Vaptans: A Narrative Review of Pharmacokinetics, Pharmacogenetics, and Clinical Applications.

Vaptans are non-peptide arginine vasopressin (AVP) receptor antagonists essential for modulating water homeostasis. This review evaluates their chemical evolution from peptide analogs to orally bioavailable molecules and summarizes the pharmacokinetic profiles of key agents, including tolvaptan, conivaptan, and mozavaptan. Significant emphasis is placed on the clinical role of tolvaptan as a disease-modifying therapy in autosomal dominant polycystic kidney disease (ADPKD) and its efficacy in syndrome of inappropriate antidiuretic hormone secretion (SIADH). Furthermore, the impact of pharmacoge

Diagnosis and management of hyponatremia in cancer patients.

BACKGROUND: Hyponatremia is among the metabolic disturbances encountered in oncology. Risk factors for hyponatremia include chemotherapy, treatment-induced nausea and vomiting, hydration, pain, narcotic drugs, and physical and emotional stress. A common cause of hyponatremia in patients with cancer is the syndrome of inappropriate secretion of antidiuretic hormone (SIADH), which may result from ectopic production of arginine vasopressin (AVP) by the tumor tissue. TREATMENT: The AVP-receptor antagonists, a new class of agents, correct hyponatremia by directly blocking the binding of AVP with i

[Hyponatremia with cisplatin administration in head and neck cancer patients].

Hyponatremia is one of the most common electrolyte disorders encountered in clinical practice of medical anticancer treatment. Cisplatin (CDDP) is a well-known chemotherapeutic agent that associates with hyponatremia. We retrospectively studied clinical features of hyponatremia CDDP administration. The incidence of hyponatremia at the first administration was 64. 1%. The significant risk factors of hyponatremia are body weight less than 60 kg, creatinin clearance less than 60mL/min, and sodium depletion and intake loss due to treatment-induced anorexia, nausea, vomiting and diarrhea. The mecha

Angiotensin II receptor antagonists in arterial hypertension.

Angiotensin II receptor antagonists (AT-1) represent a new group of orally active antihypertensive agents. Activation on AT-1 receptor leads to vasoconstriction, stimulation of the release of catecholamines and antidiuretic hormone with production of thirst, and promote growth of vascular and cardiac muscle; these effects are blocked by AT-1 antagonist agents. The first chemically useful, orally active AT-1 receptor antagonist was losartan, followed by other agents currently in clinical use, such as: valsartan, eprosartan, irbesartan, telmisartan, candesartan, and many others under investigati

Endocrine consequences of antifungal therapy: A missed entity.

With fungal infections rising worldwide, the use of azoles, polyenes, and echinocandins, the mainstay of antifungal therapy has expanded markedly. With broader use, uncommon adverse effects are increasingly being identified. Azoles, the most widely prescribed class of antifungal agents, inhibit several cytochrome P450-dependent steps in human steroidogenesis, leading to disruption of endocrine pathways. Adrenal and gonadal dysfunctions with ketoconazole, and mineralocorticoid excess with posaconazole and itraconazole, are well-documented. Uncommon manifestations, such as voriconazole-induced s

Dose-dependent stimulation of renal prostaglandin synthesis by deamino-8-D-arginine vasopressin in rats with hereditary diabetes insipidus.

It has been postulated that renal prostaglandins (PGs) function as negative feedback inhibitors of the action of antidiuretic hormone (ADH), implying a correlation between levels of ADH and the rate of renal PG synthesis. These studies have evaluated the relationship between renal PG synthesis and hormone levels in rats with hereditary diabetes insipidus, a species devoid of circulating ADH. Since vasoconstrictor agents can stimulate renal PG synthesis by mechanisms unrelated to antidiuretic activity, deamino-8-D-arginine vasopressin (dDAVP) was utilized for replacement therapy instead of argi

[Arginine vasopressin antagonism--new treatment option in chronic heart failure].

BACKGROUND: In spite of recent therapeutic advances, chronic heart failure is a frequent cause of hospitalisation and death. Inhibition of arginine vasopressin is a new, promising treatment option. We here present an overview of how vasopressin antagonists are used in chronic heart failure. MATERIAL AND METHODS: This article is based on a review of relevant literature found by searching Google and PubMed and studies of reference lists in identified articles. RESULTS AND INTERPRETATION: In chronic heart failure, different neurohormonal systems are activated. Inhibition of these by beta-adrene

Transient diabetes insipidus in post CABG patient after vasopressin withdrawal: A report.

Vasopressin is increasingly being used as inotropic agent due to its catecholamine's sparing property. Vasopressin is instrumental in rapid adjustment of water excretion according to the state of body hydration. It is also used in treatment of DI; Arginine vasopressin disorder, formerly known as diabetes insipidus (DI), is a disease process that results in either decreased release of antidiuretic hormone (Anti Diuretic Hormone (ADH), also known as vasopressin or Arginine Vasopressin (AVP) or reduced response to ADH, causing electrolyte imbalances. however, there are only anecdote reports discu

Nephrogenic diabetes insipidus. Effects of 3,5, cyclic-adenosine monophosphate.

