T2-mapping MRI Detects the Anaerobic Threshold for Flexion and Extension of the Wrist: Potential Use in Setting Rehabilitation Strength Targets.

OBJECTIVES: Upper-limb weakness significantly impairs activities of daily living in individuals with neurological disorders. Exercise at the anaerobic threshold or higher is recommended for efficient training and rehabilitation in such cases. Given a lack of standardized, non-invasive methods to assess the anaerobic threshold for small muscles in clinical practice, we used T2-mapping magnetic resonance imaging (MRI) and cumulative frequency curves of Z values (CFZ) to detect a minimum exercise strength target for physical rehabilitation of forearm muscles above the anaerobic threshold. METHOD

Equivalent of Carbon Dioxide at the Anaerobic Threshold Predicts Acute Exacerbations in Patients with Chronic Obstructive Pulmonary Disease: A Retrospective Cohort Study.

PURPOSE: To investigate the predictive value of the equivalent of carbon dioxide at the anaerobic threshold (EqCO2@AT) for acute exacerbations (AE) in patients with chronic obstructive pulmonary disease (COPD). PATIENTS AND METHODS: This retrospective cohort study included 79 patients with COPD who underwent baseline pulmonary function testing and cardiopulmonary exercise testing (CPET). EqCO2@AT = 30 was used only as an empirical threshold for descriptive stratification, rather than as an outcome-derived cutoff. Patients were also classified into non-AE and AE groups according to annualized

Factors associated with low anaerobic threshold and its impact on sleep quality and health-related quality of life in individuals with long COVID.

INTRODUCTION: Anaerobic threshold (AT), a crucial indicator of submaximal exercise capacity and cardiorespiratory function, has been reported to be impaired in individuals with long COVID. However, the predictors of reduced AT during exercise in this population and its impacts on patient-reported outcomes, including sleep quality and health-related quality of life (HRQL), remain unknown. This study aims to identify factors associated with low AT and examine its relationship with patient-reported outcomes. OBJECTIVE: To investigate the predictors of low AT and compare patient-reported outcomes

[Effect of physical training with reference to anaerobic threshold on the indicators of exercise tolerance and restoration of work capacity in patients after myocardial infarction].

In patients who had sustained myocardial infarction including that of complicated history, the anaerobic threshold was defined to choose the optimum intensity of physical training. The latter were also examined for effects on exercise tolerance. It was ascertained that measuring physical exercise by heart rate at the level of the anaerobic threshold made it possible to choose the optimal conditions for the aerobic supply of metabolic demands during physical training. Applying physical training under aerobic conditions greatly increases exercise tolerance, prevents recurrent myocardial infarcti

Comparing four non-invasive methods to determine the ventilatory anaerobic threshold during cardiopulmonary exercise testing in children with congenital heart or lung disease.

BACKGROUND: The ventilatory anaerobic threshold (VAT) is an important method to assess the aerobic fitness in patients with cardiopulmonary disease. Several methods exist to determine the VAT; however, there is no consensus which of these methods is the most accurate. OBJECTIVE: To compare four different non-invasive methods for the determination of the VAT via respiratory gas exchange analysis during a cardiopulmonary exercise test (CPET). A secondary objective is to determine the interobserver reliability of the VAT. METHODS: CPET data of 30 children diagnosed with either cystic fibrosis (

Interval training based on ventilatory anaerobic threshold improves aerobic functional capacity and metabolic profile: a randomized controlled trial in coronary artery disease patients.

BACKGROUND: Exercise training has been an essential component of cardiac rehabilitation. However, it is not known if interval training (IT) based on the ventilatory anaerobic threshold (VAT) could be effective in improving aerobic functional capacity and metabolic profile in patients without or with coronary artery disease (CAD). AIM: To investigate the effects of an IT program, based-intensity between 70-110% of workload reached at the VAT, on the aerobic functional capacity and metabolic profile of patients with and without CAD. DESIGN: Randomized controlled trial. SETTING: Outpatients fr

Does exercise prescription based on estimated heart rate training zones exceed the ventilatory anaerobic threshold in patients with coronary heart disease undergoing usual-care cardiovascular rehabilitation? A United Kingdom perspective.

