Flight physiology: science of air travel with neonatal transport considerations.

Air transportation, by rotor wing and fixed wing, is a frequent method of neonatal transportation. There are many risks involved in air transportation. It is well documented that the safety of rotor wing flights has been questioned. Fixed wing transport is viewed as a safer mode of transportation. Air transportation has risks other than vehicle accidents, and increasing altitude encountered in both fixed and rotor wing transportation can cause significant distress to both the transport crew and the patient. Knowledge and awareness of these risks by the flight team can help to alleviate unneces

Controlling pandemic flu: the value of international air travel restrictions.

BACKGROUND: Planning for a possible influenza pandemic is an extremely high priority, as social and economic effects of an unmitigated pandemic would be devastating. Mathematical models can be used to explore different scenarios and provide insight into potential costs, benefits, and effectiveness of prevention and control strategies under consideration. METHODS AND FINDINGS: A stochastic, equation-based epidemic model is used to study global transmission of pandemic flu, including the effects of travel restrictions and vaccination. Economic costs of intervention are also considered. The dist

[Deep-vein thrombosis and pulmonary embolism due to air travel].

In recent years, the association between air travel and the incidence of deep-vein thrombosis or pulmonary embolism has become clearer. Epidemiologic studies reveal an increased relative risk of thromboembolism after flights of more than 8 hours and especially in subjects at higher risk for this disease, due, for example, to congenital thrombophilia or the use of oral contraceptives. However, the absolute risk of deep-vein thrombosis or pulmonary embolism after prolonged air travel is very small. Studies have shown that a combination of factors present during prolonged air travel may account f

Exposure to mumps during air travel--United States, April 2006.

The state of Iowa has been experiencing a large mumps outbreak that began in December 2005. As of April 10, 2006, a total of 515 possible mumps cases have been reported to the Iowa Department of Public Health (IDPH) during 2006. This outbreak has spread across Iowa, and mumps activity, possibly linked to the Iowa outbreak, is under investigation in six neighboring states, including Illinois (n = four), Kansas (n = 33), Minnesota (n = one), Missouri (n = four), Nebraska (n = 43), and Wisconsin (n = four) (CDC, unpublished data, April 10, 2006). The reasons for this outbreak are under investigat

Forecasting the geographical spread of smallpox cases by air travel.

Instituting air travel restrictions to slow the geographical spread of smallpox cases would have significant consequences and present serious logistical concerns. Public health decision makers must weigh the potential benefits of such restrictions against their negative impact. The goal of this research is to provide a basic analytical framework to explore some of the issues surrounding the use of air travel restrictions as a part of an overall containment strategy. We report preliminary results of a compartmental model for the inter-city spread of smallpox cases resulting from US domestic air

Hemorrhage within brain tumors in association with long air travel.

Two cases of hemorrhage within brain tumors after air travel are described. The hemorrhages occurred in a meningotheliomatous meningioma and in a melanoma metastasis after the patients had taken transatlantic flights lasting more than seven hours. The literature concerning bleeding associated with brain tumors is reviewed, and possible mechanisms by which air travel may have precipitated these hemorrhages, based on known physiological changes that occur in-flight, are discussed.

[Air travel and oxygen therapy for passengers with respiratory diseases].

The CME article addresses the requirements and recommendations for air travel suitability in patients with respiratory diseases such as COPD, asthma, obstructive sleep apnea syndrome (OSAS), obesity hypoventilation syndrome (OHS), interstitial lung diseases, and other lung function impairments. Due to the reduced oxygen partial pressure at flight altitude, hypoxemia in affected patients can worsen, potentially leading to serious symptoms and complications. Diagnostic algorithms, including the Hypoxic Challenge Test (HCT), are presented to assess air travel suitability and oxygen requirements,

Cellular and Molecular Mechanisms Leading to Air Travel-Induced Thrombosis.

