The value of local dexmedetomidine as an adjuvant to ultrasound-guided wide awake local anesthesia no tourniquet (WALANT) in flexor tendon repair surgeries: a randomized controlled trial.

BACKGROUND: The Wide-Awake Local Anesthesia No Tourniquet (WALANT) technique allows intraoperative motor assessment of tendon repair integrity of the hand compared with general anesthesia or brachial plexus block. No studies have tested the effect of adding dexmedetomidine to lidocaine on the analgesic properties of the WALANT technique, which is the aim of our study. METHODS: A total of 128 patients aged more than 18 years were scheduled for surgical flexor tendon injury repair using WALANT technique. Patients were divided into two equal groups. Ultrasound-guided subcutaneous injection of li

Comparison of subanaesthetic ketamine and lidocaine as adjuvants to propofol-based total intravenous anesthesia on propofol requirement during emergency decompressive craniectomy for traumatic brain injury: a prospective, double-blind, three-arm randomized controlled trial.

BACKGROUND: Prevention of secondary brain injury remains a key objective during emergency decompressive craniectomy for traumatic brain injury (TBI). Propofol-based total intravenous anesthesia (TIVA) is widely used in TBI because of its favorable cerebral physiological effects but may cause dose-dependent hypotension and compromise cerebral perfusion. Ketamine and lidocaine have been proposed as adjuncts that may reduce anesthetic requirements while preserving intracranial homeostasis. This study compared the effects of ketamine and lidocaine versus placebo on propofol consumption and periope

Xenon as an adjuvant to sevoflurane anesthesia in children younger than 4 years of age, undergoing interventional or diagnostic cardiac catheterization: A randomized controlled clinical trial.

BACKGROUND: Xenon has repeatedly been demonstrated to have only minimal hemodynamic side effects when compared to other anesthetics. Moreover, in experimental models, xenon was found to be neuroprotective and devoid of developmental neurotoxicity. These properties could render xenon attractive for the anesthesia in neonates and infants with congenital heart disease. However, experience with xenon anesthesia in children is scarce. AIMS: We hypothesized that in children undergoing cardiac catheterization, general anesthesia with a combination of sevoflurane with xenon results in superior hemody

Ultrasound-Guided Costoclavicular Brachial Plexus Block Using 0.5% Levobupivacaine Alone or With Dexamethasone as an Adjuvant for Upper Arm Surgical Anesthesia and Postoperative Analgesia: A Randomized Study.

BACKGROUND: Regional anesthesia techniques, such as brachial plexus blocks, are widely used for upper limb surgeries. The costoclavicular approach is a relatively new technique that targets the brachial plexus cords in a compact arrangement. Adjuvants like dexamethasone have been shown to enhance the efficacy of local anesthetics in various regional blocks, but studies regarding their effect in costoclavicular brachial plexus block (CCB) with levobupivacaine are limited. OBJECTIVE: The objective of this study is to compare the efficacy of 0.5% levobupivacaine alone versus 0.5% levobupivacaine

A Comparative Study of Intrathecal Fentanyl and Nalbuphine as an Adjuvant to Hyperbaric Bupivacaine for Spinal Anesthesia in Lower Limb Orthopedic Surgeries: A Prospective, Double-Blind, Randomized Controlled Study.

BACKGROUND: Spinal anaesthesia is the most commonly used technique for lower limb orthopaedic surgeries as it is economical and easy to administer. Opioids as adjuvants to local anaesthetics during spinal anaesthesia have played a vital role in reducing post-operative pain qualitatively and effectively. METHODS: This prospective randomised study was conducted on 100 patients divided into two groups scheduled for lower limb orthopaedic surgeries. Group bupivacaine fentanyl (BF) received 25 mcg of fentanyl with 15 mg of bupivacaine and Group bupivacaine nalbuphine (BN) received 1 mg of nalbuphi

Efficacy of Direct Versus Peripheral Adjuvant Dexamethasone on Duration and Rebound Pain in Regional Anesthesia for Outpatient Distal Radius Fracture Fixation: A Prospective Randomized Controlled Blinded Study.

BACKGROUND: Despite increasingly wider use, there remains controversy among anesthesiologists regarding preferred formulations and the role of steroid adjuvants in regional anesthesia. There is also uncertainty in the role of dexamethasone when administered directly versus peripherally. We hypothesize that directly mixing dexamethasone into the regional nerve block rather than peripherally administered intravenous dexamethasone will demonstrate a difference in efficacy concerning duration and rebound pain, decreased postoperative pain scores, or opioid consumption within the short-term postope

Intrathecal dexmedetomidine as an adjuvant to plain ropivacaine for spinal anesthesia during cesarean section: a prospective, double-blinded, randomized trial for ED50 determination using an up-down sequential allocation method.

