Revolutionizing prostate cancer treatment: PSMA-targeted therapy in modern therapeutics.

Prostate-specific membrane antigen (PSMA)-targeted radionuclide therapy has reshaped the treatment landscape for metastatic castration-resistant prostate cancer (mCRPC), offering new therapeutic possibilities beyond conventional approaches. Among the most promising strategies is lutetium-177 PSMA-617, which has shown significant benefits in overall survival and durable PSA responses, as demonstrated in several clinical trials. At the same time, the development of novel radionuclides such as actinium-225 has opened the door to more potent treatments capable of overcoming prior resistance, parti

Response to RL-225Ac in prostate cancer: Effect of prior treatment with RL-177Lu: A systematic review of the literature.

BACKGROUND: Targeted radionuclide therapy with Actinium-225-labeled prostate-specific membrane antigen agents (225Ac-PSMA) is currently being studied in clinical trials for patients with metastatic castration-resistant prostate cancer (mCRPC). Compared to β-emitting therapeutic radionuclides, alpha-emitters (e.g., 225Ac) have a significantly higher linear energy transfer and significantly shorter range. As a result, alpha emitters could be expected to improve efficacy and reduce bystander toxicity. This systematic literature review was conducted to evaluate the impact of sequencing of 177Lu-PS

Targeted Alpha Therapy as a Multiscale Design Problem: From Radioactive Decay to Therapeutic Outcome.

Targeted Alpha Therapy (TAT) is often defined by the favorable physical properties of alpha particles, particularly their high linear energy transfer and short tissue range. However, these properties alone do not determine therapeutic success. The clinical behavior of alpha-emitting radiopharmaceuticals depends on whether radioactive decay can be matched to an appropriate biological scale and maintained within a chemically and pharmacokinetically coherent system. This review presents TAT as a multiscale design problem in which radionuclide production, radioactive decay, recoil, coordination ch

Toxicities of radioligand and radioisotope therapy in prostate cancer: a systematic review and meta-analysis.

PURPOSE OF REVIEW: This systematic review and meta-analysis investigate the toxicities of radioligand and radioisotope therapies - [ 177 Lu]lutetium-prostate-specific membrane antigen (PSMA) (Lu-PSMA), [ 225 Ac]actinium-PSMA (Ac-PSMA), and [ 223 Ra]radium-dichloride (223-Radium) - in metastatic prostate cancer. While previous studies have explored this topic, most failed to differentiate between treatment-emergent adverse events (TEAEs) and preexisting conditions, leading to inflated toxicity rates. By focusing exclusively on TEAEs, this study provides a more accurate and clinically relevant a

Therapeutic outcomes of ²²⁵Ac/¹⁷⁷Lu-PSMA combination therapy in advanced metastatic Castration-Resistant prostate cancer: A systematic review and Meta-Analysis.

BACKGROUND: Targeted radionuclide therapy (TRT) has become a standard of care for patients with metastatic castration-resistant prostate cancer (mCRPC). Lutetium-177 labeled PSMA radioligand therapy ([¹⁷⁷Lu]Lu-PSMA) is an established and effective treatment option, while [²²⁵Ac]Ac-PSMA shows promise in refractory cases. The tandem use of [225Ac] Ac and [177Lu] Lu-labeled PSMA ligands is currently being explored to harness the complementary advantages of both isotopes. This meta-analysis investigates the efficacy and safety of [²²⁵Ac]Ac -/[¹⁷⁷Lu]Lu-PSMA radioligand therapy (RLT) in patients wit

Alpha Before Beta: Exceptional Response to First-Line 225Ac-DOTATATE in a Patient of Metastatic Neuroendocrine Tumor With Extensive Skeletal Involvement.