Venous thromboembolism, characterized by deep vein thrombosis and pulmonary embolism, is the third cardiovascular disease in the world. Deep vein thrombosis occurs when a blood clot forms in areas of impaired blood flow, and it is significantly affected by environmental factors. Local hypoxia, caused by venous stasis, plays a critical role in deep vein thrombosis under normal conditions, and this effect is intensified when the Po2 decreases, such as during air travel or high-altitude exposure. The lower oxygen levels and reduced pressure at high altitudes further contribute to deep vein thromb

Suppressed DNA repair capacity in flight attendants after air travel.

Elevated cancer risk and compromised reproductive health have been well documented in flight attendants (FA), but the etiology remains unknown. Many studies using cell and animal models suggest that air travel related exposures might plausibly explain the adverse health outcomes observed in flight crew, but our understanding of the underlying biological mechanisms is incomplete. During air travel, FA are constantly exposed to complex mixtures of mutagens in the flight cabin that may contribute to genomic instability by inducing DNA damage and interfering with DNA repair. Defects in DNA repair

Air Travel-Related Spontaneous Pneumothorax in Diffuse Cystic Lung Diseases.

PURPOSE OF REVIEW: Spontaneous pneumothorax (SP) is a common manifestation of patients with diffuse cystic lung diseases (DCLDs) such as lymphangieoleiomyomatosis (LAM), pulmonary Langerhans cell histiocytosis (PLCH) and Birt-Hogg-Dubé syndrome (BHD). Air travel may pose an additional risk for the development of SP. Here, we summarize the literature pertaining to air travel related SP in DCLDs in order to assist patients and clinicians in appropriate decision-making with regards to air travel. RECENT FINDINGS: Several recent studies have estimated that the per-flight risk of SP in patients wi

Prevention of Medical Events During Air Travel: A Narrative Review.

Prior to traveling, and when seeking medical pretravel advice, patients consult their personal physicians. Inflight medical issues are estimated to occur up to 350 times per day worldwide (1/14,000-40,000 passengers). Specific characteristics of the air cabin environment are associated with hypoxia and the expansion of trapped gases into body cavities, which can lead to harm. The most frequent medical events during air travel include abdominal pain; ear, nose, and throat pathologies; psychiatric disorders; and life-threatening events such as acute respiratory failure or cardiac arrest. Accessibility to air travel correlates strongly with increasing melanoma incidence.

As the cost of air travel has decreased substantially in the USA and Europe over the past few decades, leisure travel to vacation destinations during the winter months has expanded significantly. This trend has probably increased the incidence of significant ultraviolet radiation exposure and sunburn in a broader population who could not previously afford this kind of travel. The purpose of this study was to analyse the correlation between increasing accessibility to air travel and melanoma incidence. This ecological study surveyed air travel patterns and melanoma incidence over the past three

Activation of coagulation system during air travel: a crossover study.

BACKGROUND: There is an increased risk of venous thrombosis after air travel, but the underlying mechanism is unclear. Our aim was to ascertain whether flying leads to a hypercoagulable state. METHODS: We did a crossover study in 71 healthy volunteers (15 men, 56 women), in whom we measured markers of activation of coagulation and fibrinolysis before, during, and after an 8-h flight. The same individuals participated in two control exposure situations (8-h movie marathon and daily life) to separate the effect of air travel on the coagulation system from those of immobilisation and circadian r

Air travellers' awareness of the preventability of otic barotrauma.

OBJECTIVE: Otic barotrauma is common among air travellers and can cause severe otalgia, perforation of the tympanic membrane and hearing loss. Many prevention measures exist, with varying evidence to support their use. There are no data to establish if air travellers are aware of them or indeed use them. We aimed to establish air travellers' knowledge of such prevention measures. METHODS: We surveyed air travellers at two UK airports by means of a questionnaire. Answers to the questionnaire were collected over a two-week period. RESULTS: Overall, 179 air travellers with a mean age of 28 year
