Acute kidney injury in cirrhosis.

UNLABELLED: Acute renal failure (ARF), recently renamed acute kidney injury (AKI), is a relatively frequent problem, occurring in approximately 20% of hospitalized patients with cirrhosis. Although serum creatinine may underestimate the degree of renal dysfunction in cirrhosis, measures to diagnose and treat AKI should be made in patients in whom serum creatinine rises abruptly by 0.3 mg/dL or more (>/=26.4 micromol/L) or increases by 150% or more (1.5-fold) from baseline. The most common causes of ARF (the term is used interchangeably with AKI) in cirrhosis are prerenal azotemia (volume-respo

Malaria and acute kidney injury.

Malaria is a major public health problem in tropical countries. About 500 million people suffer from malaria, leading to death in 1 to 3 million cases. Acute kidney injury (AKI) is one of the most dreaded complications of severe malaria. As per World Health Organization criteria, acute renal failure (serum creatinine level, > or =3 mg/dL or > or =265 micromol/L) occurs as a complication of Plasmodium falciparum malaria in less than 1% of cases, but the mortality rate in these cases may be up to 45%. It is more common in adults than children. Renal involvement varies from mild proteinuria to se

Absence of Kidney Tubular Injury in Patients With Acute Heart Failure With Acute Kidney Injury.

BACKGROUND: Worsening renal function (WRF) is common in hospitalized patients being treated for acute heart failure. However, discriminating clinically significant WRF remains challenging. In patients hospitalized with acute heart failure, we evaluated if blood and urine biomarkers of cardiac and kidney dysfunction were associated with adverse outcomes. METHODS: We identified 175 of 927 participants in the AKINESIS study (Acute Kidney Neutrophil Gelatinase-Associated Lipocalin Evaluation of Symptomatic Heart Failure Study) who met criteria for stage 1 or 2 Kidney Disease: Improvement Global O

RIFLE and Acute Kidney Injury Network classifications predict mortality in leptospirosis-associated acute kidney injury.

AIM: Acute kidney injury (AKI) is a common complication in leptospirosis. The aim of this study is to investigate the association between RIFLE and AKIN classifications with mortality in leptospirosis-associated AKI. METHODS: A retrospective study was conducted in patients with leptospirosis admitted to tertiary hospitals in Brazil. The association between RIFLE and AKIN classifications with mortality was investigated. Univariate and multivariate analysis was performed to investigate risk factors for death. RESULTS: A total of 287 patients were included, with an average age of 37 ± 16 years,

Acute kidney injury in paediatric kidney transplant recipients.

Acute kidney injury (AKI) in paediatric kidney transplant recipients is common. Infection including urinary tract infection (UTI) and rejection are the most common causes in children. Surgical complications often cause AKI early post-transplant, whereas BK polyomavirus nephropathy rarely occurs in the first month post-transplant. Understanding kidney physiology helps to appreciate the sensitivity of the allograft to AKI, more so than native kidneys. Although the cause of AKI is often multi-factorial, there may be an underlying process that is treatable. Eliciting the aetiology, in this regard,

Acute Kidney Injury in Kidney Transplants: New Insights.

Delayed allograft function (DGF) is defined as dialysis treatment in the kidney transplant recipient in the first week following transplantation. With the demand for kidney transplants growing and the supply limited, as well as implementation of a national allocation scheme for deceased donor kidneys, rates of DGF remain high, on average, 30% for recipients of deceased donor kidneys. DGF is associated with inferior allograft outcomes, and there are no FDA-approved therapies to mitigate this disorder. There is renewed interest in this therapeutic arena, and there are several recent clinical tri

Acute Kidney Injury to Chronic Kidney Disease Transition.

BACKGROUND: Acute kidney injury (AKI), even if followed by renal recovery, is a risk factor for the future development of chronic kidney disease (CKD) and end-stage renal disease (ESRD). In the previous years, novel insights in the pathophysiology of CKD progression suggested a causal link between AKI and CKD due to a maladaptive repair after severe and repeated injury. SUMMARY: Several pathological mechanisms have been proposed to contribute to the progression of AKI and transition to CKD/ESRD including hypoxia and microvascular rarefaction, alterations of renal resident cell phenotypes and

Ultrasonographic Features of Gastropathy in Dogs with Acute Kidney Injury and Acute-on-Chronic Kidney Injury.

