The predictive value of adjustment disorders: a follow-up study.

A previous investigation explored the diagnosis of adjustment disorder and found it to have descriptive and face validity. This study of the status at 5-year follow-up of 100 patients given this diagnosis strongly supports the validity of the category among adults but only partially among adolescents. Seventy-nine percent of the adults were well at follow-up, with 8% having had an intervening problem. Comparable figures for the adolescents were 57% and 13%. Most adults who were ill developed either major depression or alcoholism. The adolescents' illnesses included schizophrenia, schizoaffecti

Prevalence of depressive, bipolar and adjustment disorders, in Quebec, Canada.

BACKGROUND: The prevalence of mood disorders was estimated at 5.4% according to the latest Canadian survey. It has been suggested to use administrative data and self-reported data to optimize the estimation for mental health care in the population. Using administrative data, algorithms (combination of codes from the International Classification of Diseases) had been previously developed to identify the population with mood and anxiety disorders. However, the specific prevalence of each component of mood disorders (depressive, bipolar and adjustment disorders) are still unknown in Quebec, Canad

Adjustment disorders after severe life-events in four postconflict settings.

BACKGROUND: The present study explores whether severe life-events are associated with adjustment disorders (AD) by reanalyzing the data of a multisite epidemiological survey (de Jong et al. in Lancet 361:2128-2130, 2003). AD were diagnosed according to the new stress-response-model of AD (Maercker et al. in Psychopathology 40(3):135-146, 2007). METHOD: Data from 3048 persons from four different refugee settings in Ethiopia, Algeria, Gaza, and Cambodia were reanalyzed. Life events were assessed by an adapted version of the Life Events and Social History Interview (Mollica et al. in Am J Psychi

Social adjustment in in-patients with affective disorders: predictive factors.

We assessed social adjustment in 145 depressed in-patients using the self-reporting Social Adjustment Scale (42-item version) to evaluate the contribution of demographic and clinical variables and examine social functioning at different levels of depression. Our results indicate that the presence of a psychopathology in association with interpersonal sensitivity, hostility and perceived social support aspects -- and not the severity of current depressive symptoms -- were the most important factors affecting social adjustment. As expected, social disturbances are more pronounced in severe depre

[Adjustment disorders with developmental risk in children after inpatient treatment].

The relatively broad spectrum of symptoms in child psychiatry (disturbances of behaviour and of anxiety and others) was examined in order to find smaller diagnostic groups. Two selected groups of probands of the Department of Child and Adolescent Psychiatry at the University Hospital for Children in Jena were analysed. This analysis was made by recording journals of treatment and a questionnaire. Both groups differed significantly in 33 out of 128 characteristics. The following factor analysis resulted in 10 anxiety- and non-anxiety-syndromes leading back to basic disturbances of affect, psych

Is the distinction between adjustment disorder with depressed mood and adjustment disorder with mixed anxious and depressed mood valid?

BACKGROUND: In the DSM-IV, adjustment disorder is subtyped according to the predominant presenting feature. The different diagnostic code numbers assigned to each subtype suggest their significance in DSM-IV. However, little research has examined the validity of these subtypes. In the present report from the Rhode Island Methods to Improve Diagnostic Assessment and Services (MIDAS) project, we compared the demographic and clinical profiles of patients diagnosed with adjustment disorder subtypes to determine whether there was enough empirical evidence supporting the retention of multiple adjust

Seasonal affective disorder, grief reaction, and adjustment disorder.

Seasonal affective disorder is a subtype of other affective disorders. The most studied treatment is light therapy, although second-generation antidepressants are also an option. Grief reactions are normal for patients experiencing loss, and primary care providers (PCPs) should be aware of both the expected course of grief and the more severe symptoms that indicate complex grief. Adjustment disorder is a time-limited abnormal response to a stressor. PCPs can manage patients with adjustment disorder by arranging counseling, screening for suicidality, assessing for substance abuse, and ruling ou

Pituitary volumes in patients with adjustment disorder.

The pituitary gland plays an important role in the stress response mechanism. Given the direct link between adjustment disorder and stress, we hypothesized that there might be changes in the pituitary gland in these patients. The study comprised a patient group of 19 individuals with adjustment disorder according to the Diagnostic and Statistical Manual of Mental Disorders Fifth Edition, and 18 healthy controls. The mean pituitary gland volumes of the patient group were not statistically significantly different from those of the healthy control group (80.81 ± 1.82 mm3 in patients with adjustme

[Adjustment disorder and DSM-5: A review].

INTRODUCTION: This paper exposes the complexity and discrete characteristic of the adjustment disorder with reference to its clinical and scientific diagnosis. Even though the disorder occurs in frequent clinical circumstances after important life events, such as mobbing, burn-out, unemployment, divorce or separation, pregnancy denial, surgical operation or cancer, the adjustment disorder is often not considered in the diagnosis since better known disorders with similar symptoms prevail, such as major depression and anxiety disorder. Ten years ago, Bottéro had already noticed that the adjustme

Diagnosing adjustment disorder with depressive features.

Adjustment disorder (AD) with depressive features is a common but under researched diagnosis. There are particular problems associated with the diagnosis of AD using structured interviews and screening schedules have yet to be successfully developed owing to the poor specificity of those that are currently used. The problem of underdiagnosing AD due to symptom overlap with other disorders, on the one hand, and, on the other, the danger of pathologizing normal distress as AD are considered. The diagnosis of AD in practice is an exercise that requires clinical judgment concerning the context of

Adjustment disorder: a review of diagnostic pitfalls.

Adjustment disorder is a common diagnosis in psychiatric settings and carries a significant rate of morbidity and mortality. However, both current and previous diagnostic criteria are vague and lead to many difficulties in terms of validity and reliability. This review is based on a thorough literature search and a systematic evaluation of the empiric and theoretic data. The various pitfalls inherent in the process of diagnosing this disorder are discussed in light of the diagnostic criteria for the disorder.

Adjustment disorder and risk of Parkinson's disease.

BACKGROUND AND PURPOSE: It has been postulated that stress is part of the etiological process of Parkinson's disease (PD). The risk of PD was examined in a cohort of patients with adjustment disorders, a diagnosis made in the presence of a severe response to a stressful life event. METHODS: Using Danish medical registries, PD occurrence was examined in a nationwide population-based cohort of patients with adjustment disorder diagnosed between 1995 and 2011. The standardized incidence ratio of PD was calculated as the ratio of observed to expected cases, stratified by time and potential risk f

Adjustment disorder of conscripts as a military phobia.

Conscription entails many adjustment difficulties for the young soldier. When anxiety is sufficiently severe to impair performance and make discharge a possibility, a brief course of cognitive-behavioral therapy can rapidly restore the soldier to full functioning. This article explains the theoretical basis for such an approach, and describes three cases.
