National Adherence to Guidelines for Antimicrobial Prophylaxis for Patients Undergoing Radical Cystectomy.

PURPOSE: The sentinel reference for antibiotic prophylaxis for radical cystectomy with ileal conduit in the AUA Guidelines reports data from 2003-2013 and has not been updated in the interim. Here, we assess adherence to antibiotic prophylaxis guidelines among patients undergoing radical cystectomy with ileal conduit for bladder cancer using a large national database. As a secondary objective, we assess the association between antimicrobial use and postoperative infection during the index admission following cystectomy. MATERIALS AND METHODS: The Premier Healthcare Database was queried for al

Ciprofloxacin and furagin in acute cystitis: comparison of early immune and microbiological results.

Furagin (a nitrofurantoin analogue) has the same efficacy in treating acute cystitis as ciprofloxacin, however the duration of therapy is longer. We established a hypothesis that therapy with ciprofloxacin results in faster resolution of mucosal inflammation in comparison with furagin. Rates of urinary secretion of immunoglobulins class A, M and G and interleukin-8 (IL-8) were evaluated before and after initiation of therapy in adult women presenting with acute cystitis confirmed by urine culture. Women were randomised into two groups receiving either ciprofloxacin 250mg twice a day for 3 days

["Classic" vs autolubricant catheterization for endovesicular chemotherapy. Preliminary experience].

AIM: The aim of this study was to evaluate the frequency of urinary tract infections (UTI) after catheterisation for instillation comparing two systems: the "classic" method and the catheterisation using a new autolubricant device: EasiCath Coloplast. METHODS: During the period of endovesical chemotherapy (between 4 and 48 weeks), 22 patients (6 females and 18 males) were studied, aged between 53 and 78 years old. We have performed 139 instillations using Nelaton Ch 14 or 12 type catheters lubricated with gel based on lidocaine, neomicyn and fluocinolone ("classic" method). Instead 135 patien

Urinary tract infection. How has its management changed?

In patients with apparent urinary tract infection, clinical presentation and results of physical examination and a few simple laboratory studies can identify patients who need urine culture and/or imaging studies and can guide initial therapy. A wide variety of effective antibiotics are available for various causes, and local patterns of microbial sensitivity and cost-effectiveness help govern drug selection. Response to therapy governs further treatment and follow-up. Exclusion of complicated infection and particular risk factors permits simplified but effective management of symptomatic recu

Efficacy of anti-microbial catheters in preventing catheter associated urinary tract infections in hospitalized patients: A review on recent updates.

Catheter-associated urinary tract infections (CAUTIs) are the common hospital-associated infections (HAIs), which can be prevented by practicing necessary precautions and by using antimicrobial urinary catheters (UCs). The efficacy of antimicrobial UCs against standard catheters for averting CAUTIs is poorly studied. The objective of the review is to analyze the efficacy of various types of antimicrobial UCs used in hospitalized patients in preventing CAUTIs. The major antimicrobial UCs are silver and antibiotic catheters, in contrast, few antimicrobial catheters include antimicrobial peptides

Natural Polyphenols for Prevention and Treatment of Urinary Tract Infections.

Urinary tract infections (UTIs) are the second most common type of bacterial infection worldwide. UTIs are gender-specific diseases, with a higher incidence in women. This type of infection could occur in the upper part of the urogenital tract, leading to pyelonephritis and kidney infections, or in the lower part of the urinary tract, leading to less serious pathologies, mainly cystitis and urethritis. The most common etiological agent is uropathogenic E. coli (UPEC), followed by Pseudomonas aeruginosa and Proteus mirabilis. Conventional therapeutic treatment involves the use of antimicrobial

Quinolones: their uses in geriatric infections.

The quinolones represent a unique class of drugs which offer a new option for clinicians to treat a wide variety of infections in the elderly patient. Whereas the emergence of resistance is a major problem with the older quinolones such as nalidixic acid, this appears to be a minor problem with the new fluoroquinolones such as norfloxacin or ciprofloxacin. These drugs are bactericidal and are well-absorbed orally. Parenteral preparations should be available in the future. The drugs are particularly active against gram-negative bacilli, including Pseudomonas aeruginosa. The fluoroquinolones are

A preventable, life-altering case of fluoroquinolone-associated tendonitis.

