A battery of liver function tests was carried out before operation and on the 3rd--5th and 13th--15th postoperative days in patients anaesthetised with continuous infusion of thiopentone, Althesin or etomidate for an intermediate operation. Some derangement of enzyme activity was found in one quarter to one third of the patients, and was most marked after Althesin. It is a recently identified specific anatomical and rhythmic entity which gives rise to ventricular tachycardia in the young patient. The rates of degradation of the steroid anaesthetic alphaxalone (3 alpha-OH-5 alpha pregnane-11,20 dione) have been studied in vitro using isolated hepatocyte and microsome preparations. Evidence is presented that alphaxalone is metabolized rapidly by the hepatic mixed function oxygenase system. Induction of cytochrome P450 in vivo leads to an increase in the maximum rate of degradation of alphaxalone in both in vitro systems. The anaesthetic properties of pregnanolone emulsion, a new steroid preparation for intravenous administration, were evaluated in rats and compared to those of Althesin, propofol, thiopentone and midazolam. The EEG was continuously recorded on 43 patients who received Althesin. A basic pattern for this drug, typical of most general anaesthetic agents at a level of light surgical anaesthesia, was established. Ten patients with small supratentorial tumours were studied during craniotomy. Cerebral blood flow (CBF) was measured in the contralateral hemisphere by a modification of the Kety and Schmidt technique using xenon-133 i.v. The effects on baroreflex control of heart rate, of two infusion rates of Althesin (18 and 36 micrograms kg-1 min-1 as total steroid) to supplement 66% nitrous oxide in oxygen anaesthesia, have been studied in six younger (mean age 35.5 yr) and six older (mean age 61.0 yr) patients. Steady-state anaesthesia at both infusion rates was associated with diminished baroreflex sensitivity and resetting of the reflex to allow lower arterial pressures to be maintained without tachycardia.

The effects of acute and long-term lithium (LiCl) and rubidium chloride (RbCl) treatments on the sleeping time caused by intravenous thipentone, methohexitone, ketamine, propanidid. Thirty patients for elective lower segment Caesarean section were randomly divided into two groups. One group received 50 mulitre/kg of Althesin for induction of anaesthesia and the other, 100 mulitre/kg. The two maternal groups were similar. There was no significant difference in the clinical or biochemical status of the infants except that infants in the low dose group had a significantly higher PO2 in the umbilical venous and arterial blood.
