Atypical Miller Fisher Syndrome with Anisocoria and Rapidly Fluctuating Pupillary Diameter.

Miller Fisher syndrome is a variant of Guillain-Barre syndrome characterized by the classic triad of ophthalmoplegia, ataxia, and areflexia. Pupillary involvement is common in MFS and has been reported in 35-42% of MFS patients. Although case reports have discussed isolated ophthalmoplegia as a presentation of MFS, anisocoria and rapid fluctuation of pupillary diameter have not been reported in anti-GQ1b antibody positive individuals. Here we describe an individual who presented with diplopia and was found to have progressive internal and external ophthalmoplegia with frequent fluctuations in

MRI detection of cerebral lesions in post-traumatic anisocoria: specificity and prognostic significance.

AIM: To identify whether increased numbers of brainstem lesions are found in the presence of a post-traumatic pupillary function disturbance and classify them anatomically. MATERIALS AND METHODS: In this study, a diagnostic magnetic resonance imaging (MRI) examination was performed within 8 days after traumatic brain injury (TBI) in patients who had been unconscious for more than 24 hours post-TBI. The Glasgow Outcome Scale was evaluated 6 months after TBI. The data obtained from 140 consecutively enrolled patients between 2005 and 2011 were analysed. The clinical study parameter comprised th

Anisocoria in Focus: A Case of Patient Distress When the Mirror Signals Trouble.

Anisocoria, characterized by unequal pupil diameters typically ranging from 0.3 mm to over 2.0 mm, represents a clinically significant finding that requires thorough evaluation in the emergency department. This asymmetry may indicate dysfunction of the cranial nerves or iris musculature, thereby providing valuable insight into the integrity of the autonomic nervous system. Although anisocoria may be physiologic or benign, it can also serve as a warning sign of severe neurologic or ophthalmologic pathology, including intracranial hemorrhage, oculomotor nerve palsy, or Horner syndrome. In the ab

Mydriasis and anisocoria in a pediatric hyperhidrosis patient with interesting findings in the family cat.

Here, we report a case of unilateral ocular mydriasis in a pediatric patient with longstanding hyperhidrosis, as well as similar findings in her cat. The patient had been undergoing treatment of her hyperhidrosis with topical glycopyrrolate. This case highlights the potential side effect profile of topical antimuscarinics and the importance of counseling patients on proper precautions.

Anisocoria and Poor Pupil Reactivity by Quantitative Pupillometry in Patients With Intracranial Pathology.

OBJECTIVES: To describe the prevalence and associated risk factors of new onset anisocoria (new pupil size difference of at least 1 mm) and its subtypes: new onset anisocoria accompanied by abnormal and normal pupil reactivities in patients with acute neurologic injuries. DESIGN: We tested the association of patients who experienced new onset anisocoria subtypes with degree of midline shift using linear regression. We further explored differences between quantitative pupil characteristics associated with first-time new onset anisocoria and nonnew onset anisocoria at preceding observations usi

Buzzing Sympathetic Nerves: A New Test to Enhance Anisocoria in Horner's Syndrome.

Introduction: Patients with suspected Horner's syndrome having equivocal pupil dilation lag and pharmacologic testing may undergo unnecessary MR imaging and work up in the case of false positive pupil test results. Our goal was to increase the diagnostic accuracy of pupillometry by accentuating the inter-ocular asymmetry of sympathetic innervation to the iris dilator with surface electrical stimulation of the median nerve using a standard electromyography machine. We hypothesized that an accentuated difference in sympathetic response between the two eyes would facilitate the diagnosis of Horne

Transdermal scopolamine and perioperative anisocoria in craniofacial surgery: a report of 3 patients.

Postoperative nausea and vomiting (PONV) is a common complaint after plastic and reconstructive surgery. Transdermal scopolamine is a commonly used agent for prevention of PONV. Anisocoria from transdermal scopolamine use is an adverse effect that has not been reported in the plastic surgery literature. We present a series of 3 craniofacial patients in which ipsilateral mydriasis occurred and spontaneously resolved after removal of the scopolamine patch. Given the various causes and potentially grave implications of unilateral mydriasis, we discourage the use of transdermal scopolamine in cran

A Case of Pharmacologic Anisocoria in Systemic Glycopyrrolate Use from Presumed Local Ocular Inoculation.

