Concordance of Embedded Performance and Symptom Validity Tests and Associations with Mild Traumatic Brain Injury and Posttraumatic Stress Disorder among Post-9/11 Veterans.

OBJECTIVE: The present study explored both embedded symptom (SVT) and performance (PVT) validity test scores within a post-9/11 veteran sample to elucidate the degree to which there is concordance between validity indicators, as well as how frequently one SVT and four PVT indicators were failed in screened mild traumatic brain injury (mTBI) and diagnosed posttraumatic stress disorder (PTSD). METHOD: A total of 114 post-9/11 veterans were evaluated utilizing the Neurobehavioral Symptom Inventory (NSI) Validity-10, four embedded PVTs, mTBI screening, and a diagnostic interview for PTSD. Long-Term Outcomes of Thoracic Trauma in U.S. Service Members Involved in Combat Operations.

INTRODUCTION: Nearly 10% of all combat injuries during the most recent conflicts in Iraq and Afghanistan involve thoracic trauma. The long-term outcomes of these combat-related injuries with respect to lung function have not been fully evaluated. Limited research in civilian polytrauma patients have shown significant obstructive physiology in nearly half of their population without clear etiology. We sought to further characterize the extent to which these active duty service members (ADSM) are chronically affected by their thoracic injuries. MATERIALS AND METHODS: We conducted a retrospectiv

Oral-Maxillofacial Injury Surveillance of U.S. Military Personnel in Iraq and Afghanistan, 2001 to 2014.

BACKGROUND: Cranial and oral-maxillofacial injuries accounted for 33% of military visits to in-theater (Level III) military treatment facilities for battle injuries during Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF). Even after years of conflict, the size and scope of oral-maxillofacial injuries in military armed conflict is still not fully understood. This study reports U.S. Department of Defense (DoD) rates of oral-maxillofacial injuries that can be used for further surveillance and research. METHODS: The populations studied were military personnel deployed to Afghani

MORAL INJURY.

The devastating effect on the self of moral injury, often a core component of trauma, occurring when one's actions have profoundly violated one's code of ethics, when one has been a victim of such violation, or when one has been a passive witness, has been extensively explored as it has occurred in veterans of the wars in Vietnam, Iraq and Afghanistan. Two examples illustrate its prevalence in civilian life. The literature shows violation of expected empathy from and for others, inherent in our nature, is more devastating than violation of the ethical code of our culture or sub-culture, adhere

Deserving Veterans' Disability Compensation: A Qualitative Study of Veterans' Perceptions.

Veterans recently returned from Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF) experience many health and mental health problems after deployment. These OEF/OIF veterans are applying and appealing for veterans' disability compensation (VDC) at rapidly increasing rates, often for "invisible conditions" such as posttraumatic stress disorder. Little is known about how veterans experience the process of applying and receiving VDC. In-depth qualitative interviews were conducted with OEF/OIF veterans ages 35 and younger (N = 18). This article addresses how veterans perceive thems

Military Occupational Outcomes in Canadian Armed Forces Personnel with and without Deployment-Related Mental Disorders.

OBJECTIVE: Mental disorders are common in military organizations, and these frequently lead to functional impairments that can interfere with duties and lead to costly attrition. In Canada, the military mental health system has received heavy investment to improve occupational outcomes. We investigated military occupational outcomes of diagnosed mental disorders in a cohort of 30,513 personnel who deployed on the Afghanistan mission. METHODS: Cohort members were military personnel who deployed on the Afghanistan mission from 2001 to 2008. Mental disorder diagnoses and their attribution to the

[Operational fatigue: debriefing from Kapisa, Afghanistan].

The concept of operational fatigue, a term first used during the Second World War, was subsequently extended to encompass a vague series of psychological disorders. This article looks back at an overseas operation in Afghanistan, during the summer of 2011, where the tense background situation, casualties and fatalities resulted in some members of the commando parachute units suffering from such conditions.

Concussive brain injury in the military: September 2001 to the present.

