Advance Care Planning Among Adult Patients and Their End-of-Life Care Preferences.

To explore people's intentions to opt for a good death when planning for their end-of-life care, this study examined the type of end-of-life care preferred by patients receiving advance care planning (ACP) consulting services for five specified clinical and disability conditions and possible factors affecting their decision-making. This cross-sectional study analyzed 1303 hospital patients and 1032 nonhospital patients who attended a clinic providing ACP consulting services. This study revealed the following two results. First, patients who were older, were female, did not have an appointed su…

Advance Care Planning in a Geriatric Primary Care Clinic: A Retrospective Chart Review.

PURPOSE:: Advance care planning (ACP) is theorized to benefit both the patient and their family when end of life is near as well as earlier in the course of serious illness. However, ACP remains underutilized, and little is known about the nature of ACP documentation in geriatrics practices. The study investigated the prevalence and nature of ACP documentation within a geriatric primary care clinic. METHODS:: A retrospective chart review was conducted on a randomly selected sample of electronic medical record (EMR) charts. The sample consisted of patients aged 65 and older who were seen in th…

Palliative care, advance care planning, and end-of-life care: a quest for conceptual clarity.

Healthcare providers will increasingly care for patients with cardiovascular disease that has progressed to advanced or even end stage. In this setting, goals of care may shift from life-prolonging to symptom-focused, and use of approaches like advance care planning, palliative care, and end-of-life care may be increasingly important. Clinicians must be familiar with these concepts to employ them in a timely and appropriate manner. Herein, we describe the distinguishing features of each concept, describe common barriers to use, and demonstrate when patients may benefit from these approaches, i…

Advance Care Planning for Patients With Hematologic Malignancies: A Narrative Review.

Advance care planning (ACP) completion rates are higher in patients with serious illness compared to the general population, however, ACP is overall under-utilized and sub-optimal, especially for patients with hematologic malignancies. This patient population can experience unique and significant physical and psychological symptoms due to their illness and the treatment, resulting in high rates of aggressive end of life care. This high healthcare utilization pattern often triggers ACP conversations and documentation, often facilitated by specialty palliative care clinicians. This review articl…

Artificial intelligence-based approaches for advance care planning: a scoping review.

BACKGROUND: Advance Care Planning (ACP) empowers individuals to make informed decisions about their future healthcare. However, barriers including time constraints and a lack of clarity on professional responsibilities for ACP hinder its implementation. The application of artificial intelligence (AI) could potentially optimise elements of ACP in practice by, for example, identifying patients for whom ACP may be relevant and aiding ACP-related decision-making. However, it is unclear how applications of AI for ACP are currently being used in the delivery of palliative care. OBJECTIVES: To explo…

Advance care planning knowledge and documentation in a hospitalized cancer population.

To have a better understanding of our patients' knowledge of advance directive planning and execution, as well as communication with their oncologists regarding their wishes, we conducted a survey on our inpatient hematology-oncology services. A total of 68 unique hospitalized patients with a diagnosis of cancer completed surveys. Surveys were given to all oncology patients regardless of their reason for admission. Overall, 29% of the patients reported having had a discussion with their oncologist regarding their wishes if they became seriously ill or near death. Of those who did have this con

Assessing Advance Care Planning Readiness in Older Adults: A Pilot Study.

The purpose of this pilot posttest-only, randomized, controlled group study was to evaluate if a combination of educational methods (written, video, and/or group discussion) would improve the readiness of older adults to participate in advance care planning (ACP). Participants were 65 years and older, English-speaking, receiving outpatient care, having at least one life-limiting illness, and without an advance directive. An independent t test and 1-way analysis of variance compared responses to 9 survey questions of readiness between the control group and the intervention groups. This pilot st…

Living Will and Advance Care Planning: The Need of the Hour.

Advance care planning (ACP) refers to the process through which patients, their families, and healthcare providers discuss and record preferences for end-of-life care.1 Internationally, ACP has been shown to reduce unnecessary interventions, align medical decisions with patient wishes, and provide dignity at the end-of-life. In India, however, structured ACP is virtually absent. Good end-of-life care is directly linked to the quality of death that Indians achieve, which has been consistently poor; in the 2021 Quality of Death Index report, India ranked 59 out of 81 countries that were studied.…

Advance care planning among hematopoietic cell transplant patients and bereaved caregivers.

Younger, healthier patients contemplating high-risk (but potentially curative) hematopoietic cell transplants (HCT) may not consider advance care planning (ACP). We investigated the effect of pre transplant ACP in surviving HCT patients and bereaved caregivers using retrospective, audiotaped telephone surveys. Subjects were identified between 2001 and 2003 via databases at two high-volume HCT centers. Transcripts were coded by two investigators, with differences resolved by consensus. HCT survivors (n=18) were interviewed a median of 13 months after HCT for acute leukemia (7), lymphoma (5) or …

Paradoxes in advance care planning: the complex relationship of oncology patients, their physicians, and advance medical directives.

PurposeMany seriously ill patients with cancer do not discuss prognosis or advance directives (ADs), which may lead to inappropriate and/or unwanted aggressive care at the end of life. Ten years ago, patients with cancer said they would not like to discuss ADs with their oncologist but would be willing to discuss them with an admitting physician. We assessed whether this point of view still held.Patients and methodsSemi-structured interviews were conducted with 75 consecutively admitted patients with cancer in the cancer inpatient service.ResultsOf those enrolled, 41% (31 of 75) had an AD.
