AIM: This scoping review aimed to map and synthesise the available evidence on the perioperative management of patients undergoing anorectal surgery in a day-surgery setting. This study creates the first unified, evidence-based framework for day-surgery anorectal care. It addresses gaps by integrating existing recommendations and establishing new protocols for positioning and postoperative care (diet, activity, bowels) tailored for rapid discharge. By standardising workflows for the day-surgery setting, it aims to reduce complications and unplanned admissions.
DESIGN: A scoping review methodo

BACKGROUND: A vasovagal reaction is a commonly encountered event in outpatient procedures. There is a paucity of discussion on vasovagal reactions (VVRs) in the dermatologic surgery literature. However, recent investigations in the physiology, evaluation, and treatment of VVRs have been reported in other specialties.
OBJECTIVE: A comprehensive review of the physiology, evaluation, treatment, and prevention of VVRs.
MATERIALS AND METHODS: A search as performed using the PubMed/MEDLINE databases. Search terms included "vasovagal," "vasovagal reaction," "syncope," "reflex syncope," "neurocardio

INTRODUCTION: The actual high hospitalization costs have encouraged a growing attention towards reducing hospital stay. Nowadays, many simple surgical procedures are carried out in a one-day surgery regimen . A shorter hospital stay brings many advantages for the patients: lesser inconvenience, a lower risk of hospital infection and an earlier return to work. In proctology, day surgery polices are still uncommon because surgeons fear possible complications. In this paper we sum up our 15 years experience, stressing the possibility to perform even complex procedures in local-regional anesthesia

A surgical day care unit was established at the Kaplan Hospital, Rehovot, in 1975. This study includes 5,427 patients who underwent ear, nose and throat, gynecologic, eye and plastic surgery as well as other surgical procedures in the unit. The organization and staffing of the unit are also described. The feasibility of performing minor to intermediate surgical procedures requiring general anesthesia on a day basis is demonstrated. The experience gained shows that high medical standards are achieved with low resource expenditure. The cost per patient is significantly reduced. Absenteeism from

Postoperative and postdischarge nausea and vomiting are common, costly complications that delay recovery and reduce satisfaction after ambulatory surgery. They stem from complex neurotransmitter pathways targeted by antiemetics. Accurate risk prediction is key, and multimodal prophylaxis is most effective.

BACKGROUND: Ambulatory thyroid surgery has been increasingly performed in recent years. However, the feasibility of the ambulatory transoral endoscopic thyroidectomy vestibular approach (TOETVA) has not been evaluated. We aimed to evaluate the safety, economy, and mental health outcomes of ambulatory TOETVA.
METHODS: We retrospectively reviewed the data of patients who underwent TOETVA between March 2019 and August 2022. The procedure was performed by a skilled surgical team from the Department of Thyroid Surgery of the affiliated Yantai Yuhuangding Hospital of Qingdao University. Patients we

BACKGROUND: Peripheral regional anesthesia and analgesia may increase the efficiency of ambulatory surgical centers by reducing pain and preventing nausea and vomiting, which are important modifiable causes of prolonged postanesthesia care unit (PACU) length of stay. We hypothesized that the use of peripheral nerve blocks (PNB) was associated with shorter PACU length of stay in ambulatory surgery.
METHODS: In this retrospective cohort study, we analyzed data from adult ambulatory surgical cases, in which PNB was a viable anesthetic option (ie, was routinely performed for these procedures), at

Standard of care for cleft lip repair has included preadmission testing, surgical correction, and postoperative hospital care. Driven not by managed care economics but to speed the safe home care of infants by parents, the authors have gained experience in ambulatory cleft lip repair. In this retrospective study the authors evaluated the outcome of patients who underwent ambulatory cleft lip repair compared with those patients who were hospitalized after surgery. From 1989 to 1998, 24 cleft lip repairs in 24 patients performed by the senior author were evaluated. Two groups were treated. Group

