcomes and prognostic factors of patients with combined severe head and abdominal trauma are lacking. Therefore, this study aimed to compare the clinical characteristics and outcomes and identify prognostic factors in this patient population.
METHODS: This single-centre, retrospective

BACKGROUND: The Abbreviated Injury Scale (AIS) and Injury Severity Score (ISS) are widely used to assess trauma patients. In this study, the interobserver variability of the injury severity assessment for severely injured patients was analyzed based on different injured anatomical regions, and the various demographic backgrounds of the observers.
METHODS: A standardized questionnaire was presented to surgical experts and participants of clinical polytrauma courses. It contained medical information and initial X-rays/CT-scans of 10 cases of severely injured patients. Participants estimated the

ABSTRACT A retrospective analysis of the NASS/CDS database from 1993 to 2000 was used to investigate lower extremity injury in lateral impact. The analysis includes the study of the injury patterns, crash characteristics and the interactions between the occupant and the vehicle interior, including injuries to the farside occupants. The findings include significantly different injury patterns for the nearside and farside impacts. In particular, while the proportion of pelvis/hip injuries, with respect to AIS2 and AIS3 lower extremity skeletal injuries and 2-4 and 10-8 o'clock side impacts, was

Foot-ankle injuries have increased in relative importance in recent years. As a basis for future countermeasures, an epidemiology study has been undertaken using Swedish accident data from Folksam Insurance. The database consists of 805 foot-ankle injuries out of 57,949 car occupant injuries reported from 1985 to 1991. The influence of crash location, seating position and occupant age is determined for the frequency, incidence and rate of foot-ankle injury in car crashes. Frontal car crashes produce 76% of the AIS 2-3 foot-ankle injuries with 13% in side impacts and 8% in roll-overs. The rate

BACKGROUND: One-third of major burns are complicated by burns-related smoke inhalation injury (BII), which increases mortality risk. Fibreoptic bronchoscopy is the gold standard for BII diagnosis, with the Abbreviated Injury Score (AIS) being the most widely used grading system. While BII severity has been linked to poorer outcomes, this is the first systematic review and meta-analysis evaluating the association between bronchoscopic grading of BII and mortality, pneumonia, and acute respiratory distress syndrome (ARDS).
METHODS: Primary human clinical studies assessing BII severity via bronc

Motorcyclists are vulnerable road users at a particularly high risk of serious injury or death when involved in a crash. In order to evaluate key risk factors in motorcycle crashes, this study quantifies how different "policy-sensitive" factors correlate with injury severity, while controlling for rider and crash specific factors as well as other observed/unobserved factors. The study analyzes data from 321 motorcycle injury crashes from a comprehensive US DOT FHWA's Motorcycle Crash Causation Study (MCCS). These were all non-fatal injury crashes that are representative of the vast majority (8

The National Highway Traffic Safety Administration (NHTSA) recently published a Request for Comments regarding a potential upgrade to the US New Car Assessment Program (US NCAP) - a star-rating program pertaining to vehicle crashworthiness. Therein, NHTSA (a) cited two metrics for assessing head risk: Head Injury Criterion (HIC15) and Brain Injury Criterion (BrIC), and (b) proposed to conduct risk assessment via its risk curves for those metrics, but did not prescribe a specific method for applying them. Recent studies, however, have indicated that the NHTSA risk curves for BrIC significantly