BACKGROUND: Intrathecal dexmedetomidine, as an adjuvant to local anesthetics, has been reported to improve the quality of spinal anesthesia and reduce the required local anesthetic dose. However, the optimal dosage regimen for intrathecal dexmedetomidine combined with plain ropivacaine for cesarean section (CS) remains undetermined. The present study aimed to determine the median effective dose (ED50) of intrathecal dexmedetomidine as an adjuvant to plain ropivacaine for spinal anesthesia during CS. METHODS: Sixty parturients undergoing CS were randomly assigned to either group: plain ropivac

New insights into the use of dexamethasone-dexmedetomidine as perineural adjuvants in peripheral nerve and fascial blocks: an up-to-date narrative review.

The discovery of the mechanism of action of local anesthetics, involving the blocking of sodium ion channels is considered a milestone in anesthesia. Potentially lethal toxic effects occur in the case of intravascular injection of local anesthetic (LA) or in case of exceeding dosages, as a result of systemic reabsorption. Attempts to prevent these life-threatening events have been made by developing novel LA and by adding adjuvants. The aim of this up-to-date review was to provide the newest insights on the two adjuvants dexamethasone and dexmedetomidine for regional anesthesia. The addition o

A Narrative Review on Multimodal Spinal Anesthesia: Old Technique and New Use.

Spinal anesthesia has a long history in providing safe and effective anesthesia and analgesia. Its continuous evolution in block placement techniques with or without ultrasound assistance, options of spinal needles, local anesthetics and adjuvants adopted based on spinal anesthesia onset rate, duration of action, and side effect profiles, makes spinal anesthesia an important modality in current early recovery after surgery for a broad-spectrum of surgical procedures including ambulatory lower extremity orthopedic procedures and major chest, abdominal, and spine surgeries.

Efficacy of low-dose hypobaric anesthetics in spinal anesthesia for cesarean delivery: systematic review and meta-analysis.

OBJECTIVE: Many studies have compared the anesthetic effects of different doses of hypobaric local anesthetics (without glucose) in spinal anesthesia for cesarean sections. Therefore, a meta-analysis of these trials is warranted. METHODS: Systematic review and meta-analysis of eligible randomized controlled trials comparing the efficacy of low-dose spinal hypobaric anesthetics (Bupivacaine < 10 mg, levobupivacaine < 10 mg, ropivacaine < 15 mg) with conventional dose (Bupivacaine/Levobupivacaine ≥ 10 mg, ropivacaine ≥ 15 mg) in elective and subemergency Cesarean sections in data sources (PubMe

Thoracic spinal anesthesia with intrathecal sedation for lower back surgery: a retrospective cohort study.

BACKGROUND: Spinal anesthesia (SA) is a good alternative to general anesthesia (GA) for spine surgery. Despite that, a few case series concern the use of thoracic spinal anesthesia for short-duration surgical interventions. In search of an alternative approach to GA and a better opioid-free modality, we aimed to investigate the safety, feasibility, and patient satisfaction of thoracic SA for spine surgery. MATERIALS AND METHODS: We analyzed retrospectively a cohort of 24 patients operated on for a degenerative and osteoporotic pathology of the lower thoracic and lumbar spine. Data was collect

[Complications of regional anesthesia (author's transl)].

In spite of the great advances in anesthesia, regional anesthesia has not lost its importance. The development of new, longer-acting local anesthetics has extended its indication still more. The causes of accidents lie mostly in ignorance ofthe pharmacology of local anesthetics and their adjuvants, in faulty technical performance of the nerve blockade and last but not least in inadequate knowledge of the therapy, of the side effects and complications. The various complications of regional anesthesia are reported on and the necessary therapy dealt with.

A Comprehensive Review of the Use of Alpha 2 Agonists in Spinal Anesthetics.

BACKGROUND: Spinal Anesthesia was the first regional anesthetic technique to be performed. It was performed by Dr. August Bier, known for the Bier block, and his colleagues on August 16, 1898. Dr. Bier opted for, what he referred to at the time as "cocainization of the spinal cord" by introducing 15 mg of cocaine intrathecally prior to the operation. The surgery was largely uneventful and painless. The patient only experienced some vomiting and a headache postoperatively. Dr. Bier's use of neuraxial anesthesia aimed to directly inject local anesthetics in and around the central nervous system