In view of the evidence that the actions of vasopressin are mediated by the release of adenosine 3,5, cyclic monophosphate (`cyclic-AMP'), the effects of this nucleotide were studied in 3 children with nephrogenic diabetes insipidus (NDI) who were insensitive to vasopressin. Intravenous injections of cyclic-AMP reduced urine flow in 2 of the 3 patients, but did not increase urinary osmolality nor the urinary concentrations of sodium, creatinine, and urea. In these respects the antidiuresis differed from that normally produced by vasopressin. The reduction in urine flow produced by cyclic-AMP w

Severe symptomatic hyponatremia during citalopram therapy.

Hyponatremia secondary to the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is an uncommon complication of treatment with the new class of antidepressant agents, the selective serotonin reuptake inhibitors (SSRIs). Most of the reported cases were associated with fluoxetine; only 2 have previously been described in association with citalopram. We describe an elderly woman who presented with severe symptomatic hyponatremia caused by the SIADH during therapy with citalopram. Because the use of SSRIs is becoming more popular among elderly depressed patients, the present case

Vasopressin dysregulation and hyponatremia in hospitalized patients.

Hyponatremia, which is often due to dysregulation of arginine vasopressin, occurs frequently in hospitalized patients and is associated with increased morbidity and mortality. Nonosmotic secretion of arginine vasopressin is central to the pathophysiology of hyponatremia in patients with euvolemic hyponatremia (due to, for example, the syndrome of inappropriate secretion of antidiuretic hormone) and those with hypervolemic hyponatremia secondary to congestive heart failure or cirrhosis with ascites. Arginine vasopressin-receptor antagonists, a novel class of agents that block the action of argi

Evaluation and management of hypo-osmolality in hospitalized patients.

Hyponatremia is the most common electrolyte disorder encountered in the clinical setting. Abnormalities of the mechanisms that maintain normal water and sodium metabolism are often present in hospitalized patients, including defects in renal water excretion. All of the current therapeutic approaches in patients with the syndrome of inappropriate antidiuretic hormone secretion and other forms of vasopressin-induced hyponatremia have significant limitations. Studies in animal models and humans have demonstrated that antagonists of the AVP V2 receptor in the kidney are effective in correcting hyp

Enuresis in Children.

Enuresis, also known as nocturnal enuresis or bedwetting, refers to intermittent incontinence during sleep. It warrants medical attention, in children older than five years of age, or earlier if desired by the child or family, due to its significant emotional and psychological impact on quality of life. Children who have never achieved nighttime continence are diagnosed with primary enuresis, while those who begin bedwetting after at least six months of being dry, with secondary enuresis. The condition is further classified as monosymptomatic (MNE) or non-monosymptomatic (NMNE), depending on t

Clonidine-induced diuresis in the rat: evidence for a renal site of action.

Clonidine, an alpha adrenergic agonist which causes a diuresis in experimental animals, was studied in unanesthetized, conscious Brattleboro rats heterozygous or homozygous for hereditary hypothalamic diabetes insipidus to determine if the diuresis was due to alpha adrenergic inhibition of antidiuretic hormone (ADH) release or to another mechanism of action. Heterozygous rats given clonidine s.c. in doses of 50 to 300 mu/kg b.w. exhibited a prompt dose-related diuresis. The diuresis was transient and could not be maintained beyond 4 hr even when clonidine was administered continuously by s.c.

Role of tolvaptan in acute decompensated heart failure.

Acute decompensated heart failure accounts for more than 1 million hospitalizations in the USA every year. Currently, the most common treatment for symptom relief is the use of loop diuretics, despite recent concerns for potential adverse effects. With the growing understanding of the role of neurohormonal dysregulation in the pathophysiology of heart failure, there has been increasing interest in novel pharmacologic therapies targeting specific neurohormonal axes. Serum arginine vasopressin is a potent vasoconstrictor, as well as an antidiuretic, and serum concentrations are upregulated in he

Desmopressin: a nontransfusional hemostatic agent.

Desmopressin (1-deamino-8-D-arginine vasopressin, DDAVP) is a synthetic analogue of the antidiuretic hormone L-arginine vasopressin. Because it can raise circulating levels of Factor VIII and of von Willebrand's factor, DDAVP is used for nontransfusional treatment of mild and moderate hemophilia and von Willebrand's disease. DDAVP also shortens the prolonged skin bleeding time in patients with uremia, liver cirrhosis, and platelet dysfunctions and is given to prevent or stop excessive bleeding in such conditions. Finally, there is evidence that DDAVP can reduce blood loss and transfusion requi

Isoproterenol-stimulated renin secretion in the rat: second messenger roles of Ca and cyclic AMP.

These experiments were designed to elucidate which of two second messengers (cyclic 3',5' adenosine monophosphate [c-AMP]; intracellular calcium [Cai]) was more closely related to the renin secretory process. The rat renal cortical slice preparation was used. Agents which previously were shown to inhibit basal renin secretion by increasing Cai (ouabain, vanadate, angiotensin II, antidiuretic hormone, and 60 mM K) antagonized and/or blocked isoproterenol-stimulated secretion, which is thought to be mediated by adenylate cyclase activation and increased levels of c-AMP.
