Animal Venoms as Potential Source of Anticonvulsants.

Epilepsy affects millions of people worldwide, and there is an urgent need to develop safe and effective therapeutic agents. Animal venoms contain diverse bioactive compounds like proteins, peptides, and small molecules, which may possess medicinal properties against epilepsy. In recent years, research has shown that venoms from various organisms such as spiders, ants, bees, wasps, and conus snails have anticonvulsant and antiepileptic effects by targeting specific receptors and ion channels. This review underscores the significance of purified proteins and toxins from these sources as potenti

Anticonvulsants to treat post-traumatic stress disorder.

OBJECTIVE: We reviewed the existing literature on the efficacy of anticonvulsants in treating post-traumatic stress disorder. METHODS: We performed a literature search using PubMed, EMBASE and the Cochrane database on 30 September 2013. Randomized, controlled studies that investigated the efficacy of anticonvulsants for post-traumatic stress disorder were included in this review. Studies with retrospective designs, case reports and case series were excluded. RESULTS: A total of seven studies met the inclusion criteria for this review. Three studies used topiramate with negative findings regar

Prophylactic anticonvulsants in patients with brain tumour.

OBJECTIVE: We conducted a clinical trial to determine if prophylactic anticonvulsants in brain tumour patients (without prior seizures) reduced seizure frequency. We stopped accrual at 100 patients on the basis of the interim analysis. METHODS: One hundred newly diagnosed brain tumour patients received anticonvulsants (AC Group) or not (No AC Group) in this prospective randomized unblinded study. Sixty patients had metastatic, and 40 had primary brain tumours. Forty-six (46%) patients were randomized to the AC Group and 54 (54%) to the No AC Group. Median follow-up was 5.44 months (range 0.13

[Acute pharmacologic poisoning (II). Analgesics and anticonvulsants].

In this second chapter on Acute Drugs Poisoning we deal with two groups of substances of great transcendence from the point of view of their use and morbidity/mortality. Within the group of analgesic-anti-inflammatory drugs we consider paracetamol and the salicylates, which are easily available to the population. With respect to the anticonvulsants, although they are barely involved in the ensemble of acute drug poisonings, their effects can be serious. We concentrate on four drugs: valproic acid, phenobarbitol, carbamacepine, and phenytoin. Finally, a section is dedicated to isoniazid, a drug

Comparative study of different anticonvulsants in eclampsia.

One hundred eclampsia patients were admitted in this hospital from January 1993 to November 1993 and they were subjected to different anticonvulsant regimes. There were 28 patients in lytic cocktail group, 16 in diazepam group, 16 in magnesium sulphate group and 40 patients in phenytoin sodium group. The results are compared in every aspect. Feto maternal outcome is best with magnesium sulphate regime followed by phenytoin sodium regime. They produce significantly better results than lytic cocktail and diazepam.

Spina bifida: anticonvulsants and other maternal influences.

At least two maternal conditions have been shown to be associated with an increased risk for the occurrence of anencephaly, spina bifida and encephalocele: insulin-dependent diabetes mellitus in the mother and maternal epilepsy for which anticonvulsant treatment is being used. Of 147 infants born at the Brigham and Women's Hospital with anencephaly, spina bifida and encephalocele among 123,489 live-born, still-born and elective terminations surveyed, 7.5% were infants of insulin-dependent diabetics and 0.7% had been exposed to anticonvulsants in utero. In addition to these proven causal associ

Studies on opioid peptides as endogenous anticonvulsants.

In both normal and hypophysectomized rats, electroconvulsive shock (ECS) produced a significant postictal rise in seizure threshold (S.T.) to flurothyl, a volatile convulsant. This ECS-induced increase in S.T. was markedly attenuated by naloxone (10 mg/kg s.c.), which itself did not alter basal S.T. A dose of bicuculline (0.1 mg/kg i.p.) which was slightly proconvulsant in the flurothyl test did not significantly alter the postictal rise in S.T. produced by ECS. With hypophysectomized rats, there was a 24% increase in basal S.T. to flurothyl challenge. Hypophysectomy had no influence on the po

Latent osteomalacia in epileptic patients on anticonvulsants.

The bone mineral content was measured in 10 epileptic patients on long-term treatment with phenytoin before and during treatment with vitamin D. None of the patients showed biochemical signs of osteomalacia. Initially subnormal values for bone mineral content were found, which increased significantly during treatment. The results suggest the occurrence of latent osteomalacia in a fairly high proportion of epileptic patients on anticonvulsants.

[Anticonvulsant hypersensitivity syndrome and lamotrigine-associated anticonvulsant hypersensitivity syndrome].

