Non-analgesic effects of opioids: the interaction of opioids with bone and joints.

Opioid consumption is associated with a wide range of side effects. Potential interference of opioids with bone and joint activity has emerged in the last few years. Opioids increase the risk of fracture, especially in elderly patients. Moreover, opioids interfere with the endocrine system, and their chronic use is a recognized risk factor for osteoporosis. Pathogenetic mechanisms at the basis of the interference of opioids with bones and joints are analysed in this review.

[Analysis the Selection of Opioid Analgesics with Mild and Moderate Cancer Pain Patients for Opioid Naïve].

In 2018, the World Health Organization revised its cancer pain therapy, abolishing the three-step pain relief ladder and recommending the use of opioid analgesics(OA)according to the pain intensity. Of opioid naive patients who were admitted to Chibaken Saiseikai Narashino Hospital from July 2015 to June 2017, treatment with weak OA was initiated in 13 patients(WOA group)and low-dose strong OA in 12 patients(SOA group). The numerical rating scale values immediately before the start of OA and 3, 7 and 14 days later were not significantly different between the 2 groups. As for adverse events, th

Characteristics of Prescription Opioid Analgesics in Pregnancy and Risk of Neonatal Opioid Withdrawal Syndrome in Newborns.

IMPORTANCE: Prescription opioids are often used during pregnancy even though they are associated with neonatal opioid withdrawal syndrome (NOWS). Most studies of adverse outcomes of opioid use for pain have assessed only the class-wide outcome despite the pharmacodynamic and pharmacokinetic heterogeneity across opioid medications. OBJECTIVE: To compare the risk of NOWS across common types of opioids when prescribed as monotherapy during the last 3 months of pregnancy. DESIGN, SETTING, AND PARTICIPANTS: This cohort study analyzed administrative claims data of Medicaid-insured mothers and newb

Utilization of opioid versus non-opioid analgesics in Medicaid and CHIP enrolled children with current asthma.

PURPOSE: Opioid analgesics are frequently dispensed in children despite its known risk in children with a compromised airway function. The objectives of the study were to assess the prevalence of opioid analgesic dispensing in children with current asthma and to identify patient and prescriber factors associated with the dispensing of opioid versus non-opioid analgesics. METHODS: Children <18 years of age, having current asthma and receiving an incident analgesic prescription were identified from a large Medicaid Managed Care Plan during years 2013 through 2018. Current asthma was defined as

IUPHAR Review - Bivalent and bifunctional opioid receptor ligands as novel analgesics.

Though efficacious in managing chronic, severe pain, opioid analgesics are accompanied by significant adverse effects including constipation, tolerance, dependence, and respiratory depression. The life-threatening risks associated with µ opioid receptor agonist-based analgesics challenges their use in clinic. A rational approach to combatting these adverse effects is to develop agents that incorporate activity at a second pharmacologic target in addition to µ opioid receptor activation. The promise of such bivalent or bifunctional ligands is the development of an analgesic with an improved sid

Do Resting Plasma β-Endorphin Levels Predict Responses to Opioid Analgesics?

OBJECTIVES: Clinically feasible predictors of opioid analgesic responses for use in precision pain medicine protocols are needed. This study evaluated whether resting plasma β-endorphin (BE) levels predicted responses to an opioid analgesic, and whether chronic pain status or sex moderated these effects. METHODS: Participants included 73 individuals with chronic low back pain (CLBP) and 88 pain-free controls, all using no daily opioid analgesics. Participants attended 2 identical laboratory sessions during which they received either intravenous morphine (0.08 mg/kg) or saline placebo, with bl

The present and future of opioid analgesics in small animal practice.

Opioids are the cornerstone for the treatment of acute pain in small animal patients. This is primarily because of their remarkable safety profile, high efficacy, and benefit of reversibility. There have been some significant advances in our knowledge on opioid pharmacology and clinical usage in companion animal medicine. This review discusses the progression of opioid use in small animal practice providing current misconceptions and controversies in light of routes of administration. Potential targets for research and drug development and novel therapies are discussed in addition to the conce

Compulsive-like responding for opioid analgesics in rats with extended access.

The abuse of prescription opioids that are used for the treatment of chronic pain is a major public health concern, costing ∼$53.4 billion annually in lost wages, health-care costs, and criminal costs. Although opioids remain a first-line therapy for the treatment of severe chronic pain, practitioners remain cautious because of the potential for abuse and addiction. Opioids such as heroin are considered very rewarding and reinforcing, but direct and systematic comparisons of compulsive intake between commonly prescribed opioids and heroin in animal models have not yet been performed. Study of non-opioid analgesics tolerance in young and adult rats.

It was demonstrated that systemic injections of metamizol and lysine-acetylsalicylate (LASA) induce inhibition of tail-flick reflex and hot-plate responses and their repeated administration leads to the development of tolerance. However, it has not been established whether these effects can be elicited by other non-steroidal anti-inflammatory drugs (NSAIDs). In this study the authors used other commonly applied analgesics--analgine, ketorolac, xefocam, which are the representatives of the three diverse groups of NSAIDs. In particular, analgine is a derivative of pirozolon, while ketorolac belo

Mood-elevating effects of opioid analgesics in patients with bipolar disorder.

Opioids can have mood-elevating effects in healthy subjects and have been used successfully to treat refractory depressed patients. A few case reports indicate that opioid analgesics can induce mania. The authors investigated the mood reaction of opioid analgesics in patients with bipolar disorder. Nine (27%) of 33 patients who took opioid analgesics for medical reasons experienced a significant hypomanic/manic reaction, and two other patients reported an antidepressant effect. None of the comparison subjects reported a significant mood reaction from opioid analgesics. These results indicate t

Comparison of four opioid analgesics as supplements to nitrous oxide anesthesia.

The hemodynamic changes, plasma norepinephrine levels, pupil size, and rapidity of postoperative mental recovery were measured and compared in 72 patients randomly assigned in a double-blind manner to receive either morphine, meperidine, fentanyl, or sufentanil as supplements to nitrous oxide anesthesia. Meperidine, unlike the other opioids, which did not change heart rate, consistently increased it. Hemodynamic responses to laryngoscopy and intubation were suppressed by fentanyl and sufentanil, but not by morphine or meperidine. The sufentanil group was the only group in which median norepine

Factors influencing the prescribed dose of opioid analgesics in cancer patients.

The dose of opioids prescribed for cancer pain does not always correlate with the actual pain severity. However, there is little evidence to explain this observation. In the present study, we sought to determine factors that influence the dose of opioid analgesics. A total of 227 patients who were administered opioids between August 2012 and May 2016 and later expired within the Department of Palliative Care at Ashiya Municipal Hospital were included, and the following variables were examined: age, sex, type of cancer, Verbal Rating Scale before and after the administration of the maximum pre-

Opioid analgesics for chronic noncancer pain in patients prescribed opioid agonist therapy or with opioid use disorder: A systematic review.

BACKGROUND: Opioid use disorder (OUD) is a growing public health concern in North America, often coexisting with chronic noncancer pain (CNCP). Managing both conditions presents unique challenges, highlighting the need for evidence to guide decision making. AIM: The study aimed to conduct a systematic review that summarizes evidence on the efficacy, effectiveness, and safety of opioid analgesics alone or in combination with opioid agonist therapy (OAT) to manage CNCP in people with OUD or with a history of OUD. METHODS: We searched MEDLINE, Embase, PsycINFO, CINAHL and AMED from inception to