BACKGROUND: In the United Kingdom (UK), exercise intensity is prescribed from a fixed percentage range (% heart rate reserve (%HRR)) in cardiac rehabilitation programmes. We aimed to determine the accuracy of this approach by comparing it with an objective, threshold-based approach incorporating the accurate determination of ventilatory anaerobic threshold (VAT). We also aimed to investigate the role of baseline cardiorespiratory fitness status and exercise testing mode dependency (cycle vs. treadmill ergometer) on these relationships. DESIGN AND METHODS: A maximal cardiopulmonary exercise te

Poor anaerobic threshold and VO2 max recorded during cardiopulmonary exercise testing (CPET) prior to cytoreductive surgery in advanced (stage 3/4) ovarian cancer (AOC) is associated with suboptimal cytoreduction but does not preclude maximum effort cytoreduction.

This study assessed Cardiopulmonary Exercise Testing (CPET) in predicting oncological outcomes, post-operative recovery and complications in advanced ovarian cancer (AOC) cytoreductive surgery. We reviewed all patients who had CPET prior to AOC cytoreductive surgery with evidence of upper abdominal disease on preoperative imaging at the University Hospitals of Derby and Burton (UHDB) between August 2016 and July 2019. Patients were stratified by AT and maximum VO2 levels. 43 patients were identified. AT showed no relationship with major complications. 100% of patients in the AT ≥11 group recei

Detecting abnormalities in left ventricular function during exercise by respiratory measurement.

The degree of exercise-induced cardiac dysfunction and its relation to the anaerobic threshold were evaluated in 23 patients with chronic heart disease. A symptom-limited exercise test was performed with a cycle ergometer with work rate increased by 1 W every 6 seconds. Left ventricular function, as reflected by ejection fraction, was continuously monitored with a computerized cadmium telluride detector after the intravenous injection of technetium-labeled red blood cells. The anaerobic threshold (mean, 727 +/- 166 ml/min) was determined by the noninvasive measurement of respiratory gas exchan

Dyspnea Postpulmonary Embolism From Physiological Dead Space Proportion and Stroke Volume Defects During Exercise.

BACKGROUND: Many patients with pulmonary embolism (PE) report dyspnea on exertion following long-term treatment. Increased physiological dead space proportion (VD/VT) and decreased cardiac stroke volume reserve may distinguish persistent effects of PE itself from symptoms reflecting comorbid conditions or deconditioning. METHODS: This retrospective study analyzed a consecutive series of incremental symptom-limited cardiopulmonary exercise tests that had been ordered to evaluate persistent dyspnea on exertion following long-term treatment for acute PE. Physiological VD/VT was determined at ana

Evolutions in the physiology of skiing, skating and running in the Olympics.

Cross-country skiing, biathlon, Nordic combined, short track speed skating, and speed skating (12+11+3+8+14=48 events), i.e. five of the 15 disciplines in the 2018 Winter Olympics, require participants to reach the finish line in minimum time, while exerting mechanical propulsion power through flat terrain, uphill, and downhill. This article compares distances and times for these disciplines systematically with each other and with running, walking, and swimming in the Summer Olympics. Regarding physiological implications, the absence of distances below 6 km in biathlon, 5 km in Nordic combined

The respiratory system as an exercise limiting factor in normal trained subjects.

Recently, we have shown that an untrained respiratory system does limit the endurance of submaximal exercise (64% peak oxygen consumption) in normal sedentary subjects. These subjects were able to increase breathing endurance by almost 300% and cycle endurance by 50% after isolated respiratory training. The aim of the present study was to find out if normal, endurance trained subjects would also benefit from respiratory training. Breathing and cycle endurance as well as maximal oxygen consumption (VO2max) and anaerobic threshold were measured in eight subjects. Subsequently, the subjects train

Determination of ideal target exercise heart rate for cardiac patients suitable for rehabilitation.

BACKGROUND: Exercise prescription for patients with cardiovascular disease remains a challenge. The concept of exercising at an intensity equivalent to one's anaerobic threshold has been well studied and highly recommended in the fitness industry for other populations. For this concept to be applicable to patients with cardiovascular disease, the level and intensity of activity must not trigger myocardial ischemia. HYPOTHESIS: We hypothesized that the heart rate at ventilatory anaerobic threshold (HRVAT ) will not exceed heart rate at ischemic threshold (HRIT ) (ie, HRVAT ≤ HRIT in a majority
