Coordinated Care Organizations and mortality among low-income infants in Oregon.

OBJECTIVE: To examine the impact of Oregon's Coordinated Care Organizations (CCOs), an accountable care model for Oregon Medicaid enrollees implemented in 2012, on neonatal and infant mortality. DATA SOURCES: Oregon birth certificates linked with death certificates, and Medicaid/CCO enrollment files for years 2008-2016. STUDY DESIGN: The sample consisted of the pre-CCO birth cohort of 135 753 infants (August 2008-July 2011) and the post-CCO birth cohort of 148 650 infants (August 2012-December 2015). We used a difference-in-differences probit model to estimate the difference in mortality bet

Artificial Intelligence as a Catalyst for Value-Based Health Insurance in the United States: Narrative Review and Policy Perspective.

The United States health insurance system is at a critical crossroads. Inflating costs, fragmented care, and administrative inefficiencies have revealed the limitations of the Fee-for-Service (FFS) model. This long-standing structure, while once effective in expanding access, now struggles to deliver efficiency and value. Value-based care (VBC) aims to realign incentives toward outcomes, quality, and efficiency. This article explores how artificial intelligence (AI) can serve as the digital backbone to accelerate the transition from FFS to VBC. The article reviews evidence from bundled payment

A narrative account of the key drivers in the development of the Learning from Deaths policy.

OBJECTIVE: In recent years there has been a proliferation of patient safety policies in the United Kingdom triggered by well publicized failures in health care. The Learning from Deaths (LfD) policy was implemented in response to failures at Southern Health National Health Service (NHS) Foundation Trust. This study aims to develop a narrative to enable the understanding of the key drivers involved in its evolution and implications for future national patient safety policy development. METHODS: A qualitative study was undertaken using documentary analysis and semi-structured interviews (n = 12

Ensuring Equitable Application of Interventions to Vulnerable Subpopulations in the Kentucky Consortium for Accountable Health Communities (KC-AHC).

INTRODUCTION: The Centers for Medicare and Medicaid Services (CMS) has funded the Accountable Health Communities (AHC) model to test whether systematically identifying and addressing the health-related social needs (HRSNs) of individuals would impact healthcare utilization and total cost of care for Medicare and Medicaid beneficiaries. Toward this effort, AHCs implement screening, referral, and community navigation services in their local areas. There are 28 CMS-funded AHCs nationwide, including the Kentucky Consortium for Accountable Health Communities (KC-AHC). PURPSOE: This study aims to a

Effects of Global Payment and Accountable Care on Tobacco Cessation Service Use: An Observational Study.

BACKGROUND: Tobacco use is the leading cause of preventable death and disability. New payment and delivery system models including global payment and accountable care have the potential to increase use of cost-effective tobacco cessation services. OBJECTIVE: To examine how the Alternative Quality Contract (AQC) established in 2009 by Blue Cross Blue Shield of Massachusetts (BCBSMA) has affected tobacco cessation service use. DESIGN: We used 2006-2011 BCBSMA claims and enrollment data to compare adults 18-64 years in AQC provider organizations to adults in non-AQC provider organizations. We e

Health system-community partnerships in a Medicaid nutrition and housing program, 2020-2023: a mixed-methods evaluation.

INTRODUCTION: Health systems are partnering with social service organizations (SSOs) to establish health-related social needs (HRSN) programs, yet best practices have not been established. METHODS: This study assessed the implementation of a partnership between one accountable care organization (ACO) and SSOs in Massachusetts' Medicaid Flexible Services Program (FSP) from 2020 to 2023 to address food and housing insecurity. To be FSP-eligible, individuals needed to be in the ACO, have food or housing insecurity, and meet specific health criteria. Quantitative measures of reach, enrollment, an

Postpartum Linkage to Primary Care Among Patients With Diabetes and Hypertensive Disorders of Pregnancy Following Medicaid ACO Implementation.

BACKGROUND: Timely postpartum linkage to primary care is important for managing diabetes and hypertensive disorders of pregnancy (HDP), yet linkage remains suboptimal. Medicaid accountable care organizations (ACOs) aim to strengthen care coordination and improve postpartum linkage, although effects may vary by ACO design. OBJECTIVE: Evaluate whether Massachusetts Medicaid ACO implementation was associated with changes in postpartum primary care linkage among enrollees with diabetes or HDP and whether effects differed by ACO model. DESIGN: Retrospective cohort study using the 2016-2023 Massac

Goal alignment and unintended consequences of accountable care: How the structure of Oregon's Medicaid coordinated care model shapes health plan-clinic partnerships.

INTRODUCTION: Accountable care models for Medicaid reimbursement aim to improve care quality and reduce costs by linking payments to performance. Oregon's coordinated care organizations (CCOs) assume financial responsibility for their members and are incentivized to help clinics improve performance on specific quality metrics. This study explores how Oregon's CCO model influences partnerships between payers and primary care clinics, focusing on strategies used to enhance screening and treatment for unhealthy alcohol use (UAU). METHODS: In this qualitative study, we conducted semi-structured i

Supporting Primary Care for Medically and Socially Complex Patients in Medicaid Managed Care.

IMPORTANCE: In 2023, the Massachusetts Medicaid and Children's Health Insurance Program (MassHealth) required accountable care organizations (ACOs) to increase payments to primary care practices and shift to monthly payments, currently calibrated to historical revenues and enhanced practice capabilities, such as being staffed to address behavioral health needs. To prevent rewarding practices for avoiding difficult patients, future payments to primary care practices should reflect their patients' apparent need. OBJECTIVE: To describe MassHealth's initiative and a complexity-adjusted payment mo

Medicaid ACO Implementation and Pediatric Asthma Care Equity.