Fluoroquinolones, such as ciprofloxacin and levofloxacin, are broad-spectrum antibacterial agents that have historically been widely used for urinary tract infections, pneumonia, and intra-abdominal infections but are associated with several serious adverse reactions, including tendinopathy and tendon rupture, peripheral neuropathy, and aortic aneurysm. These drugs should not be used for uncomplicated infections unless no other antimicrobial treatment is feasible. This article describes a patient who experienced life-altering disability from a fluoroquinolone, reviews the adverse reactions of

[Antibacterial and anti-algal activity if two traditional drugs in the treatment of urinary tract infection and cystitis in Mali].

For the treatment of the urinary infections and the cystitis, the medicinal plants constitute a source of new molecules with an antimicrobial activity and economically accessible to face the apparition of a phenomena of germ resistance to antibiotics. The urinary infections and the cystitis are very spilled and constitute a problem of public health in developing countries. About 50% of women develop a symptomatic urinary tract infection at least once in their life. The urinary infection is, by order of frequency, the first of the non epidemic infectious illnesses. The objective of our survey i

Prevention of urinary tract infection in patients with spinal cord injury--a microbiological review.

The importance of urinary tract infection (UTI) in patients with spinal cord injury cannot be understated. Many patients with significant bacteriuria are considered to be colonised rather than infected, and treatment should be reserved for those with clinical symptoms or other signs of infection. Published research on the prevention and management of UTI in patients with spinal cord injury often has limitations due to differences in definitions of UTI, studies on groups using different urinary drainage appliances, the mixture of newly injured and longstanding injured patients and studies being

Catheter-associated infections in urology: possible use of silver-impregnated catheters and the Erlanger silver catheter.

Indwelling urinary catheters play a very important part in urology. However, their use is accompanied by a considerable increase in the risk of nosocomial urinary tract infections. The pathophysiological cause is ascribed to pathogens that adhere to the catheter surface, proliferate and produce a biofilm. In addition to aseptic techniques, modification of the catheter material to confer antimicrobial activity plays an essential part in the prevention of catheter-related urinary tract infections. The antimicrobial efficacy of silver against gram-positive and gram-negative bacteria is well known

Pharmacodynamics of D-mannose in the prevention of recurrent urinary infections.

Recurring urinary tract infections (rUTIs) are frequently caused by Escherichia coli, which invades urothelial cells and forms quiescent bacterial reservoirs. D-mannose, an inert monosaccharide, represents a notable agent for rUTI prevention; however, there is no agreement on its dosage. To provide pharmacological basis for an effective dose, we evaluated its ability to inhibit adhesion of E. coli to urothelial cells. E. coli strains isolated from the urine of a woman with recurrent urinary tract infections were selected according to adhesion capacity. Anti-adhesive efficacy and invasion were

Urinary tract infections in pregnancy.

QUESTION: My pregnant patients often present with urinary tract infections. Are the medications commonly used for the management of urinary tract infections safe to use during pregnancy? ANSWER: Existing data indicate that exposure to penicillins, cephalosporins, fluoroquinolones, nitrofurantoin, or phenazopyridine during pregnancy is not associated with increased risk of fetal malformations. Trimethoprim-sulfamethoxazole should be avoided, if possible, during the first trimester of pregnancy because of the antifolate effect associated with neural tube defects.

Antibiotic treatment for urinary tract infections: pharmacokinetic/pharmacodynamic principles.

Bacterial urinary tract infections (UTIs) frequently occur in the outpatient as well as in the nosocomial setting. The stratification into uncomplicated and complicated UTIs has proven to be clinically useful. Bacterial virulence factors on the one hand, and the integrity of the host defense mechanisms on the other, determine the course of the infection. Late complications or sequelae might ensue if the defense mechanisms are disturbed, either by a genetic or an acquired cause. Escherichia coli is the most prevalent organism in uncomplicated UTIs, whereas the bacterial spectrum is much broader
