Inpatient utilization among Medicaid members with substance use disorder in Oregon.

BACKGROUND: In the United States, substance use disorders (SUDs) remain a critical public health concern with significant morbidity and mortality. Oregon exemplifies this crisis with high rates of illicit drug use and overdose deaths, compounded by systemic challenges like behavioral health workforce shortages. This study's objective is to characterize patterns of acute care utilization and ED visit and admission reasons among Medicaid enrollees with opioid use disorder, stimulant use disorder, and/or unintentional substance overdoses in the Portland metro area. METHODS: We conducted a cross-

Vulnerable populations at risk of potentially avoidable hospitalizations: the case of nursing home residents with Alzheimer's disease.

This study explores whether nursing home residents with Alzheimer's disease and related dementias (ADRD) are affected differently by facility-level risk factors of ambulatory care-sensitive (ACS) conditions, a measure of timely access to medical care. Three years of quarterly Medicaid reimbursement data from over 525 Massachusetts nursing homes were linked with four years of Medical Provider Analysis and Review hospital claims data and facility-level attribute data to investigate whether facility effects differed by resident ADRD status. The findings suggest that nursing home residents with AD

Utilization Patterns Among Heterogeneous Subgroups of Homebound Older Adults: A Latent Class Analysis.

BACKGROUND: Homebound older adults represent a high-risk, high-needs population characterized by significant clinical and sociodemographic heterogeneity. Little is known about how such heterogeneity shapes care needs and utilization patterns. We characterized utilization patterns among distinct subgroups of homebound older adults and examined associations between levels of outpatient care and emergency department (ED) utilization. PARTICIPANTS AND SETTING: We used nationally representative data from the National Health and Aging Trends Study (NHATS) linked to Medicare Fee-For-Service (FFS) cl

Transforming GP practices into multiprofessional primary care centers: The role of nursing professionals (PRIMA) - study protocol for a multicenter non-randomized, controlled, parallel-group intervention study.

BACKGROUND: Germany faces increasing challenges in primary care due to demographic change, workforce shortages, and the growing prevalence of chronic conditions. Integrating nursing professionals into primary care teams may improve continuity, comprehensiveness, and efficiency of care, but evidence for the German setting remains limited. The PRIMA project (DRKS00039104) implements a new care model that transforms general practices into multiprofessional primary care centers integrating specially trained nursing professionals. The objective is a productivity- and quality-oriented reorganization

Avoidable Hospitalizations in Persons With Rheumatoid Arthritis: A Population-Based Study Using Administrative Data.

OBJECTIVE: We estimated incidence rates of avoidable hospitalizations by persons with rheumatoid arthritis (RA) relative to the general population. METHODS: We identified individuals meeting a validated case definition for RA based on International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) and ICD-10-CA codes in years 2002 to 2023. Four general population controls were matched to each RA case by age and sex. We identified hospitalizations for ambulatory care sensitive conditions (ACSCs), including grand mal seizures, chronic lower respiratory diseases, asthm

Using digital assessment technology to detect neuropsychological problems in primary care settings.

INTRODUCTION: Screening for neurocognitive impairment and psychological distress in ambulatory primary and specialty care medical settings is an increasing necessity. The Core Cognitive Evaluation™ (CCE) is administered/scored using an iPad, requires approximately 8 min, assesses 3- word free recall and clock drawing to command and copy, asks questions about lifestyle and health, and queries for psychological distress. This information is linked with patients' self- reported concerns about memory and their cardiovascular risks. METHODS: A total of 199 ambulatory patients were screened with th

Gestational pyelonephritis as an indicator of the quality of ambulatory maternal health care services.

OBJECTIVE: Inpatient conditions that might be avoided through improved outpatient services are called Ambulatory Care Sensitive Indicators, and they include pyelonephritis in nonpregnant adults. No such indicators have been developed for pregnant women. We examine whether hospital-specific rates of gestational pyelonephritis may serve as a measure of the quality of ambulatory maternal care. METHODS: The California Department of Health Services provided an administrative data set linking maternal and newborn delivery records for 1997 with antepartum hospital admissions. We created a "low-risk"

Hospitalization for urinary tract infections and the quality of preventive health care received by people with spina bifida.