The utility of β-emitter 177Lu-DOTATATE in patients of neuroendocrine tumors (NETs) with widespread skeletal metastases is limited by its relatively modest response rates and a significant concern for hematotoxicity. In such situations, targeted α therapy with 225Ac-DOTATATE can be potentially beneficial. In this report, a 46-year-old man with rectal NET and extensive skeletal metastases was treated upfront with 6 cycles of 225Ac-DOTATATE at 8 weeks' intervals. The patient showed excellent symptomatic, biochemical, and radiological response with no grade 3/4 adverse events. The first-line use

Preclinical Evaluation of [225Ac]Ac-Sibrotuzumab Employing a PEG4-Macropa Site-Specific Chelation Strategy for Targeted Ablation of Cancer-Associated Fibroblasts in Desmoplastic Tumors.

Fibroblast activation protein is highly expressed in tumor-associated fibroblasts and represents a promising stromal target for cancer therapy. Actinium-225 (225Ac) offers potent α-emissions for targeted radiotherapy but requires exceptionally stable chelation. We report the synthesis and preclinical evaluation of the novel FAP-targeted α-immunoconjugate [225Ac]Ac-Macropa-PEG4-Sibrotuzumab. Sibrotuzumab was site-specifically modified with Macropa-PEG4 and radiolabeled with 225Ac under mild conditions. Radiochemical purity, serum stability, and immunoreactivity were assessed by radio-iTLC, SEC-

Development of [225Ac]Ac‑LNC1011 for targeted alpha-radionuclide therapy of prostate cancer.

PURPOSE: Prostate-specific membrane antigen (PSMA) radioligand therapy (PRLT) has become a promising option for treating metastatic castration-resistant prostate cancer (mCRPC). Radioligands labelled with the 68Ga/177Lu theranostic pair have been most widely used in the clinic for diagnosis and therapy, respectively. This study aims to develop a novel PSMA-targeted radioligand, LNC1011, radiolabeled with alpha-emitter 225Ac, to optimise pharmacokinetic properties and assess its potential for targeted alpha therapy (TAT) in prostate cancer treatment. METHODS: LNC1011 (Dan-PSMA) was synthesised

Framework to calculate 225Ac, 177Lu, and 161Tb radiation dose and biological effect in metastatic castration-resistant prostate cancer treatment.

BACKGROUND: In the treatment of metastatic castration-resistant prostate cancer (mCRPC), the standard radionuclide 177Lu (β⁻ emitter) is being challenged by alternatives, particularly the α-emitter 225Ac and Auger electron emitter 161Tb, due to their superior radiobiological properties. These include higher linear energy transfer (LET) and shorter ranges, which enhance localized cell killing while minimizing off-target effects. While these radionuclides induce DNA damage both in source-cells and neighboring cells (crossfire effect), their distinct radiation profiles provide critical metrics to

Estimated absorbed dose in tissues and radiation effects in Japanese Thorotrast patients.

The absorbed dose in the liver, spleen and bone marrow of Thorotrast patients was estimated for 71 autopsy cases. 232Th amounts in tissues were determined from measurements of 228Ac gamma-rays and the activity ratio of 228Ac to 232Th. In calculating the absorbed dose, Kaul's data were used as values of the steady state activity ratio between 232Th and its daughters and the self-absorption of alpha-rays in Thorotrast aggregates. The autopsy cases in which the cumulative dose was estimated consisted of 45 cases of malignant hepatic tumor, 12 cases of liver cirrhosis, 7 cases of blood disease and

Preclinical evaluation of [225Ac]Ac-crown-TATE - An alpha-emitting radiopharmaceutical for neuroendocrine tumors.

BACKGROUND: Targeted alpha therapy (TAT) of somatostatin receptor-2 (SSTR2) positive neuroendocrine tumors (NETs) involving Ac-225 ([225Ac]Ac-DOTA-TATE) has previously demonstrated improved therapeutic efficacy over conventional beta particle-emitting peptide receptor radionuclide therapy agents. DOTA-TATE requires harsh radiolabeling conditions for chelation of [225Ac]Ac3+, which can limit the achievable molar activities and thus therapeutic efficacy of such TAT treatments. Macropa-TATE was recently highlighted as a potential alternative to DOTA-TATE, owing to the mild radiolabeling condition

Calcium Carbonate Core-Shell Particles for Incorporation of 225Ac and Their Application in Local α-Radionuclide Therapy.

