The promiscuous and highly mobile resistome of Acinetobacter baumannii.

Antimicrobial resistance (AR) is a major global threat to public health. Understanding the population dynamics of AR is critical to restrain and control this issue. However, no study has provided a global picture of the whole resistome of Acinetobacter baumannii, a very important nosocomial pathogen. Here we analyse 1450+ genomes (covering >40 countries and >4 decades) to infer the global population dynamics of the resistome of this species. We show that gene flow and horizontal transfer have driven the dissemination of AR genes in A. baumannii. We found considerable variation in AR gene conte

Cathelicidin-like Peptide for Resistant Acinetobacter baumannii Control.

The growing global threat of antimicrobial resistance (AMR), particularly in cutaneous wound infections, represents a significant clinical and economic challenge. Biofilm formation by multidrug-resistant pathogens, such as Acinetobacter baumannii, often complicates healing and leads to therapeutic failure. Antimicrobial peptides (AMPs) are a promising alternative to conventional antibiotics due to their potent membrane-disrupting mechanism of action and lower propensity to induce resistance. Background/Objectives: This study aimed to evaluate the antibacterial, antibiofilm, and in vivo efficac

Therapeutic options for carbapenem-resistant Acinetobacter baumannii infections.

INTRODUCTION: Carbapenem-resistant Acinetobacter baumannii (CRAB) infections pose a global health threat due to high mortality, rising resistance, and limited treatment options. AREAS COVERED: Worldwide, OXA-type carbapenemases are the predominant mechanism of resistance; other mechanisms, including efflux pumps, porin mutations, and, rarely, metallo-β-lactamases, may contribute to resistance in resistant isolates, complicating management. Despite the different publication dates, both the Infectious Diseases Society of America (IDSA) and the European Society of Clinical Microbiology and Infec

Acinetobacter baumannii: Virulence Strategies and Host Defense Mechanisms.

Acinetobacter baumannii is a highly antibiotic-resistant bacterial pathogen known to cause severe life-threatening infections, including pneumonia, meningitis, and sepsis. Recent emergence of this bacterium as a serious nosocomial pathogen has led to categorization of A. baumannii as a "high-priority" pathogen by the World Health Organization (WHO), for which research efforts are urgently required to develop therapeutic interventions. Some of the properties that make A. baumannii a serious pathogen include its capacity to tolerate high levels of stress and enhanced expression of efflux pumps t

Antibacterial mechanism of peptide Cec4 against Acinetobacter baumannii.

BACKGROUND: A case of Acinetobacter baumannii (A. baumannii), known as gram-negative bacteria, causes a range of nosocomial infections. Due to the continuous detection of multi-drug resistant A. baumannii in the clinic, there is an urgent need to find alternative therapies, including broad-spectrum antibacterial peptides (AMP). Recently it has been found that the peptide Cec4 has good antibacterial activity against A. baumannii, but the antibacterial mechanism remains elusive. MATERIALS AND METHODS: The basic structure of Cec4 was analyzed by circular dichroism (CD) spectroscopy, and the pote

Aminoglycoside Resistance Genes in Acinetobacter baumannii Clinical Isolates.

BACKGROUND: Acinetobacter baumannii (A. baumannii) is an opportunistic pathogen associated with serious hospital acquired infections. It is resistant to multiple antibiotics. The therapy for infections due to this bacterium is a combination of aminoglycosides with carbapenem antibiotics. There are recent reports of the prevalence of ami-noglycoside resistance among A. baumannii. The aim of the present study was to determine the prevalence of phosphotransferases APH (3')-Via (aphA6), acetyltransferases AAC (3)-Ia (aacC1), nucleotidyl transferases ANT (2'')-Ia (aadB), and ANT (3") -Ia (aadA1) ge

Drug treatment for multidrug-resistant Acinetobacter baumannii infections.

Acinetobacter baumannii has emerged in the last decades as a major cause of healthcare-associated infections and nosocomial outbreaks. Multidrug-resistant (MDR) A. baumannii is a rapidly emerging pathogen in healthcare settings, where it causes infections that include bacteremia, pneumonia, meningitis, and urinary tract and wound infections. Antimicrobial resistance poses great limits for therapeutic options in infected patients, especially if the isolates are resistant to the carbapenems. Other therapeutic options include sulbactam, aminoglycosides, polymixyns and tigecycline. The discovery o

Influence of genospecies of Acinetobacter baumannii complex on clinical outcomes of patients with acinetobacter bacteremia.

BACKGROUND: acinetobacter baumannii complex infections are increasing in prevalence and are associated with a high mortality. Biochemical classification tests cannot differentiate A. baumannii (genospecies 2) from other genospecies. Genospecies typing offers a potential tool to determine whether there are major differences in pathogenicity among the genospecies. METHODS: adult patients with A. baumannii complex bacteremia in intensive care units were prospectively observed from January 2007 through July 2009. A. baumannii complex was identified by biochemical methods and the Phoenix bacterial

Current Methods for Reliable Identification of Species in the Acinetobacter calcoaceticus-Acinetobacter baumannii Complex.

