Ureteral embolisation for the management of persistent uretero-ileal anastomotic leak.

Ureteral embolisation has been described in the use of complete occlusion of the ureter to manage urinary leaks, haematuria, and fistulas. This is usually a management option of last resort in patients who have many co-morbidities, poor surgical candidates, or previous pelvic surgery. We report the use of vascular coils and glue to manage a uretero-ileal anastomotic leak following radical cystoprostatectomy.

Risk Factors and Preventive Measures for Anastomotic Leak in Colorectal Cancer.

Anastomotic leak (AL) represents one of the most detrimental complications after colorectal surgery. The patient-related factors and surgery-related factors leading to AL have been identified in previous studies. Through early identification and timely adjustment of risk factors, preventive measures can be taken to reduce potential AL. However, there are still many problems associated with AL. The debate about preventive measures such as preoperative mechanical bowel preparation (MBP), intraoperative drainage, and surgical scope also continues. Recently, the gut microbiota has received more at

Predictors of anastomotic leak after total gastrectomy in patients with adenocarcinoma.

BACKGROUND: Esophagojejunal anastomotic leakage (EJAL) is among the most feared complications after gastric cancer surgery; they entail an uncertain prognosis and relate with increased morbidity and mortality. Factors associated with their development are not well determined, and their diagnosis and treatment vary between institutions. MATERIAL AND METHODS: Retrospective case-control study of patients operated of total gastrectomy with Roux-en-Y esophagojejunostomy from January 2002 to December 2018. We divided our sample into two groups based on the presence of EJAL, and compared demographic

How to Prevent Anastomotic Leak in Colorectal Surgery? A Systematic Review.

Anastomosis leakage (AL) after colorectal surgery is an embarrassing problem. It is associated with poor consequence. This review aims to summarize published evidence on prevention of AL after colorectal surgery and provide recommendations according to the Oxford Centre for Evidence-Based Medicine. We conducted bibliographic research on January 15, 2020, of PubMed, Cochrane Library, Embase, Scopus, and Google Scholar. We retained meta-analysis, reviews, and randomized clinical trials. We concluded that mechanical bowel preparation did not reduce AL. It seems that oral antibiotic or oral antibi

Symptomatic malignant gastroesophageal anastomotic leak: management with covered metallic esophageal stents.

OBJECTIVE: Gastroesophageal anastomotic leak after cancer resection has a mortality rate of up to 60% and significant morbidity, whatever the mode of treatment. We assessed the efficacy of esophageal stenting as a therapeutic option to reduce the mortality and morbidity associated with symptomatic intrathoracic anastomotic leakage. SUBJECTS AND METHODS: During a 52-month period, 14 patients had placement of stents for clinically significant postoperative leaks: 10 patients had an esophagogastrectomy and four patients had a total gastrectomy with esophagojejunal anastomosis. Thirteen of 14 pat

Exploring the boundaries of anastomotic leak: experience in a high-volume center.

BACKGROUND: Gastric cancer remains a major global health challenge, ranking fourth in cancer-related deaths. Total gastrectomy with lymphadenectomy is the standard treatment, with advancements in surgery shifting towards minimally invasive techniques to reduce surgical trauma and metabolic response. Esophagojejunal anastomotic leak is a frequent complication of gastrectomy, significantly increasing morbidity and mortality rates by up to 64%. MATERIALS AND METHODS: A retrospective cohort study reviewed adults undergoing total gastrectomy for gastric cancer who developed esophagojejunal anastom

A development study of drain fluid gastrografin as a biomarker of anastomotic leak.

PURPOSE: Anastomotic leakage (AL) is the anathema of colorectal surgery. Its occurrence leads to increased morbidity and mortality and a prolonged hospital stay. Much work has gone into studying various biomarkers in drain fluid to facilitate early detection of AL. This stage 2a development study aims to assess the safety and feasibility of reliably detecting the iodine in Gastrografin (GG; Bayer Australia Ltd.) in drain fluid and stool samples by dual-energy computed tomography (DECT). METHODS: This is a prospective, observational, controlled, consecutive cohort study establishing the safety

Involvement of the Commensal Organism Bacillus subtilis in the Pathogenesis of Anastomotic Leak.

Background: It is now well established that microbes play a key and causative role in the pathogenesis of anastomotic leak. Yet, in patients, determining whether a cultured pathogen retrieved from an anastomotic leak site is a cause or a consequence of the complication remains a challenge. The aim of this study was to test a methodology to invoke causality between a retrieved microbe from a leak site and its role in anastomotic leak. Methods: The commensal organism Bacillus subtilis was isolated from an esophagojejunostomy leak site in a 35-year-old patient with a CDH1 mutation after a prophyl

Early, minimally invasive closure of anastomotic leaks: a new concept.

Chronic pelvic sepsis after ileoanal or coloanal anastomosis precludes ileostomy closure and, even if closure is ultimately possible, function of the neorectum is badly affected. Early closure of the anastomotic leak might prevent chronic pelvic sepsis and its adverse sequelae. In our experience of early closure in a consecutive group of six patients with a leaking low anastomosis (five with ileoanal pouch anastomosis and one after a low anterior resection), we were able to achieve anastomotic closure in five by means of initial endosponge therapy followed either by early suture (four patients

Use of pedicled omentum in esophagogastric anastomosis for prevention of anastomotic leak.

BACKGROUND: Esophagogastrectomy for carcinoma of the esophagus is the standard surgical treatment for cure or palliation. Esophagogastric anastomotic leakage is a life-threatening postoperative complication, more so if the leakage occurs in the chest. METHODS: A prospective, randomized study was conducted on 238 patients treated for carcinoma of the esophagus between January 2000 and January 2006. The study excluded 44 patients (18.49%) who were inoperable. The patients were assigned to two treatment groups of 97 each (A and B) according to a restricted permuted block randomization plan. Grou

Anastomotic leaks in colorectal cancer surgery: a risk factor for recurrence?

This study examines anastomotic leaks as a potential influence on the long term outcome of patients with colorectal cancer. 167 patients were studied who had clinical and radiological assessment of anastomotic integrity in the post-operative period, following potentially curative resections for left-sided colonic or rectal cancer. There was no evidence of a leak in 135 of these patients, while the remaining 32 developed a clinical and/or a radiological leak. At the end of a mean follow-up of 25 months, 15 patients with leaks (46.9%) developed tumour recurrence, compared with 25 of those withou

Antibiotic resistance is associated with anastomotic leak after surgery for diverticular disease.

INTRODUCTION: Patients with diverticulitis frequently receive multiple courses of antibiotics before surgery, which may promote antibiotic resistance. We hypothesized that antibiotic resistance is associated with anastomotic leak after surgery for diverticular disease. METHODS: We retrospectively reviewed patients undergoing surgery for diverticular disease and linked microbiology data to our institution's National Surgical Quality Improvement Program database. RESULTS: From 2016 to 2024, 159 patients underwent surgery for diverticular disease. The rates of anastomotic leak, infectious compl

Delayed Diagnosis of Anastomotic Leak and Failure to Rescue After Colon Resection.

IMPORTANCE: Anastomotic leak remains a leading cause of morbidity and mortality following colon resection. There is increasing evidence to suggest that failure to rescue (FTR), defined as death after a complication, is the culmination of a series of cascading events, which may be exacerbated by delays in diagnosis. Timely identification and management of anastomotic leaks may represent a crucial strategy for reducing FTR after colon resection. OBJECTIVE: To determine whether delayed diagnosis of anastomotic leak is associated with FTR following colon resection. DESIGN, SETTING, AND PARTICIPA
