Effect of the antipsychotic agent amisulpride on glucose lowering and insulin secretion.

AIMS: To investigate the effects of the second generation antipsychotic (R/S)-amisulpride, and the chirally purified enantiomers, on glucose homeostasis in diet-induced obese (DIO) mice. METHODS: Normal and DIO mice were treated with pharmacologically relevant doses of amisulpride prior to oral glucose tolerance tests (OGTTs). Blood glucose, insulin, glucagon-like peptide-1, prolactin and amisulpride drug levels were determined. RESULTS: Racemic amisulpride significantly reduced glucose excursions during OGTT in both normal and DIO mice. This potent effect was preserved with the 'off-isomer'

Amisulpride treatment of adolescent patients with schizophrenia or schizo-affective disorders.

In this article, our aim was to share our clinical experience regarding the effectiveness and the side effects of amisulpride treatment in adolescent cases. We reviewed all the cases who had amisulpride treatment during their stay in a newly opened in-patient unit, and three female cases who had undergone amisulpride treatment. We observed that the drug was effective and caused side effects like galactorrhea due to increased prolactin levels, which were not severe enough to terminate the treatment. Our clinical experience provides preliminary evidence that amisulpride might be an effective ant

Hyperprolactinemia induced by low-dosage amisulpride in Korean psychiatric patients.

AIM: Amisulpride at low dosages enhances dopaminergic neurotransmission by preferentially blocking presynaptic D2/D3 receptors. Thus, low dosages of amisulpride are expected not to increase prolactin levels. The aim of this study was to examine whether low dosages of amisulpride can increase serum levels of prolactin or not clinically in Korean patients. METHOD: Serum prolactin levels were measured in 20 Korean patients (12 men and eight women) with various diagnoses who were treated with less than 300 mg of amisulpride per day. RESULTS: The mean dosage of amisulpride was 195.0 ± 51.0 mg/day

Amisulpride vs. fluoxetine treatment of chronic fatigue syndrome: a pilot study.

Different pharmacologic agents have been evaluated in the treatment of Chronic Fatigue Syndrome (CFS), albeit with moderate efficacy. Among the compounds thought to present with potential to be efficacious in CFS patients stands out low-dose amisulpride, a substituted benzamide that has been shown to be an useful treatment for conditions which exhibit some overlap with CFS such as dysthymia and somatoform disorders. We thus recruited forty non-depressed CFS patients that were randomized to receive either amisulpride 25mg bid, or fluoxetine 20mg uid; all subjects were un-blinded to the treatmen

How do we choose between atypical antipsychotics? The advantages of amisulpride.

Clinician choice of an atypical antipsychotic may depend on a number of factors such as perceived efficacy, tolerability and cost. It is also important that the choice of treatment takes into consideration the previous response to treatment, experience of side-effects and personal clinical characteristics. The receptor-affinity profiles of the atypical antipsychotics differ; with the exception of amisulpride, a selective D2/D3 antagonist, all the atypical antipsychotics exhibit a greater affinity for the serotonin-2A receptors than dopamine receptors. However, there is no evidence that the var

A systematic review and combined meta-analysis of concentration of oral amisulpride.

AIMS: Amisulpride, a first-line schizophrenia treatment, has shown large interindividual variability in plasma/serum levels, often outside the reference range (100-320 ng/mL). This study aims to clarify the impact of dose, sex, age and related factors for the interpatient variability in amisulpride plasma/serum concentration. METHODS: Both English and Chinese databases were searched from their inception to May 16, 2019, using the terms: amisulpride and (plasma OR serum OR blood OR "drug monitoring" OR concentration). Studies reporting concentrations and either a dose, associated factor, clini

False-positive buprenorphine by CEDIA in patients prescribed amisulpride or sulpiride.

Buprenorphine is a potent partial opioid agonist that is analyzed in urine to (i) monitor adherence to maintenance or detoxification therapy and (ii) detect illicit use. Buprenorphine analysis is commonly conducted on urine by immunoassay, but is subject to cross-reactivity from other drugs/drug metabolites, including morphine, codeine and dihydrocodeine. This study reports false-positive buprenorphine analysis [Thermo Fisher Scientific cloned enzyme donor immunoassay (CEDIA)] in patients who denied unauthorized buprenorphine use prior to sampling, but who had been prescribed amisulpride. Treatment with amisulpride of addicted patients to psychoactive substances and psychotic symptoms.

INTRODUCTION: There is ever more available information on the effectiveness of second-generation antipsychotic drugs used in addictive behaviour patients with psychotic symptoms. Due to its characteristics, Amisulpride is a medicine that can be considered as a valid therapeutic option to treat this group of patients. OBJECTIVE: To assess the value of Amisulpride to treat patients with addictive behaviours, and the associated morbidity. METHOD: An experimental, prospective study was conducted. A total of 97 ambulatory patients, who were initiating, or already receiving, treatment at the Addic

Amisulpride the 'atypical' atypical antipsychotic--comparison to haloperidol, risperidone and clozapine.

INTRODUCTION: Amisulpride's high and selective affinity for dopamine D2/3 (Ki 1.3/2.4 nM) receptors, lack of affinity for serotonin receptors, and its unusually high therapeutic doses (400-800 mg/day) makes it unique among atypical antipsychotics and prompted us to compare its actions with other antipsychotics in animal models. METHODS: Amisulpride's effects on amphetamine and phencyclidine induced locomotor activity (AIL/PIL), conditioned avoidance response, catalepsy (CAT), subcortical Fos expression, and plasma prolactin was correlated to its time-course striatal D2/3 and prefrontal 5-HT2

Amisulpride--an open clinical study of a new benzamide in schizophrenic patients.

