Serial optical coherence tomography and angioscopic assessments of 10-year in-stent restenosis of Cypher sirolimus-eluting stent treated with drug-coated balloon angioplasty.

The drug-coated balloon (DCB) angioplasty is considered a standard therapeutic option for in-stent restenosis. In the present case, we observed high-intensity spots on optical coherence tomography (OCT) and bright spots on coronary angioplasty (CAS) immediately after DCB angioplasty. The superficial high-intensity area on OCT presumably corresponded with the bright spots on CAS. The high-intensity superficial regions were thought to represent an iopromide/paclitaxel mixture. The present serial observation demonstrated that the eluted drugs remained for at least 2 months but disappeared within

Late-acquired stent malapposition after sirolimus-eluting stent implantation following acute coronary syndrome: angiographic, IVUS, OCT and coronary angioscopic observation.

Drug-eluting stents (DES) have been shown to significantly reduce the incidence of restenosis and target lesion revascularization in a wide variety of clinical situations. DES have also been shown to significantly reduce neointimal hyperplasia as compared to bare-metal stents. However, the antiproliferative properties of DES also delay vascular healing and have been associated with stent malapposition, hypersensitivity reactions and late stent thrombosis. Stent thrombosis could result in myocardial infarction or death. We describe here a case report of late stent malapposition following siroli

The development of a new, ultra-fine, and flexible neuroendoscope for intracranial observation.

BACKGROUND: A neuroendoscope is a technical advance that allows surgeons to visualize certain regions of the brain that was previously inaccessible through the use of a surgical microscope. Several neuroendoscope designs have been implemented by other neurosurgeons over the past 5 years. The advantage of a neuroendoscope is the addition of a flexible and narrow tip that allows for safe entry into intracranial structures for clinical observation. However, there are some limitations to this approach. Here, we report the use of a modified angioscope as a newly developed neuroendoscope to be emplo

Association of Subclinical Intrastent Thrombus Detected 9 Months After Implantation of 2nd-Generation Drug-Eluting Stent With Future Major Adverse Cardiac Events - A Coronary Angioscopic Study.

BACKGROUND: Detection of yellow plaques (YP) by coronary angioscopy (CAS) 1 year after 1st-generation drug-eluting stent (DES) implantation has been related to future coronary events. However, the association between CAS findings and clinical outcomes following 2nd-generation DES implantation has not been investigated. METHODS AND RESULTS: This study included a total of 248 2nd-generation DES in 179 patients, who were examined by CAS 9±2 months after implantation. Angioscopic evaluation included dominant neointimal coverage (NIC) grade, heterogeneity of NIC, presences of YP and intrastent thr

Spinal endoscopy combined with selective CT myelography for dural closure of the spinal dural defect with superficial siderosis: technical note.

The authors describe a new procedure to detect the tiny dural hole in patients with superficial siderosis (SS) and CSF leakage using a coronary angioscope system for spinal endoscopy and selective CT myelography using a spinal drainage tube. Under fluoroscopy, surgeons inserted the coronary angioscope into the spinal subarachnoid space, similar to the procedure of spinal drainage, and slowly advanced it to the cervical spine. The angioscope clearly showed the small dural hole and injured arachnoid membrane. One week later, the spinal drainage tube was inserted, and the tip of the drainage tube

Comparison of Chronic Angioscopic Findings of Bare Metal Stents, 1st-Generation Drug-Eluting Stents and 2nd-Generation Drug-Eluting Stents - Multicenter Study of Intra-Coronary Angioscopy After Stent (MICASA).

BACKGROUND: No previous study has reported a comprehensive comparison of the chronic angioscopic findings after bare metal stent (BMS), and 1st- and 2nd-generation drug-eluting stents (DES). METHODS AND RESULTS: The Multicenter Study on Intra-Coronary Angioscopy after Stent (MICASA) is a multicenter registry of coronary angioscopy. A total of 264 stents were observed by coronary angioscopy 1 year after PCI. There were 15 BMS, 90 1st-generation DES, and 159 2nd-generation DES. Neointimal coverage (NC) of the stent was classified into 4 grades from 0 (no coverage) to 3 (complete coverage). Yell

Clinical application of angioscopy during carotid endarterectomy for patients with internal carotid artery stenosis.

The accuracy of angioscopy in detecting atherosclerotic changes, such as plaque, ulcer, and mural thrombus, in the extracranial cerebral arteries was evaluated during carotid endarterectomy by comparison with angiographic and operative findings. Ten patients with internal carotid artery stenosis underwent intraoperative angioscopy during surgery. After clamping the carotid bifurcation, intra-arterial atherosclerotic lesions were observed with an angioscope (0.8 or 1.4 mm outer diameter) inserted through a small incision in the common carotid artery. Angioscopic findings correlated well with bo

Detection of vulnerable atherosclerotic plaque during thoracic endovascular aortic repair using nonobstructive angioscopy.

An angioscope was used to observe the intima of the aorta in an 82-year-old patient who had undergone thoracic endovascular aortic repair. The aortic angioscopic findings showed vulnerable plaques from the ascending aorta to the aortic arch that had not been visualized using preoperative computed tomography. After deploying a stent graft from zone 1 to zone 4, the proximal edge of the stent graft was adjacent to the ruptured plaque with mixed thrombi. In spite of these findings, the patient had an uneventful recovery. Angioscopy may have the potential to stratify the risk of thoracic endovascu

Flexible endoscopy in primary dye testing of the lacrimal system.

