Surgical crown lengthening: evaluation of the biological width.

BACKGROUND: Previous surgical crown lengthening studies have investigated positional changes of the free gingival margin but not the biological width. Histological studies utilizing animal models have shown that postoperative crestal resorption allowed reestablishment of the biological width. However, very little work has been done in humans. Therefore, the purpose of this study was to evaluate the positional changes of the periodontal tissues, particularly the biological width, following surgical crown lengthening in human subjects. METHODS: Twenty-three (23) patients who needed surgical cro

Experimental tooth movement into bone induced by recombinant human bone morphogenetic protein-2.

OBJECTIVE: The development of biomaterials for secondary bone grafting to the alveolar cleft, recombinant human morphogenetic protein-2 (rhBMP-2) may have great potential to promote osseous regeneration. The purpose of this study was to investigate the osteoinductive activity of low-dose rhBMP-2 (10 microg/100 microL) and the biological response of newly generated bone to orthodontic stimulation, compared with previous results using high-dose rhBMP-2 (40 microg/100 microL). METHOD: After extraction of the maxillary first premolars, bone defects were surgically created in eight adult beagle do

A radiological method to evaluate alveolar bone regeneration in the Chacma baboon (Papio ursinus).

INTRODUCTION: The evaluation of alveolar bone healing may have a role in dental implantology, in prosthodontics in the post-extraction phase and in monitoring fracture repair. There are several radiological techniques described to evaluate alveolar bone regeneration. However, most are expensive and time consuming. OBJECTIVES: To develop and evaluate a radiological method utilising readily available equipment to measure alveolar bone regeneration. MATERIALS AND METHODS: An apparatus was designed to enable the acquisition of standardized x-ray images, consisting of a disposable impression tray

Application of calcium sulfate in surgical-orthodontic treatment of impacted teeth: a new procedure to control hemostasis.

PURPOSE: Calcium sulfate (CaS) is a simple, biocompatible material with a very long, safe clinical history in several different fields of medicine. CaS is a rapidly resorbing material that leaves behind a calcium phosphate lattice, which promotes bone regeneration and hemostasis. The aim of this study was to perform a clinical evaluation of a possible hemostatic effect of calcium sulfate hemihydrate (CaSO(4)), commonly known as plaster of Paris, in the surgical and orthodontic treatment of impacted teeth. MATERIALS AND METHODS: Forty-three patients with 66 impacted cuspids and premolars were

Excessive gingival display--etiology, diagnosis, and treatment modalities.

Extensive exposure of the gingiva during a smile, called excessive gingival display, may be a point of concern for both patients and clinicians. Patients often present to the dental clinic seeking a solution to their "gummy" appearance. A clinician must fully understand the various factors involved in this situation, to provide patients with an appropriate answer. Thorough examination followed by the right diagnosis is imperative for achieving an esthetic and predictable result in the treatment of such situations. The aim of this article is to discuss the various aspects of excessive gingival

Subgingival acellular dermal matrix allograft for the treatment of gingival recession: a case report.

Root coverage procedures using subgingival soft tissue grafts or guided tissue regeneration have attracted much interest within the past 2 decades. Recently, acellular dermal matrix allograft (ADMA) has been introduced as a substitute for palatal donor tissue in gingival augmentation procedures. This study was undertaken to examine the potential of ADMA to be used as a substitute for autogenous connective tissue graft material in a root coverage procedure in a case with moderate gingival recession combined with reduced keratinized attached gingiva. After thorough root planing and conditioning

Successive removal of periodontal tissues. Marginal healing without plaque control.

The aim of the present study was to compare periodontal healing after successive removal of periodontal tissue components, from the alveolar bone to the dentin surface. The prevailing tissue reaction when adhering PDM was left on the exposed roots was that most of the bone tissue that had been removed from the buccal surfaces had regenerated and the integrity of the PDM between the new alveolar bone and cementum surface had been reestablished. On exposed etched cementum surfaces, 2 prevailing healing results were recorded. In half the number of the roots, the root surfaces were covered by conn

An update on crown lengthening. Part 1: Gingival tissue excess.

This is the first article in a two-part series which aims to provide an overview of the different techniques used to increase clinical crown height. In the first paper, the focus will be on the management of patients who present with gingival tissue excess. The different aetiologies are discussed and illustrated with clinical cases, following which a range of procedures that may be employed in the management of these patients are presented. With an increasingly ageing population, more patients are taking regular medications prescribed from their general medical practitioner, and so having a wo

Exposure of unerupted palatal canines: a survey of current practice in the United Kingdom, and experience of a gingival-sparing procedure.

Patients with impacted ectopic canines often present to Orthodontic and Oral and Maxillofacial units, with 85% being displaced palatally. Current orthodontic opinion suggests that the creation of adequate space ultimately allows the spontaneous eruption of most impacted canines, but we are often requested to operate to facilitate their eruption. There are several approaches, techniques that vary depending on whether orthodontically assisted or spontaneous eruption is expected. There are few publications that describe either the techniques or their merits. We have used a gingival-sparing techni

Tooth movement - clinical implications.

The tissue-based phenomena and the tissue interactions responsible for tooth displacement can be modulated by varying the intensity of the applied forces or by means of medicated molecules absorbed by the patient or prescribed by the orthodontist. However, harmful side-effects such as inflammatory root resorption or replacement resorption (ankylosis) can occur requiring appropriate management on the part of the practitioner.

Effect of additional surgical procedures on root development of transplanted immature third molars.

The aim of this investigation was to determine the influence of additional surgical procedures on the root development of transplanted teeth. The study sample consisted of 90 immature third molars transplanted in 88 patients. All transplanted teeth were at root development stages 3 to 4. Free bone autografts were used in 23 cases (bone autograft group), mainly because of vertical atrophy of the alveolar process. A splitting osteotomy of the alveolar process was performed in 25 cases with marked horizontal atrophy (osteotomy group). Forty-two teeth transplanted into a fresh extraction site imme

Root reshaping: an integral component of periodontal surgery.

It is the aim of this article to present a surgical option to the traditional method of returning lost biologic width where invasion of the junctional and/or connective tissue attachment associated with a tooth has occurred. The alternative to conventional osseous resection involves reshaping the existing tooth surface in combination with conservative removal of the supporting alveolar bone to create the width needed for the restoration to be biologically acceptable. This procedure accomplishes several goals: (1) minimum supporting bone is removed; (2) deleterious root surface anatomy, such as

Establishment of a rat model for alveolar cleft with bone wax.

PURPOSE: Although treatment of cleft lip and cleft palate is becoming standardized, treatment of alveolar cleft remains controversial. Thus, preclinical animal work remains necessary to improve clinical outcome. This study established an alveolar cleft model in the rat. MATERIALS AND METHODS: Maxillary first molars were extracted to create a 4- × 4- × 3-mm complete alveolar cleft on the right and left sides in 25 8-week-old Sprague-Dawley rats. The left cleft was filled with bone wax and the right side remained untreated. Animals were sacrificed at 0, 4 and 8 weeks after surgery. Harvested al