In pharmacological screening amisulpride produces no catalepsy, no inhibition of stereotypic movements, yet a blockade of drug-induced vomiting. During an open clinical trial lasting 4 weeks, 14 patients (13 schizophrenics) were treated with the compound. The (BPRS-) syndromes anxiety/depression, thought disorder, activity, hostility and the global score showed significant improvement. With the AMDP system significant changes were seen in the paranoid-hallucinatory, manic, depressive and hostility syndromes as well as in the global score. No changes were revealed in anergia (BPRS) and apathia

A Case of Probable Amisulpride Induced Mania after Eight Months of Therapy.

Development of manic symptoms during treatment with atypical antipsychotics can be a troublesome side effect that has been described with most atypical antipsychotics. However, reports of amisulpride induced mania have been rare. Here, we report the case of an 18-year-old male patient diagnosed with schizophrenia, who developed manic symptoms while on treatment with amisulpride. While previous reports have described occurrence of mania within days to three months of treatment with amisulpride, we report a case where manic symptoms occurred after around eight months of therapy. We have also att

A population approach to characterise amisulpride pharmacokinetics in older people and Alzheimer's disease.

INTRODUCTION: Current prescribing guidelines for the antipsychotic amisulpride are based largely on pharmacokinetic (PK) studies in young adults, and there is a relative absence of data on older patients, who are at greatest risk of developing adverse events. METHODS: This study aimed to develop a population PK model for amisulpride specifically in older people, by combining data from a richly sampled phase 1, single (50 mg) dose study in healthy older people (n = 20, 65-79 years), with a clinical dataset obtained during off label, low-dose (25-75 mg daily) amisulpride prescribing in older pe

Atypical antipsychotics--effects of amisulpride on salivary secretion and on clozapine-induced sialorrhea.

OBJECTIVE: Amisulpride is suggested for treatment of clozapine-induced sialorrhea. However, objective measurements of its effectiveness are lacking and, preclinically, amisulpride has no effect. We currently hypothesise that amisulpride acts by reducing the nervous- rather than the clozapine-driven salivary secretion. MATERIAL AND METHODS: Effects of intravenous amisulpride (as well as of clozapine and raclopride, a dopamine D2/D3 antagonist) were investigated in rats, including those subjected to chronic preganglionic parasympathetic denervation (submandibular glands) or combined postganglio

Pergolide as adjuvant therapy to amisulpride in the treatment of negative and depressive symptoms in schizophrenia.

Negative and cognitive symptoms of schizophrenia are associated with a hypodopaminergic state in the frontal cortex and do not respond to neuroleptics equally well as positive symptoms. Therefore pharmacological strategies, which increase dopamine metabolism in the mesocortical pathways, may prove beneficial to ameliorate these symptoms. We report on a case of a patient with paranoid schizophrenia, who still presented negative and depressive symptoms during treatment with amisulpride for more than 6 weeks. We prescribed pergolide (a mixed D1/D2 agonist) as adjuvant therapy to treat these sympt

A possible effect of amisulpride on a prolactinoma growth in a woman with borderline personality disorder.

The authors report the case of a woman with borderline personality disorder who developed a prolactinoma probably induced by amisulpride. This patient was treated with 300 mg of amisulpride per day for a delirium. Her brain MRI was normal. Four months later this patient developed a micro-adenoma (5mm) on the right side of the pituitary gland with amenorrhea and galactorrhea. Amisulpride treatment was withdrawn and replaced by a treatment of quetiapine 100 mg day(-1). The hyperprolactinemia symptoms disappeared. This report shows that the short-term use of amisulpride treatment was linked to an

Optimizing Intranasal Amisulpride Loaded Nanostructured Lipid Carriers: Formulation, Development, and Characterization Parameters.

BACKGROUND: Nanostructured lipid carriers (NLCs) are lipid-based nanoparticles composed of a mixture of solid and liquid lipids, which are stabilized by the outer surface of a surfactant. OBJECTIVES: This research aimed to prepare intranasal nanostructured lipid carriers loaded with amisulpride to enhance its dissolution and bioavailability using different formulation compositions. METHODS: Amisulpride nanostructured lipid carriers were formulated using ultra-sonication methods. Solid lipids like stearic acid, palmitic acid, and glyceryl monostearate were used, while liquid lipids like oleic

[Treatment resistant schizophrenia, residual subtype: symptom improvement after additional treatment with amisulpride].

BACKGROUND: Therapy resistant, severe residual schizophrenia poses major challenges for the psychiatric pharmacological treatment. METHOD: We report about a 44-year old schizophrenic male who has been detained in a closed forensic ward for 10 years, following a conviction in one manslaughter case. Despite various pharmacological treatment efforts severe residual schizophrenia remained. RESULT: Adding Amisulpride to the ongoing treatment with Olanzapine achieved a noticeable improvement of symptoms, resulting in the possibility of relaxing imprisonment conditions. CONCLUSION: A combination o

Population pharmacokinetic modeling of amisulpride drug interactions and a transporter binding analysis.

PURPOSE: The combination of amisulpride and metformin is common in schizophrenia, but their potential drug-drug interaction has been largely overlooked, despite both drugs being prototypical substrates for renal elimination and potentially subject to transport by OCT2 and MATE1. METHODS: In this study, we employed population pharmacokinetic modeling to quantify the impact of renal function and metformin on amisulpride pharmacokinetics, and complemented it with molecular docking to predict binding modes on key renal transporters, thereby exploring potential transporters mediating the interacti
