Infected abdominal aortic aneurysm caused by Brucella abortus: a case report.

This case report describes an infected aneurysm caused by Brucella abortus. To the best of our knowledge, this case represents the first reported occurrence of an infected abdominal aortic aneurysm caused by this pathogen. The clinical findings included mild fever, fatigue, cold sweats, and abdominal pain. The diagnosis was reached by computed tomography scanning. The patient was treated by urgent surgery, which consisted of resection of the aneurysmal sac, prosthetic graft replacement, and omental wrapping. Antibiotics were administrated for 3 months. A bacteriologic specimen was isolated fro

Infective and traumatic aneurysms.

Traumatic intracranial aneurysms are rare lesions but should be considered in penetrating head injury, particularly in cases of low-velocity gunshot wounds or stab wounds to the head. Because most of these aneurysms are false aneurysms, treatment may require excision or trapping procedures.

In Situ Graft Replacement for a Ruptured Abdominal Aortic Aneurysm Infected with Listeria monocytogenes after Endovascular Aneurysm Repair.

Listeria monocytogenes infection and rupture of the aneurysm sac, after endovascular aneurysm repair (EVAR), are both rare. We report the case of an 82-year-old man who presented with a ruptured aneurysm by infection with L. monocytogenes after EVAR. We successfully treated him by in situ reconstruction with a bifurcated expanded polytetrafluoroethylene (ePTFE) graft, with partial removal of the infected stent graft. At 30 months from the reoperation, the patient was in good health at home, with no symptoms of infection, and the gallium-67-citrate single-photon emission computed tomography/com

Impending rupture of mycotic aortic aneurysm infected with Streptococcus equi subspecies zooepidemicus.

Streptococcus equi subspecies zooepidemicus is a beta-haemolytic, group C streptococcal bacterium. Although it is an opportunistic pathogen commonly found in horses, transmission to human can lead to severe infections. Here, we present a patient with S. equi subspecies zooepidemicus bacteraemia and consequent development of mycotic aneurysms.

[Aortoesophageal Fistula with Infected Thoracic Aortic Aneurysm Rupture;Report of a Case].

A 67-year-old man was referred to our hospital because of fever and discomfort of the throat. Gastrointestinal endoscopy revealed hematoma at the middle thoracic esophagus. Computed tomography revealed posterior mediastinal hematoma extending the descending aorta. Bacillus was detected in the blood culture. Aortoesophageal fistula with an infected thoracic aortic aneurysm rupture was diagnosed. First, thoracic endovascular aortic repair (TEVAR) was performed. Resection of the thoracic esophagus and omentopexy was conducted 15 days after TEVAR. Esophageal reconstruction using a gastric tube was

Occurrence of intracerebral aneurysm in an HIV-infected child: a case report.

A 12-year-old female diagnosed with human immunodeficiency virus infection at age 1 was admitted to our hospital with complaints of headache, left hemiparesis, and blurry vision. On admission, her CD4+ count was 97 cells/mm(3) (13%) and her viral load 44,783 ribonucleic acid copies/mL. Treatment had been initiated 3 months before admission with lopinavir/ritonavir, lamivudine, and stavudine. A computed tomographic scan and magnetic resonance imaging of the head revealed an aneurysm of the right middle cerebral artery. A head angiographic scan was consistent with intracranial arteritis. Evolution of the infected abdominal aortic aneurysm: CT observation of early aortitis.

Infected aortic aneurysm is an uncommon, life-threatening disease. Early surgical treatment is crucial to survival. An early diagnosis could be made on CT in suspected cases, although CT features of infectious aortitis overlap with retroperitoneal fibrosis, hemorrhage, and lymphadenopathy. We report the case of an infected abdominal aortic aneurysm and describe the additional potentially useful CT finding of early infectious aortitis, which helps localize the abnormality to the aortic wall.

[An infected partially thrombosed giant aneurysm of the azygos anterior cerebral artery].

The authors report a case of partially thrombosed giant aneurysm which was secondarily infected with purulent meningitis. The relationship between the infection of the aneurysm, the rapid growth of the aneurysm and the development of severe cerebral edema was discussed. A 53 year-old man was admitted on September 1, 1986, with a diagnosis of bacterial meningitis. On his admission, his body temperature was 39 degrees C, and he showed mental confusion but no neurological deficits. Laboratory data revealed signs of infection in white blood cell count, CRP, and erythrocyte sedimentation rate. Comp

Successful Surgical Treatment of an Infected Thoracoabdominal Aneurysm Accompanied with Leriche Syndrome.

A 56-year-old man presenting with massive melena and loss of consciousness was diagnosed with an infected thoracoabdominal aneurysm, an aortoduodenal fistula, and Leriche syndrome following an evaluation by computed tomography. Emergency surgery for uncontrolled infection included the reconstruction of the superior mesenteric and bilateral renal arteries using a four-branched graft. The aortoduodenal fistula was resected after omental filling, and an enterostomy was performed for feeding. Intestinal reconstruction was performed in two stages. The patient was discharged on postoperative day 48

Contained rupture of a mycotic infrarenal aortic aneurysm infected with Campylobacter fetus.

