Fatal aspiration (Mendelson's) syndrome despite antacids and cricoid pressure.

Two patients died after inhalation of gastric contents during general anaesthesia for caesarean section. Both had received antacid therapy before surgery and their stomachs contained large volumes of fluid. Failure to empty the stomach preoperatively may have been a major contributory cause of these deaths.

The in-vitro adsorption of some antirheumatics on antacids.

The adsorption of sodium salicylate, salicylamide, acetylsalicylic acid, paracetamol, mefenamic acid, flufenamic acid, phenylbutazone, oxyphenbutazone, phenazone, aminophenazone, indometacin and methiazinic acid on some antacids was studied. The antacids used were magnesium trisilicate, magnesium oxide, aluminium hydroxide, bismuth oxycarbonate, calcium carbonate and kaolin. Magnesium oxide, followed by aluminium hydroxide and bismuth oxycarbonate showed a fairly high adsorptive capacity for salicylates, mefenamic acid, flufenamic acid, methiazinic acid, indometacin and to a lesser extent for

Is antacid treatment necessary in obstetric anesthesia?

All the obstetric units in Jeddah were surveyed regarding the use of antacid prophylaxis and the methods of anesthesia used for emergency and elective cesarian section. The results were compared with the Western practice where marked variation was found but this apparently did not influence mortality from acid aspiration.

Antacid use in a family-practice population.

According to market surveys, antacid use is common in the general population. This study of antacid use in a university-based family-practice clinic supports this conclusion. In a sample of 731 adult patients, 51.3% reported taking antacids at least once in the previous six months and 10.9% took antacids daily. Highest antacid users were people 31-40 years of age. Heartburn was the most common symptom in patients taking antacids, but antacid use increased as the patients reported more symptoms. Antacid use by patients is often unknown to the physician. This has some clinical importance because

The effect of food and antacid on the absorption of fendosal.

Fendosal (200 mg) was given orally to each of two separate groups of twelve healthy male volunteers on separate occasions to assess the influence of food or antacid on the bioavailability of fendosal. Blood samples (20 ml) were drawn during 12 hours post-dosing and fendosal plasma concentrations were quantitated by a validated fluorescence technique. Food was shown to have no significant effect (p greater than 0.05) on fendosal bioavailability. However, the bioavailability of fendosal in the presence of an antacid was reduced by 80 per cent. In vitro studies suggested that a complexation betwe

The effect of flavour on acceptability of antacid tablets.

One hundred healthy volunteers recorded their subjective impressions of taste, mouth feel, after-taste and preference, after receiving in turn two tablets of each of four commonly-used antacids. The subjects remained unaware of the appearance of name of the products. The orange flavoured tablets, Asilone Orange, were shown to have a more acceptable taste, mouth feel and after-taste than three other commonly used antacids, Gaviscon, Altacite Plus, and Aluminium Hydroxide B.P., and these differences were highly significant (P less than 0.0001). Furthermore, 93% of cases expressed a willingness t

[Antacids and gastric mucosa. Protective effect of an antacid on acetylsalicylic acid induced functional lesions in the human gastric mucosa].

Salicylates do induce functional and morphological changes of the gastric mucosa: increased H+-back-diffusion, increased Na+-influx, and increased blood loss. Several functional parameters of the gastric mucosal barrier correlate with the transmural electrical potential difference (PD) which can be used as a sensitive, but unspecific parameter to detect functional changes of the gastric mucosal barrier. The effect of a salicylate alone and in presence of a liquid antiacid (Gastropulgit Gel) on transmural PD was measured in healthy volunteers. The presence of an antacid prevented the salicylate

[Efficacy of antacids in the treatment of chronic gastritis].

This article presents main principles of chronic gastritis treatment. Therapeutic abilities and possible side-effects due to components of antacids are analyzed. Special attention is paid to antisecretory and cytoprotective activity of Pepsan-R, which contains haiasulen (the main active component of chamomilla) and dimeticon. The authors of the article emphasize opportunity of using Pepsan-R in case of heartburn, gastric pain, abdominal distention during pregnancy and lactation.

In vitro availability of gliclazide in presence of antacids.

Gliclazide is the most commonly used sulfonylurea derivative for NIDDM therapy. Due to its prolonged therapy, there is always a likelihood of its use with other drugs. On the other hand antacids are commonly prescribed to encounter gastric acidity etc. Present paper deals with the in vitro availability studies of gliclazide in presence of antacids. These studies were carried out in simulated gastric juice and in buffer of pH 7.4 at body and accelerated temperature. The antacids used in these studies were aluminum hydroxide, aluminum trisilicate, magnesium oxide, magnesium trisilicate, sodium b

Antacids and ulcer healing. A review of the evidence.