OBJECTIVES: Poorly controlled asthma disproportionately impacts children from lower-income and racially or ethnically minority groups. We examined whether Massachusetts Medicaid Accountable Care Organization (ACO) implementation was associated with improved pediatric asthma care and outcomes and whether associations varied by race and ethnicity. METHODS: Using a difference-in-differences approach, we compared pre-implementation (2016-2017) to post-implementation (2018-2021) outcomes for Medicaid-insured children assigned to ACOs to those not assigned to ACOs. We examined differences overall a

A cluster-based approach for integrating clinical management of Medicare beneficiaries with multiple chronic conditions.

BACKGROUND: Approximately 28% of adults have ≥3 chronic conditions (CCs), accounting for two-thirds of U.S. healthcare costs, and often having suboptimal outcomes. Despite Institute of Medicine recommendations in 2001 to integrate guidelines for multiple CCs, progress is minimal. The vast number of unique combinations of CCs may limit progress. METHODS AND FINDINGS: To determine whether major CCs segregate differentially in limited groups, electronic health record and Medicare paid claims data were examined in one accountable care organization with 44,645 Medicare beneficiaries continuously e

Integrated, Accountable Care For Medicaid Expansion Enrollees: A Comparative Evaluation of Hennepin Health.

Hennepin Health, a Medicaid accountable care organization, began serving early expansion enrollees (very low-income childless adults) in 2012. It uses an integrated care model to address social and behavioral needs. We compared health care utilization in Hennepin Health with other Medicaid managed care in the same area from 2012 to 2014, controlling for demographics, chronic conditions, and enrollment patterns. Homelessness and substance use were higher in Hennepin Health. Overall adjusted results showed Hennepin Health had 52% more emergency department visits and 11% more primary care visits

Asynchronous vascular consultation via electronic methods: A feasibility pilot.

Management of chronic disease often requires multidisciplinary clinical efforts and specialist care. With the emergence of Accountable Care Organizations (ACOs), health care systems are incentivized to evaluate methods of information exchange between generalists and specialists in order to provide value while preserving quality. Our objective was to evaluate patient and referring provider satisfaction and outcomes of asynchronous electronic consultations in vascular care in a large tertiary academic medical center. Referring providers were offered a vascular 'e-consult' option through an elect

A New Era of National Guideline Development in Saudi Arabia.

Saudi Arabia's ambitious Vision 2030 project was launched in 2016 as a strategy for economic development and national growth, with 11 Vision Realization Programs put in charge of its implementation. The backbone of its Transformation Program for the Health Sector has been the definition of a new Model of Care aiming to deliver 42 coordinated interventions across 6 Systems of Care, with the development of clinical guidelines identified as a key cross-cutting intervention to foster the use of national, evidence-based practices across KSA, reduce care variation, and promote accountable care. The Association of Frailty and Neighborhood Disadvantage with Emergency Department Visits and Hospitalizations in Older Adults.

BACKGROUND: Risk stratification and population management strategies are critical for providing effective and equitable care for the growing population of older adults in the USA. Both frailty and neighborhood disadvantage are constructs that independently identify populations with higher healthcare utilization and risk of adverse outcomes. OBJECTIVE: To examine the joint association of these factors on acute healthcare utilization using two pragmatic measures based on structured data available in the electronic health record (EHR). DESIGN: In this retrospective observational study, we used

Health access, health disparities, and Medicare's transforming episode accountability model (TEAM) in the field of spine surgery.

In the past, many health reform efforts have been touted as a means to tangentially reduce healthcare disparities. Few have shown any demonstrable efficacy in this arena. There is reasonable concern that the machinations of Medicare's Transforming Episode Accountability Model (TEAM) may also exert unintended effects on health access and delivery, potentially worsening existent disparities for racial and ethnic minorities as well as other vulnerable populations. As TEAM has yet to be implemented, this review intends to prognosticate potential pitfalls and behaviors that may be motivated by this

Associations Between ACO Enrollment Status and Drug and Nondrug Costs Among Older Adults Newly Diagnosed With ADRD.

OBJECTIVE: Despite the promising role of accountable care organizations (ACOs) in improving care coordination and lowering total costs of care, it is unclear whether ACO enrollment leads to reduced drug costs among individuals with Alzheimer's disease and related dementias (ADRD). This longitudinal cohort study examined the long-term effects of ACO enrollment on drug and nondrug Medicare payments among Medicare beneficiaries with ADRD, focusing on their comorbidity burden. SETTING, PARTICIPANTS, AND MEASUREMENTS: This study tracked annual Part D prescription drug and nondrug payments of 111,2

Association of Pathways to Success Launch With Quality inBeneficiaries With Traditional Medicare.

OBJECTIVE: To evaluate the association between implementation of "Pathways to Success" and quality among beneficiaries cared for in Shared Savings Program accountable care organizations (ACOs). STUDY SETTING AND DESIGN: Medicare initiated "Pathways to Success" in 2019 that required upside-risk only ACOs in Shared Savings Program to transition to a two-sided risk model and prior two-sided ACOs to assume even greater financial responsibility. We examined the association between Pathways and ACO-targeted (hospitalizations for congestive heart failure [CHF] and all-cause 30-day readmissions) and