Mycotic abdominal aortic aneurysms (MAAAs) are rare entities accounting for 0.65-2% of aortic aneurysms. Campylobacter fetus has a tropism for vascular tissue and is a rare cause of mycotic aneurysm. We present a 73-year-old male patient with contained rupture of a MAAA caused by C. fetus, successfully treated with endovascular aortic repair (EVAR) and antibiotics, which is not previously described for this aetiology. Although open surgery is the gold standard, EVAR is nowadays feasible and potentially represents a durable option, especially in frail patients.

Treatment of infected abdominal aneurysms by extraanatomic bypass, aneurysm excision, and drainage.

Five patients with infected abdominal aortoiliac aneurysms were treated. The diagnosis was made preoperatively based upon fever, leukocytosis, positive blood culture findings, and presence of an aneurysm in all five patients. Two patients had salmonella species, two had staphylococcus species, and one had bacteroides species cultured from the blood and aneurysm contents. All patients were treated with appropriate antibiotics and a single operative procedure consisting of preliminary extraanatomic bypass followed by complete aneurysm excision and posterior drainage of the retroperitoneum. Infected pseudo-aneurysm of the femoral artery caused by a monitoring catheter.

A patient suffering from severe burns developed a mycotic aneurysm in the femoral artery as a result of insertion of a monitoring catheter. Treatment with antibiotics and repeated arterial surgery failed and a below knee amputation was finally performed. The use of monitoring intraarterial catheters in burn patients is to be avoided, because of impaired immunity, wound infection and septicaemia in this type of patient which could facilitate the development of septic aneurysm. If, however, a monitoring catheter is mandatory, it should be inserted and cared for by highly experienced staff who ap

A Surgical Case of Infected Popliteal Artery Aneurysm From Rare Bacteria, Eubacterium sp.

A 79-year-old man presented to our hospital with complaints of a sudden worsening of swelling in the right popliteal fossa and fever persisting for a week. Upon close examination, an infected popliteal artery aneurysm (PAA) was identified. Given the risk of rupture, the patient was advised to undergo surgery. The surgical procedure involved resecting the infectious PAA using a lateral approach. Additionally, a bypass was performed from the superficial femoral artery to the below-knee artery, utilizing the great saphenous vein located at the posterior aspect of the knee. Surgical findings revea

Endovascular salvage for contained rupture of infected thoracic aortic aneurysm with esophageal fistula.

Aortoesophageal fistula is a rare but lethal condition. The initial presentations of the disease are mostly nonspecific, such as midthoracic pain, dysphagia, or episodes of gastrointestinal bleeding. Therefore, diagnosis is often delayed. Furthermore, despite timely diagnosis and management, the outcome is generally reported to be disappointing. Surgical intervention is the only chance for patient survival, but the optimal operative approach remains controversial. We present a case of esophageal perforation resulting from the contained rupture of an infected thoracic aortic aneurysm that was s

Aorto-iliac aneurysm infected by Brucella: distinctive presentation patterns of a rare entity.

There is currently a lack of information on presentation patterns and the appropriate investigation and treatment of aortic brucellosis. Herein a case affecting the iliac component of an aorto-iliac aneurysm, managed successfully with in situ graft repair, is reported. A review of the literature identified 25 cases, with the infrarenal abdominal aorta (65%) followed by the ascending thoracic aorta (23%) being mostly affected; only our case involved the iliacs. Aortic brucellosis affected mostly older men, caused pain more often than fever (in 73% and 57%, respectively), and involved frequently

Osteomyelitis of multiple lumbar vertebrae associated with infected aortic aneurysm: a case report.

A 73-year-old male patient presented with a pulsating abdominal mass and intractable low back pain for several days. Magnetic resonance imaging revealed an infected abdominal aortic aneurysm invading the second, third, and fourth lumbar vertebrae. He underwent radical debridement of the infected aneurysm with reconstruction using vascular bypass, partial corpectomy of the L2 to L4 vertebrae, anterior reconstruction with autogenous fibular shaft, and posterior instrumentation with posterolateral fusion. Culture of the necrotic tissues grew oxacillin-resistant Staphylococcus aureus. He received

Surgical repair of infected peripheral graft and abdominal aortic aneurysm using arterial homograft.

We report a case of combined surgical repair including lower limb revascularization (below-knee bypass) and abdominal aortic aneurysm repair using cryopreserved arterial homograft. The patient experienced lower limb ischemia due to repeated thrombosis of a long-infected polytetrafluoroethylene (PTFE) graft, and was also shown to have a complicating abdominal aortic aneurysm. Infection was eradicated with total graft excision and intravenous antibiotics. Two-year patency of the in situ arterial homograft revascularization was demonstrated with hemodynamic and tomographic controls; no degenerati