There have been a number of controlled trials of antacids in the treatment of patients with peptic ulcer disease. As a general rule the size of studies has been small and there have been difficulties ensuring adequate binding, because of the formulation and taste of the antacids. Despite these difficulties, antacids appear to be effective ulcer healing agents with efficacies resembling those of other antiulcer drugs. Definite dose relationships are unclear but high doses of buffering capacity over 200 mmol/day appear unnecessary and are associated with increasingly frequent adverse effects. Lo

Antacids and anticholinergics in the treatment of duodenal ulcer.

Anticholinergic agents are most often divided into tertiary and quaternary ammonium compounds. Tertiary ammonium agents are fully and rapidly absorbed. Quaternary ammonium agents are slowly and incompletely absorbed, but do not cross the blood-brain barrier, or do so to a very limited degree, and have a very low rate of central nervous system side-effects. There are probably subclasses of cholinergic receptors, and anticholinergic agents have different affinities for these. Anticholinergics can decrease the basal acid secretion as well as secretion stimulated by histamine, pentagastrin, insuli

[Metabolic effects of antacids and interactions with other drugs].

The incidence of side effects from antacids is low, but patients with renal insufficiency are at risk to develop alkalosis with high doses of calcium carbonate or magnesium hydroxides, or to develop hypercalcemia due to insufficient calcium elimination by the kidneys.--A great potential exists for drug interactions with antacids. In most instances, the effectiveness of other drugs is decreased in the presence of antacids, but effectiveness may be increased for L-dopa (less degradation in stomach) or quinidine (renal elimination reduced). Interactions at the absorption level can be avoided by a

[Calcium-containing antacids in the therapy of duodenal ulcers].

The neutralization of acid by antacids and the inhibition of acid secretion by H2-receptor antagonists are the two essential factors in the standard therapy for peptic ulcer disease. In order to determine whether a high dosage therapy with a calcium-containing antacid accelerates the healing of the ulcer or retards it through the "Acid-rebound" effect of the calcium, we examined in a clinical study 10 patients with an endoscopically proven duodenal ulcer. After 2 weeks the duodenal ulcer was healed in 5 of the 10 patients. Thus a rate of healing was attained which was achieved in our clinic wi

The effects of antacids on enteric-coated salicylate preparations.

A volunteer study was undertaken in which the effect of the co-administration of therapeutic doses of aluminium hydroxide and magnesium trisilicate on the excretion of aspirin derived from enteric-coated preparations was studied. A significant alteration in the pattern of salicylate excretion was seen, but the mechanism of the interaction cannot be deduced from the present study. It was concluded that the interaction was of potential therapeutic importance, and further studies based on this pilot investigation have been initiated.

Acid pulmonary aspiration syndrome after antacids. A case report.

This is a report of a patient who developed acid pulmonary aspiration syndrome following pulmonary aspiration of gastric contents at a pH of 3.5. The volume of the stomach contents was large and 15 ml of magnesium trisilicate was insufficient to prevent the effects of acid aspiration. After operation, considerable pulmonary shunting was demonstrable for several days. The patient was discharged home well, and a chest x-ray 2 months later showed no abnormality.

Antacid therapy for emergency caesarean section.

One hundred patients undergoing emergency Caesarean section were treated with cimetidine 200 mg intramuscularly when the decision was made to deliver the patient by Caesarean section and, preceding general anaesthesia, 30 ml of 0.3 M sodium citrate was administered orally. No routine antacid therapy was employed during labour. No patient had a gastric aspirate pH of less than 2.7, and only one of less than 3. The regimen is simple and effective, and treats only those patients at risk of acid aspiration.

Enhancement of drug absorption by antacids. An unrecognised drug interaction.

Antacids are widely used for many disorders. The potential of antacids to interact with other concomitantly ingested drugs is well recognised. These interactions usually result in reduced or delayed absorption of the affected drug. However, this is not always the case. In contrast to aluminium hydroxide, magnesium hydroxide and sodium bicarbonate can enhance the absorption of some drugs. For example, magnesium hydroxide can increase the rate and sometimes even the extent of absorption of certain nonsteroidal anti-inflammatory drugs (e.g. tolfenamic acid, mefenamic acid and ibuprofen), sulphony
