[Intramedullary nail for the surgical treatment of unstable fractures of the femur in previously amputated patients].

INTRODUCTION: Femoral fractures in previously amputated patients (PAP) have been poorly reported in international literature, however it is worth mentioning that advances in the manufacture of orthotics for lower limbs have allowed these patients to remain functionally active. Surgery is then an option but difficulties may appear in positioning the patient for surgery due to the absence of the foot or ankle from where to pull and optimize the fracture reduction. OBJECTIVE: We report 3 cases of PAP with ipsilateral unstable femur fractures, treated surgically with cervicodiaphyseal endomedular

Optimal surgical preparation of the residual limb for prosthetic fitting in below-knee amputations.

A well-performed below-knee amputation can result in a high level of patient satisfaction and function. The potential for a successful result and independent ambulation is good after a transtibial amputation, with approximately 70% to 100% of patients being able to ambulate independently after this procedure. A properly fitted and appropriately designed prosthesis is integral to the achievement of these goals. The approaches and techniques discussed here are essential in optimally preparing the residual limb for prosthetic fitting. Attention to factors such as the preoperative evaluation and t

Association of Surgical Service Line and Frailty with Outcomes after Major Lower Extremity Amputation.

BackgroundMajor lower extremity amputations (LEA) are frequently performed by vascular, trauma, and orthopedic surgeons, yet comparative outcomes across services and the role of frailty remain unclear.Materials and MethodsWe conducted a single-center retrospective review of adults undergoing major LEA (above-knee, through-knee, and below-knee) from 2015-2022. Frailty was assessed using the 5-factor modified Frailty Index (mFI-5). Outcomes included complications, length of stay (LOS), readmission, re-amputation, mortality, and prosthetic ambulation. We evaluated 30-day return to the operating r

Surgical Repair of a Partial Penile Amputation From a Hair Tourniquet Under Local Anesthesia.

Penile hair tourniquet syndrome is a rare but serious condition necessitating prompt diagnosis. It is defined as a circumferential strangulation of the penis secondary to a hair coil and is predominantly encountered in the pediatric population and seldom in adults. The severity of presentation is governed by the duration of hair strangulation, with prolonged cases often necessitating surgical reconstruction. This case report describes the surgical approach in managing a near-complete penile amputation secondary to a penile hair tourniquet. Due to the patient's comorbidities, the reconstruction

The incidence of surgical site infection following major lower limb amputation: A systematic review.

Surgical site infections (SSIs) following major lower limb amputation (MLLA) in vascular patients are a major source of morbidity. The objective of this systematic review was to determine the incidence of SSI following MLLA in vascular patients. This review was prospectively registered with the International Prospective Register of Systematic Reviews (CRD42023460645). Databases were searched without date restriction using a pre-defined search strategy. The search identified 1427 articles. Four RCTs and 21 observational studies, reporting on 50 370 MLLAs, were included. Overall SSI incidence pe

Progressive ossification due to retained surgical sponge after upper leg amputation: a case report.

INTRODUCTION: Numerous cases are described of patients in whom foreign objects were found after surgery. Foreign body granuloma caused by retained surgical sponge, also called gossypiboma, mostly occur in the abdominal cavity but very seldom in limbs. CASE PRESENTATION: A 29-year-old Caucasian man presented with asymmetrical walking pattern and progressive pain in his leg, which was severely injured and amputated seven years before. A firm swelling of soft tissue with calcifications was localized in the stump. Roentgenogram and MRI showed a retained surgical sponge with calcifications. Open s

Surgical revascularization versus amputation for peripheral vascular disease in dialysis patients: a cohort study.

BACKGROUND: Surgical treatment of peripheral vascular disease (PVD) in dialysis patients is controversial. METHODS: We examined the post-operative morbidity and mortality of surgical revascularization or amputation for PVD in a retrospective analysis of United States Renal Data System. Propensity scores for undergoing amputation were derived from a multivariable logistic regression model of amputation. RESULTS: Of the Medicare patients initiated on dialysis from Jan 1, 1995 to Dec 31, 1999, patients underwent surgical revascularization (n = 1,896) or amputation (n = 2,046) in the first 6 mon

The Prevalence of Depression and PTSD in Adults With Surgically Managed Traumatic Upper-Extremity Amputations.

BACKGROUND: Upper-extremity limb loss has been associated with serious psychological sequelae. Despite advancements in surgical procedures and prostheses for upper limb amputees, it is critical to recognize the psychosocial component of these patients' care. Although the role of psychological factors in outcomes is increasingly acknowledged, little is known about the prevalence of depression and post-traumatic stress disorder (PTSD) in the civilian population after traumatic upper-extremity amputation. METHODS: In this retrospective observational single-center study, adult patients evaluated

TUNEL-positive cells in the surgical border of an amputation due to infected diabetic foot.

Diabetic infected foot is the outcome of progressive vascular and neurological damage caused by persistent chronic hyperglycemia. Due to acute hypoxia and infection, the tissues develop extensive necrosis and gangrene, which often require amputation. The decision regarding the level of amputation relies mainly on the personal experience of the surgeon who must identify the healthy tissue without necrosis. However, tissue cells under stress may succumb before clear evidence of necrosis is present. In this study, dying cells with DNA damage were identified in the necrotic lesions and surgical bo

Variables Associated with Amputation Among 600 Community-Acquired Hand Infections-A Surgical and Infectiology Cohort.

Background/Objectives: Posttraumatic infections are common in emergency hand surgery. Amputation is the worst outcome and can occur at admission or during the course of antibiotic treatment and iterative surgical debridement. We wondered if the prolongation of antibiotic treatment beyond the usual indication, and more surgical debridement besides its immediate benefit, could reveal additional preventive effect against amputation during or immediately after therapy. Methods: We investigate 166 risk association (variables) of community-acquired (traumatic) hand infection with overall treatment f

Surgical amputation for patients with diabetic foot ulcers: A Chinese expert panel consensus treatment guide.

BACKGROUND: Diabetic foot disease is a serious complication of diabetes mellitus. Patients with diabetes mellitus have a 25% lifetime risk for developing a foot ulcer, and between 14% and 24% of patients require a major or minor lower limb amputation due to severe gangrene. However, decisions concerning whether to amputate or whether to perform a major or minor lower limb amputation, and how best to determine the amputation plane remain unclear. METHODS: To consolidate the current literature with expert opinion to make recommendations that will guide surgical amputation for patients with diab

Ankle Disarticulation: An Underutilized Approach to Staged Below Knee Amputation-Case Series and Surgical Technique.

The staged approach to below knee amputation has proven to be an effective method of achieving functionality after amputation and reducing risk for postamputation complications. Transtibial or "guillotine" amputations are often used as the first-stage amputation. Disarticulation at the ankle joint is an alternative method that can be used with favorable results and benefits. These include rapid infection decompression while minimizing the blood loss of traditional tibial osteotomy. Here we describe our operative technique, perioperative management, and indications for this surgical procedure.