Acinetobacter baumannii is one of the most challenging nosocomial pathogens associated with a variety of hospital infections, such as ventilator-associated pneumonia, wound and urinary tract infections, meningitis, and sepsis, primarily in patients treated in critical care settings. Its classification as a high-priority pathogen is due to the emergence of multidrug-resistant strains in healthcare environments and its tendency to spread clonally. A. baumannii belongs to the Acinetobacter calcoaceticus-Acinetobacter baumannii (Acb) complex, a group of genotypically and phenotypically similar spe

Isolation, characterization and therapeutic evaluation of a new Acinetobacter virus Abgy202141 lysing Acinetobacter baumannii.

Acinetobacter baumannii is an opportunistic pathogen that easily resists currently available antibiotics. Phages are considered alternative therapeutic agents to conventional antibiotics for the treatment of multidrug-resistant bacteria. We isolated an Acinetobacter virus Abgy202141 from underground sewage in a residential area of Guiyang City in China. Transmission electron microscopy (TEM) analysis showed that Acinetobacter virus Abgy202141 has an icosahedral head attached to a tail. This phage infects A. baumannii strain GY-4, and was found to have a short latent period of 5 min and with a

Sulbactam-Durlobactam for Carbapenem-Resistant Acinetobacter baumannii-calcoaceticus Complex.

Carbapenem-resistant Acinetobacter baumannii infections pose a significant challenge due to their severity and the poor prognoses they often result in, particularly in cases where there are risk factors present. The United States (US) Centers for Disease Control and Prevention (CDC) identified carbapenem-resistant Acinetobacter baumannii (CRAB) infections as a threat to human health. The World Health Organization (WHO) has classified it as a top priority for research. In 2023, the US FDA approved sulbactam-durlobactam for treating certain A. baumannii infections. As of 2024, this combination i

Colistin Resistance in Acinetobacter baumannii: Basic and Clinical Insights.

The emergence of drug-resistant bacteria has posed a significant problem in medical institutions worldwide. Colistin, which targets lipopolysaccharide (LPS), serves as a last-resort antimicrobial agent against multidrug-resistant Gram-negative bacteria. Nevertheless, Acinetobacter baumannii, a pathogen with a worldwide prevalence of antimicrobial resistance, has been reported to develop resistance to colistin frequently. In this review, we discuss how A. baumannii acquires resistance to colistin, focusing on modification as well as loss of LPS present in its outer membrane, which is the primar

Rapid Design of Combination Antimicrobial Therapy against Acinetobacter baumannii.

Treatment of serious bacterial infections with antimicrobial agents, such as antibiotics, is a major clinical challenge, because of growing bacterial resistance to multiple agents. Combination therapy (i.e. combined dosing of more than one agent) is often used for the purpose, but its systematic design remains a challenge. To address this, we recently reported a method to mathematically model bacterial response to antimicrobial agents, and to use this model for systematic design of clinically relevant combination therapy. The method relies on (a) longitudinal data of bacterial load, estimated

Repetitive Acinetobacter baumannii pneumonia induces infection tolerance in mice.

Some long-term hospitalized patients with lung infections exhibit pathogen tolerance. To investigate whether long-term chronic infection can induce tolerance, we constructed a mouse model of pneumonia in which mice were infected once, twice, or three times with Acinetobacter baumannii. The results revealed that the inflammatory factor levels decreased in the lung lavage fluid and that pathological damage to the lung tissue was alleviated in the mice infected three times. Flow cytometry and transcriptome analysis of mouse lung tissue revealed that the expression of genes related to T cell activ

Treatment of infections caused by carbapenem-resistant Acinetobacter baumannii.

Patients with severe carbapenem-resistant Acinetobacter baumannii (CRAB) infections currently face significant treatment challenges. When patients display signs of infection and the clinical suspicion of CRAB infections is high, appropriate treatment should be immediately provided. However, current treatment plans and clinical data for CRAB are limited. Inherent and acquired resistance mechanisms, as well as host factors, significantly restrict options for empirical medication. Moreover, inappropriate drug coverage can have detrimental effects on patients. Most existing studies have limitation

Crystal Structures of the Acinetobacter baumannii Macrolide Phosphotransferase E.

Acinetobacter baumannii (A. baumannii) challenges clinical infection treatment due to its resistance to various antibiotics. Multiple resistance genes in the core genome or mobile elements contribute to multidrug resistance in A. baumannii. Macrolide phosphotransferase gene mphE has been identified in A. baumannii, which is particularly relevant to macrolide antibiotics. Here, we determined the structure of MphE protein in three states: the apo state, the complex state with erythromycin and guanosine triphosphate (GTP), and the complex state with azithromycin and guanosine. Interestingly, GTP