Anticonvulsant hypersensitivity syndrome (AHS) is defined by the association of high fever, cutaneous rash and multiorgan-system abnormalities (incidence, one in 1000 to one in 10,000 exposures). Fatal complications are described in 10%. This reaction usually develops 1 to 12 weeks after initiation of an aromatic anticonvulsant. Drug rash with eosinophilia and systemic symptoms (DRESS) can be discussed as differential diagnosis. Several hypotheses have been put forward to explain the pathogenesis of AHS. These include accumulation of toxic metabolites, antibody production and viral infection.

Denzimol, a new anticonvulsant drug. I. General anticonvulsant profile.

The anticonvulsant profile of N-[beta-[4-(beta-phenylethyl)phenyl]-beta-hydroxyethyl]imidazole hydrochloride (denzimol, Rec 15-1533) has been evaluated in mice, rats and rabbits in comparison with some standard antiepileptic drugs. Denzimol suppressed electrically and chemically induced tonic seizures but did not prevent the clonic ones. In mice and rabbits the anticonvulsant activity of denzimol against maximal electroshock seizures was almost equal to that of phenytoin and phenobarbital with more rapid onset of action, whereas in rats the compound resulted in being the most potent and the le

The anticonvulsive action of adenosine: a postsynaptic, dendritic action by a possible endogenous anticonvulsant.

Neural afterdischarges generated in the presence of penicillin or low extracellular calcium concentrations were found to be inhibited by adenosine in the rat hippocampus in vitro. This anticonvulsant effect of adenosine is observed in the absence, as well as in the presence, of chemical synaptic transmission and apparently occurs at a postsynaptic site which is most sensitive in the apical dendritic region of the CA1 pyramidal cells. The methylxanthine theophylline antagonizes the effect of adenosine; and, the anticonvulsant action of the L-isomer of the adenosine analogue phenylisopropyladeno

Anticonvulsants and suicide attempts in bipolar I disorders.

OBJECTIVE: To identify risk factors for suicide attempts (SA) in individuals commencing treatment for a manic or mixed episode. METHOD: A total of 3390 manic or mixed cases with bipolar disorder (BD) type I recruited from 14 European countries were included in a prospective, 2-year observational study. Poisson regression models were used to identify individual and treatment factors associated with new SA events. Two multivariate models were built, stratified for the presence or absence of prior SA. RESULTS: A total of 302 SA were recorded prospectively; the peak incidence was 0-12 weeks afte

[Effect of anticonvulsants on the nitric oxide system].

The effect of phenobarbital, carbamazepine, valproate sodium, depakine, topiramate and lamotrigine on the content of NO and NO-synthase activity in white rat brain tissues has been studied. It was established that the action of carbamazepine, valproate sodium, topiramate and lamotrigine decreases the activity of NO-synthase and the level of NO in the brain tissues. The amount of NO does not change while NO-synthase activity increases with the introduction of phenobarbital. The involvement of nitric oxide in the mechanism of action of the studied anticonvulsant drugs is discussed.

[Anticonvulsants treatment of psychiatric disorder in elderly patients].

Agents introduced for the treatment of epilepsy have also been usedsimultaneously for psychiatric indications. The therapeutic effects of anticonvulsants are recognized in many psychiatric disorders. Growing literature suggests that anticonvulsant medication have efficacy in treating acute mania, bipolar disorder, psychotic disorders, behavioral disturbances especially agitation and impulsive aggression and anxiety, alcohol withdrawal and dependence. However, study of anticonvulsants among elderly patients has been limited. Further research in the field of psychogeriatry is needed to clarify t

The diverse roles of anticonvulsants in bipolar disorders.

Anticonvulsant drugs (ACs) have diverse antiseizure, psychotropic, and biochemical effects. Carbamazepine and valproate have mood-stabilizing actions, benzodiazepines and gabapentin have anxiolytic actions, lamotrigine is useful in rapid cycling and acute treatment and prophylaxis of bipolar depression, and topiramate and zonisamide can yield weight loss. Limited controlled data suggest the carbamazepine keto derivative oxcarbazepine has antimanic effects. A categorical approach to the diverse roles of ACs in bipolar disorders is proposed, using broad categories of ACs, on the basis of their p

Major system toxicities and side effects of anticonvulsants.

The use of anticonvulsants is expanding in the treatment of bipolar and related disorders. Although they have characteristics in common, the anticonvulsants currently used are quite diverse and vary in their spectrum of activity, quality of supporting evidence, and organ toxicities. Common side effects of anticonvulsants that can limit tolerability but are not physiologically severe include sedation and other cognitive impairments, tremor, and gastrointestinal side effects. Possibly less common, but of more physiologic significance, are effects on body weight and metabolism and dose-related he