Anisocoria is a common finding in ophthalmic clinical practice. History taking and examination is critical in appropriately diagnosing and managing anisocoria, as the differential can be extensive ranging from benign to life-threatening entities. This case discusses the presentation of a 22-year-old female with a history of myopia and hyperhidrosis who presented with pharmacologic anisocoria which was presumed to be from inadvertent topical exposure to conventional glycopyrrolate tablets. To our knowledge, pharmacologic mydriasis from exposure to residue from conventional glycopyrrolate tablet

Anisocoria as initial manifestation of multiple sclerosis. Use of 3 tesla magnetic resonance imaging.

A 21-year-old woman seen in this clinic with non-reactive mydriasis in the right eye that contracted with 1% pilocarpine. Cranial angio-CT and 1.5 T magnetic resonance imaging (MRI) did not detect any disease. Given a subsequent limitation of adduction, supraduction, and infarction of the right eye, a 3 T MRI was requested. This showed a lesion of the midbrain at the exit of the 3rd cranial nerve. After improvement, no new episodes were observed until 18 months later, when the patient presented with probable optic neuritis and systemic symptoms. At this time the 1.5 T MRI detected infratentori

[Painful anisocoria in a five-year-old child: A rare diagnosis which must not be missed].

UNLABELLED: Congenital ectropion uveae (CEU) is a rare anomaly of the embryonic development of the anterior segment of the eye. We report the case of a 5-year-old child with an undiagnosed CEU who was treated urgently for an acute angle closure attack. CASE DESCRIPTION: A 5-year-old child was referred urgently for evaluation of anisocoria with mydriasis of the right eye and severe headache. Brain imaging with contrast injection was initially performed in the pediatric emergency department and ruled out central nervous system pathology. The initial examination of the right eye revealed an intr

Asymmetry in the autonomic nervous system with reference to the nasal cycle, migraine, anisocoria and Menière's syndrome.

Studies on the nasal cycle have demonstrated that the autonomic tone to the nose is asymmetrical and oscillates in a regular cycle. Autonomic tone may be regulated from a centre in the hypothalamus and normally there is a balance between he autonomic tone of the right and left halves of the body. However, under stress or with hypothalamic instability this balance may be disrupted and result in the marked autonomic asymmetry seen in migraine or Meniér's syndrome. Research on the nasal cycle in conditions where autonomic asymmetry is apparent could change the entire concept of autonomic control.

[Horner type anisocoria associated with brain infarction of the internal carotid artery axis].

Brain infarction caused by arterial occlusion of the internal carotid axis sometimes develops Horner syndrome. The purpose of this study is to clarify the characteristics and mechanism of "Horner type" anisocoria, which is one of the symptoms of Horner syndrome, in patients with brain infarction in regions supplied by the internal carotid artery (ICA). We studied 112 consecutive patients (71 males and 41 females, mean age of 60.8 +/- 12.3 years) with brain infarction with either ICA or the middle cerebral artery (MCA) occlusion, who were admitted to the National Cardiovascular Center within se

[Anisocoria after unilateral photorefractive keratectomy. Result of a lesion of the pupillary sphincter muscle?].

BACKGROUND: After unilateral photorefractive keratectomy (PRK) for myopia correction, relative mydriasis of the treated eye was noticed. The aim of this study was to determine the incidence and possible etiology of this anisocoria. PATIENTS AND METHODS: In a prospective clinical study we examined eight consecutive patients after PRK of a spherical equivalent of -1.0 to -8.5 D with a 6.5-7.0 mm optical zone (Schwind-Keratom) for up to 10 months. Measurements of the pharmacological uninfluenced pupillary diameter were carried out with a Goldmann perimeter under 31.5 asb and under four different

Anisocoria After Direct Light Stimulus is Associated with Poor Outcomes Following Acute Brain Injury.

BACKGROUND: Assessing pupil size and reactivity is the standard of care in neurocritically ill patients. Anisocoria observed in critically ill patients often prompts further investigation and treatment. This study explores anisocoria at rest and after light stimulus determined using quantitative pupillometry as a predictor of discharge modified Rankin Scale (mRS) scores. METHODS: This analysis includes data from an international registry and includes patients with paired (left and right eye) quantitative pupillometry readings linked to discharge mRS scores. Anisocoria was defined as the absol