The Olympus PF-22 angioscope was used to examine the inferior meatus during primary dye testing in 80 lacrimal systems in 48 patients. Dye testing was done bilaterally in 32 subjects with no known abnormalities of lacrimal excretory function; it also was performed on the apparently normal contralateral side of 16 patients with unilateral epiphora. Dye was observed in the nose in 79 of 80 lacrimal systems (99%). Dye was not observed in one patient, who appeared to have a dysfunctional lacrimal excretory system. Primary dye testing of the lacrimal system is reliable when the Olympus PF-22 angios

Neoatherosclerosis and mural thrombus detection after sirolimus-eluting stent implantation.

BACKGROUND: Although both optical coherence tomography (OCT) and angioscopy are robust tools for detecting intrastent thrombi and neoatherosclerosis in vivo, whether OCT findings are comparable with angioscopy findings remains unclear. METHODS AND RESULTS: 22 patients presenting with de novo lesions underwent 26 sirolimus-eluting stent (SES) implantations, with follow-up OCT and angioscopy at 10 months post-implantation for segmental assessment of the proximal, mid-, and distal SES segments (66 segments). Coronary angioscopy during cardiac catheterization.

Coronary angioscopy should permit direct inspection of the luminal cross section and identification of disease. The feasibility of introducing a 5F Olympus Ultrathin fiberscope into the obstructed right coronary artery in five patients after routine cardiac catheterization by the brachial approach was therefore tested. An 8.3F USCI woven Dacron angioplasty guiding catheter was modified to enlarge its lumen. After engaging the right coronary ostium with the catheter, an attempt was made to pass the angioscope coaxially to the tip of the catheter. Visualization of the coronary lumen was then ach

Angioscopy of the inferior vena cava: preliminary observations in cases with involvement by neoplasm. Work in progress.

Angioscopy of the inferior vena cava (IVC) was performed in 10 patients. Tumor thrombus extending into the IVC was suspected in six patients, and direct invasion of the IVC wall was suspected in four, based on computed tomographic and venographic findings. In each case, a fiberscope was directed to the area of interest in the IVC via a transfemoral 5-F catheter. Blood displacement by means of a saline infusion enabled angioscopic observation of the IVC wall. The neoplasms were seen clearly in all patients but one. The color and texture of the tumor thrombi surface made it easy to distinguish f

[Intravascular ultrasound: a new dimension of invasive vascular diagnosis].

Intravascular ultrasound is a new imaging modality which allows the visualization of the vessel wall in vivo and could thus become an important adjunct to conventional angiography. Using miniature high-frequency transducers mounted at the tip of flexible catheters, high-resolution cross-sectional images are created and displayed in real-time. Several investigations have shown good correlations between ultrasound and histologic or angiographic techniques for the measurement of cross-sectional dimensions. Atherosclerotic lesions can be characterized by their differences in echo-density as calcif

Preliminary report of a new approach to sparing the greater saphenous vein for grafting: valvuloplasty combined with axial transposition of a competent tributary vein.

PURPOSE: To compare a new vessel-sparing technique combining valvuloplasty with axial transposition of a competent tributary vein versus single valvuloplasty for the treatment of greater saphenous vein (GSV) incompetence. METHODS: In 55 patients with GSV incompetence, 29 of 57 limbs were treated by angioscopic valvuloplasty of the subterminal valve alone, whereas the remaining 28 limbs underwent angioscopic valvuloplasty combined with axial transposition of a competent tributary vein identified preoperatively by duplex scanning. After angioscopic valvuloplasty in the latter group, the compete

Morphologic comparison of atherosclerotic lesions in native coronary arteries and saphenous vein graphs with intracoronary angioscopy in patients with unstable angina.

BACKGROUND: Coronary vein grafts develop accelerated atherosclerosis after aortocoronary bypass surgery. Previous pathologic studies have suggested that the morphologic appearance of atherosclerotic lesions in saphenous vein grafts may have subtle differences compared with those of native coronary arteries and may be more prone to disruption and thrombus formation. However, a comparative in vivo assessment of the angioscopic morphology differences between these two types of vessels has not been reported previously. We compared the angioscopic lesion morphology of native coronary arteries and s

Value of angioscopy for intraoperative assessment of carotid endarterectomy.

From March 1992 to November 1993 we used angioscopy and arteriography for intraoperative assessment of 103 carotid endarterectomies in 96 patients. The indication for surgery was asymptomatic stenosis in 55 cases and neurologic and/or ocular symptoms in 48. Intraoperative angioscopy and arteriography were performed to allow comparison of findings. Intraoperative angioscopic images were normal in 67 cases and abnormal in 36. The defect was an intimal flap in 26 cases, detachment of the distal plaque in seven cases, and an intimal wedge in five cases. In two cases both detachment and a wedge wer

Percutaneous transluminal angioscopy during coronary intervention.

To investigate the feasibility of angioscopic-guided percutaneous transluminal coronary angioplasty and to elucidate the mechanism of efficacy of coronary stenting for acute myocardial infarction, we performed coronary angioscopy in 102 patients with stable angina or acute myocardial infarction. Thrombi and intimal flaps were observed in most patients after coronary angioplasty. Large intimal splits were seen in one third of patients. Stents were inserted in 10 patients who were revealed to have a large flap or protruding split to the inner lumen. Thrombolytic agents were administered in 2 pat