Actinium-225 (225Ac) radiolabeled submicrometric core-shell particles (SPs) made of calcium carbonate (CaCO3) coated with biocompatible polymers [tannic acid-human serum albumin (TA/HSA)] have been developed to improve the efficiency of local α-radionuclide therapy in melanoma models (B16-F10 tumor-bearing mice). The developed 225Ac-SPs possess radiochemical stability and demonstrate effective retention of 225Ac and its daughter isotopes. The SPs have been additionally labeled with zirconium-89 (89Zr) to perform the biodistribution studies using positron emission tomography-computerized tomogr

Selective alpha-particle mediated depletion of tumor vasculature with vascular normalization.

BACKGROUND: Abnormal regulation of angiogenesis in tumors results in the formation of vessels that are necessary for tumor growth, but compromised in structure and function. Abnormal tumor vasculature impairs oxygen and drug delivery and results in radiotherapy and chemotherapy resistance, respectively. Alpha particles are extraordinarily potent, short-ranged radiations with geometry uniquely suitable for selectively killing neovasculature. METHODOLOGY AND PRINCIPAL FINDINGS: Actinium-225 ((225)Ac)-E4G10, an alpha-emitting antibody construct reactive with the unengaged form of vascular endoth

Clinical translation of targeted alpha therapy: safety, dosimetry, radiation protection, and regulatory considerations.

Targeted alpha therapy (TAT) has moved from a largely experimental approach into clinically relevant radiopharmaceutical therapy, although evidence varies across radionuclides and products. Alpha particles combine high linear energy transfer with a tissue range of only a few cell diameters. This permits dense, localized energy deposition but creates challenges in dosimetry, radiopharmaceutical production, quality control, contamination control, patient management, waste disposal, and regulation. This review examines evidence for safe clinical implementation and distinguishes established practi

Radiopharmaceutical: An Update and Comparison of Preclinical Investigation Result of Alpha and Beta Emitter Radioisotope.

Radiopharmaceuticals are radioactive compounds used for diagnostic imaging and therapeutic purposes. In diagnostic, gamma emitters are commonly utilized. Conversely, radioisotopes employed for therapy are beta and alpha emitters, which generally have contrasting Linear Energy Transfer (LET) and tissue penetration profiles. These distinguishable characteristics allow for the complementary or improvement of radiopharmaceuticals. For instance, noticeable breakthroughs have been made with the approval of targeted beta-emitting radiopharmaceuticals using agents such as Lutathera ([177Lu]Lu-DOTA-TAT

[Clinical Application of Targeted Alpha Therapy Using Astatine].

Targeted alpha therapy is gaining prominence owing to its superior therapeutic efficacy compared with conventional radionuclide therapies using beta emitters. Astatine (211At, half-life: 7.2 h) has recently emerged as a promising alpha emitter, along with actinium (225Ac) and lead (212Pb). It can be produced domestically by irradiating natural bismuth targets with alpha particles using a 30 MeV cyclotron. Chemically analogous to iodine, astatine accumulates in differentiated thyroid cancer cells via the sodium/iodide symporter and demonstrated significant antitumor effects in a xenograft model

Synergistic Treatment of Prostate Cancer and Multiple Myeloma using CD46-Targeted Radioimmunotherapy Antibody-Drug Conjugates.

PURPOSE: CD46 is highly expressed across multiple cancer types, including prostate cancer and multiple myeloma. We have developed CD46-targeting antibody-drug conjugate (ADC) and actinium-225 (225Ac)-based α particle therapy agents that demonstrated a tumor-selective therapeutic effect. We hypothesized that a treatment strategy targeting CD46 using simultaneous ADC and radioimmunotherapy methods would have synergistic therapeutic efficacy with acceptable toxicity. EXPERIMENTAL DESIGN: Two CD46-targeted combination treatment strategies were evaluated: (i) co-administration of the ADC (YS5-MMAE