BACKGROUND: The preventive health care needs of people with disabilities often go unmet, resulting in medical complications that may require hospitalization. Such complications could be due, in part, to difficulty accessing care or the quality of ambulatory care services received. OBJECTIVE: To use hospitalizations for urinary tract infections (UTIs) as a marker of the potential quality of ambulatory care services received by people affected by spina bifida. METHODS: MarketScan inpatient and outpatient medical claims data for 2000 through 2003 were used to identify hospitalizations for UTI,

Addressing inequalities in medical workforce distribution: evidence from a quasi-experimental study in Brazil.

BACKGROUND: Brazil faces huge health inequality challenges since not all municipalities have access to primary care physicians. The More Doctors Programme (MDP), which started in 2013, was born out of this recognition, providing more than 18 000 doctors in the first few years. However, the programme faced a restructuring at the end of 2018. METHODS: We construct a panel municipality-level data between 2008 and 2017 for 5570 municipalities in Brazil. We employ a difference-in-differences empirical approach, combined with propensity score matching, to study the impacts of the programme on hospi

Association of living alone and hospitalization among community-dwelling elders with and without dementia.

BACKGROUND: Older persons account for the majority of hospitalizations in the United States.1 Identifying risk factors for hospitalization among elders, especially potentially preventable hospitalization, may suggest opportunities to improve primary care. Certain factors-for example, living alone-may increase the risk for hospitalization, and their effect may be greater among persons with dementia and the old-old (aged 85+). OBJECTIVES: To determine the association of living alone and risk for hospitalization, and see if the observed effect is greater among persons with dementia or the old-ol

Cellulitis: A Review.

IMPORTANCE: Cellulitis is an infection of the deep dermis and subcutaneous tissue, presenting with expanding erythema, warmth, tenderness, and swelling. Cellulitis is a common global health burden, with more than 650,000 admissions per year in the United States alone. OBSERVATIONS: In the United States, an estimated 14.5 million cases annually of cellulitis account for $3.7 billion in ambulatory care costs alone. The majority of cases of cellulitis are nonculturable and therefore the causative bacteria are unknown. In the 15% of cellulitis cases in which organisms are identified, most are due

Using Avoidable Admissions to Measure Quality of Care for Cardiometabolic and Other Physical Comorbidities of Psychiatric Disorders: A Population-Based, Record-Linkage Analysis.

OBJECTIVE: Quality of care for comorbid physical disorders in psychiatric patients can be assessed by the number of avoidable admissions for ambulatory care sensitive (ACS) conditions. These are admissions for physical conditions that, with appropriate primary care, should not require inpatient treatment. Avoidable admissions for ACS conditions feature prominently in Australia's National Health Performance Framework and have been used to assess health care provision for marginalized groups, such as Indigenous patients or those of lower socioeconomic status. They have not been applied to people

Co-ordination of health care: the case of hospital emergency admissions.

The recognition that chronic care delivery is suboptimal has led many health authorities around the world to redesign it. In Norway, the Department of Health and Care Services implemented the Coordination Reform in January 2012. One policy instrument was to build emergency bed capacity (EBC) as an integrated part of primary care service provided by municipalities. The explicit aim was to reduce the rate of avoidable admissions to state-owned hospitals. Using five different sources of register data and a quasi-experimental framework-the "difference-in-differences" regression approach-we estimat

Impact of a Medicaid primary care provider and preventive care on pediatric hospitalization.

OBJECTIVE: This study evaluates the impact that a Medicaid managed care program had on avoidable hospitalization, a form of health care misuse that we hypothesize can be reduced by improved access to and quality of primary care in the context of a managed care program. Ambulatory care sensitive (ACS) hospitalizations, a previously defined categorization of hospitalization, as well as all pediatric hospitalizations were also studied. INTERVENTION: The Maryland Access to Care (MAC) was a fee-for-service, gatekeeper, Medicaid managed care program with assigned primary medical providers and requi

A Protocol for Comprehensive Analysis of Gait in Individuals with Incomplete Spinal Cord Injury.

This is a protocol for comprehensive analysis of gait and affecting factors in individuals with incomplete paraplegia due to spinal cord injury (SCI). A SCI is a devastating event affecting both sensory and motor functions. Due to better care, the SCI population is changing, with a greater proportion retaining impaired ambulatory function. Optimizing ambulatory function after SCI remains challenging. To investigate factors influencing optimal ambulation, a multi-professional research project was grounded with expertise from clinical rehabilitation, neurophysiology, and biomechanical engineerin