PURPOSE OF REVIEW: The increasing number of procedures done in the ambulatory surgical setting necessitates the need for analgesic modalities that enable the management of postsurgical pain with fast onset, predictable duration of action, and minimal need for management of undesirable side-effects.
RECENT FINDINGS: The novel strategies for administration of local anesthetics in the ambulatory setting include prolonging their analgesic action at the site of surgical trauma while reducing systemic effects that result from their metabolism. Development of opioids aims to address receptor sites t

BACKGROUND: Outpatient spine surgery has expanded over the past decade, but this transition has largely involved anterior cervical procedures. Safety data regarding anterior lumbar interbody fusion (ALIF) and artificial disc replacement (ADR) performed in ambulatory surgery centers remain limited.
OBJECTIVE: In this study, we investigated the safety and efficacy of anterior lumbar surgeries in outpatient settings based on operative and early postoperative outcomes.
METHODS: A total of 33 patients underwent anterior lumbar surgery: ALIF or of 1 to 3 levels. Patient data were collected and ana

Introduction: Older adults represent a growing proportion of surgical patients. Ambulatory surgery is increasingly used for its efficiency and lower costs. This study assessed its safety in individuals over 75 compared to younger cohorts. Methods: A retrospective observational study was conducted in a dedicated Ambulatory Surgery Unit, including patients over 75 years of age who underwent general surgical procedures under general anesthesia from January 2020 to January 2023. Ambulatory surgery was defined as procedures requiring less than 24 hours of hospital stay, including cases with overnig

Extracorporeal shock wave lithotripsy has become the standard method of treating renal stone disease. The appropriate choice of anaesthetic technique depends on the shock wave technic mode. Recent modifications of lithotriptors have reduced the physiological consequences of the procedure and most often these treatments can be performed without local or general anaesthesia. However, some sedation/analgesia may be necessary and a good knowledge of anaesthesia for lithotripsy remains most useful so as to meet any particular requirements.

The aim of the study was the evaluate of results of 2000 surgical operations for ano-rectal disease performed in the day-surgery setting (7-24 hours hospital stay) with improvement of both cost effectiveness and patient comfort. From January 1980 to December 1998, 2000 patients underwent surgical operations: 1011 for haemorrhoids; 708 for anal fissure; 172 for fistula in ano; 80 for pylonidal disease; and 45 for anal stenosis. 97.6% of patients were operated on with loco-regional anaesthesia; the others with narcosis and peripheral anaesthesia. The hospital-stay was 24 hours in 697 patients (3

The use of regional anaesthesia in day care practice is discussed. Five criteria of discharge are described: the four "A"s: awake, ambulation, alimentation, analgesia plus micturition. Complementary sedation with the regional block, if needed, should be midazolam and fentanyl. These drugs should be titrated in order to administer the efficient minimal dosage. The main techniques of regional anaesthesia are as follows: for upper limbs, intravenous regional anaesthesia, axillary and interscalenic brachial plexus blocks; for the lower limbs epidural and spinal blocks. However, urinary retention a

Anatomically, the indications of ambulatory phlebectomy (A.P.) concern the entire varicose network of the lower extremities. The presence of a complication in the course of varicose veins disease (ulcer, hypodermatitis, or post-phlebitic varices) is not a contra-indication to A.P. The aims are: complete eradication of the varices, stability of the long-term result, harmless nature of the treatment and the aesthetic quality of the result.

BACKGROUND: In November 2017, a working feasibility analysis commenced of a local anaesthetic endonasal procedures out-patient clinic service at Freeman Hospital, Newcastle upon Tyne. Fundamental to introducing an innovative ambulatory out-patient practice is the development of a novel local safety standard for invasive procedures to support this service.
OBJECTIVE: This paper presents the new safety standard developed for this purpose and implemented in our institution.
CONCLUSION: Increasingly, there is a shift toward ambulatory services, directed by patient choice, technological advances