Despite the frequent gastrointestinal involvement, no studies have described the ultrasonographic appearance of the stomach in dogs with acute kidney injury (AKI) or acute-on-chronic kidney disease (ACKD). This retrospective study aimed to characterize the ultrasonographic features of gastropathy in such patients. Dogs diagnosed with AKI or ACKD and showing gastric abnormalities on ultrasound between January 2014 and January 2024 were included. A total of 113 dogs met the inclusion criteria. Gastric wall thickening was observed in all cases. Mucosal and/or submucosal abnormalities were present

Peritoneal dialysis and acute kidney injury in acute brain injury patients.

Acute kidney injury (AKI) is a heterogeneous syndrome with multiple etiologies. It occurs frequently in the neurocritical intensive care unit and is associated with greater morbidity and mortality. In this scenario, AKI alters the kidney-brain axis, exposing patients who receive habitual dialytic management to greater injury. Various therapies have been designed to mitigate this risk. Priority has been placed by KDIGO guidelines on the use of continuous over intermittent acute kidney replacement therapies (AKRT). On this background, continuous therapies have a pathophysiological rationale in p

Acute kidney injury in dengue fever using Acute Kidney Injury Network criteria: incidence and risk factors.

The aim of this study was to assess incidence and risk factors for acute kidney injury (AKI) in patients with dengue fever (DF). A total of 223 patients (males, 130; females, 93; mean age, 26.2 ± 18.2 years) from a tertiary care centre in southern India were retrospectively analysed. Acute renal failure (ARF) developed in 24 (10.8%) patients. Based on the Acute Kidney Injury Network (AKIN) criteria, the results revealed that: 12 (5.4%) had mild AKI; seven (3.1%) had moderate AKI; and five (2.2%) had severe AKI. A further 54 (24%) were diagnosed with dengue haemorrhagic fever (DHF); 11 (5%) wer

Acute kidney injury, acute kidney disease and chronic kidney disease: challenges and research perspectives.

Acute kidney injury (AKI) and chronic kidney disease (CKD) are interconnected, with AKI often leading to CKD and vice versa. Despite research advances, their causal relationship in clinical settings remains unclear. Inflammation, oxidative stress and maladaptive repair are key factors in AKI's progression to CKD. AKI episodes may hasten CKD progression, influenced by demographics, comorbidities and treatment factors like blood pressure control. This underscores the need for careful management to prevent long-term damage. Prospective cohort studies addressing confounding factors are essential t

Acute kidney injury in the tropics.

The tropics are a region consisting of more than 125 countries, accounting for 40% of the world's population. The region's population is expected to increase up to 60% in the coming decades. Many tropical countries continue to experience public health problems such as high rates of infectious diseases, lack of sanitation, climate change impacts, poor regulation of herbal medicines and low access to healthcare. These conditions produce the unique problem of tropical acute kidney injury (AKI), which is associated with high morbidity and mortality. Tropical infections such as leptospirosis, dengu

Acute kidney injury and childhood acute myeloid leukemia.

BACKGROUND: Acute kidney injury (AKI) appears to be particularly common in children with acute myeloid leukemia (AML), although the epidemiology data on this patient population is sparse. The objective of this study was to assess the prevalence and factors associated with AKI in childhood AML during chemotherapy treatment. METHODS: The medical records of 112 children aged under 15 years diagnosed with AML who received chemotherapy in a major tertiary-care referral center in southern Thailand were reviewed. Logistic regression was used to identify factors associated with AKI. RESULTS: Fifty-s

[Acute kidney injury related to acute vascular disorders].

Acute kidney injury related to acute vascular disorders. A broad spectrum of vascular disease characterized by primary vessel involvement may be associated with acute renal failure. These diseases are usually classified according to the size of the injured vessel that include the small vessels (thrombotic microangiopathy, kidney injury due to malignant hypertension, scleroderma renal crisis, and cholesterol crystal embolism disease), the medium vessels (polyarteritis nodosa) or the large size vessels (acute renal infarction). Regardless of the primary pathogenic mechanisms and/or the size of t

Acute kidney injury in the tropics.

Acute kidney injury (AKI) is one of the most challenging problems faced by clinicians in the tropics owing to its fast-changing burden. AKI in the tropics is strikingly different from that in the developed world in terms of etiology and presentation. In addition, there is a stark contrast between well-developed and poor areas in the tropics. The true epidemiological picture of AKI in the tropics is not well understood due to the late presentation of patients to tertiary centers. Infections remain the major culprit in most cases of AKI, with high mortality rates in the tropics. Human immunodefi