Since the terrorist attacks of September 11, 2001, 1,348,405 citizens have been deployed to combat in Operation Iraqi Freedom (OIF), Operation New Dawn in Iraq, and Operation Enduring Freedom in Afghanistan (OEF). During this same period 266,810 (20%) of these individuals have been diagnosed with a traumatic brain injury (TBI). The majority of these were Army soldiers, with 155,282 (58%) receiving the diagnosis. Mild TBI comprised 82% of the total, with the remainder being moderate to severe. Over this same period the Department of Defense (DoD) has invested $374.9 million to enhance access an

Confirmed malaria cases among active component U.S. Army personnel, January-September 2012.

Of 26 cases of malaria reported among active component U.S. Army personnel during January through September 2012, 16 were laboratory-confirmed according to electronic medical records. Medical records and responses on postdeployment health assessment questionnaires were used to assess demographic and clinical characteristics, adherence to malaria prevention measures, and compliance with prescriptions for chemoprophylaxis. All but two cases were confirmed by peripheral blood smears. Twelve cases were caused by Plasmodium vivax, one by P. falciparum, and three unspecified diagnoses were deemed li

Fever in a soldier returned from Afghanistan.

We present a case of Plasmodium vivax infection in a soldier, 4 months after returning from Afghanistan. Primary care physicians should be reminded of the possible delay in presentation of P. vivax when evaluating fever and the importance of terminal prophylaxis with primaquine to prevent relapse following return from malarious regions.

Trajectory of self-rated health after combat-related injury.

BACKGROUND: The U.S. military conflicts in Iraq and Afghanistan yielded the most combat casualties since Vietnam. With more service members than ever surviving their wounds, prospective research on factors related to long-term, patient-reported outcomes, including self-rated health (SRH), has increased importance. This study's objective was to use preinjury and postinjury SRH measures to identify trajectories and predictors of SRH after combat-related injury. METHODS: The Wounded Warrior Recovery Project was queried for U.S. military personnel with combat-related injuries from Iraq or Afghani

The association between blast exposure and transdiagnostic health symptoms on systemic inflammation.

Chronic elevation of systemic inflammation is observed in a wide range of disorders including PTSD, depression, and traumatic brain injury. Although previous work has demonstrated a link between inflammation and various diagnoses separately, few studies have examined transdiagnostic symptoms and inflammation within the same model. The objective of this study was to examine relationships between psychiatric and health variables and systemic inflammation and to determine whether mild traumatic brain injury (mTBI) and/or exposure to blast munitions moderate these relationships. Confirmatory facto

Review of a Role 2 in Afghanistan: Understanding the Data on Medical and Surgical Volumes in a Deployed Setting.

INTRODUCTION: The U.S. forward military surgical assets have deployed throughout the Iraq and Afghanistan theaters of operations to maintain surgical support for injured service members in compliance with the "golden hour" as specified in the Gates Memorandum. The support of evacuation times of less than 60 minutes to a surgical capability has resulted in smaller surgical teams being deployed to an increased number of locations. Over the last 5 years, the combat trauma patient encounters have decreased. Although some Role 2 medical treatment facilities (MTFs) maintain a medical mission, most o

Direct and indirect relationships among posttraumatic stress disorder, depression, hostility, anger, and verbal and physical aggression in returning veterans.

Hostility, anger, and aggression are conceptually related but unique constructs found to occur more often among veterans with posttraumatic stress disorder (PTSD) than among civilians or veterans without PTSD. However, the pathways between PTSD, depression, hostility, anger, and aggression have not been comprehensively characterized. Therefore, drawing on a sample of returning Operation Enduring Freedom/Operation Iraqi Freedom combat veterans ( N = 175; 95% male; mean age 30 years), this study sought to examine the direct and indirect relationships among PTSD, depression, hostility, anger, and

A recent case of periorbital necrotizing fasciitis--presentation to definitive reconstruction within an in-theater combat hospital setting.

First described in 1924, necrotizing fasciitis (NF) is a rapidly progressing, severe suppurative infection of the superficial fascia, often associated with vascular thrombosis and necrosis of the overlying skin. Despite advances in medical therapy, the mortality remains high, with rates exceeding 25 to 50% in some studies. Early diagnosis and treatment is paramount in the management of this serious infection and should include wide surgical debridement and drainage of all necrotic tissues, coupled with aggressive parenteral antibiotics. This infectious process has been commonly associated with
