[Subcutaneous nodules and sensitivity to aluminum in patients undergoing hyposensitivity immunotherapy].

Hypersensitivity reactions to aluminium are very rate. Sensitization has been reported to occur during continuous application of aluminium-containing antiperspirants or by aluminium adjuvants in vaccines and hyposensitization immunotherapy. We report 5 patients who developed persistent and itching subcutaneous nodules following immunotherapy. Histologically, the nodules showed a granulomatous reaction consistent with delayed hypersensitivity to aluminium. Patch tests with 2% aluminium chloride in water were positive in three patients, and positive results of patch test with other metals (nicke

The Blend of Taurine and Aloe Vera Extract Boosts Action Against Skin Irritation: In Vitro and Clinical Evaluations.

Regular usage of cosmetic products and drugs in dermatological vehicles may cause irritant contact dermatitis. For example, aluminum chloride (AlCl3), the most efficacious antiperspirant salt to treat hyperhidrosis, shows high irritancy potential. To mitigate the irritant contact dermatitis caused by topical application of products containing AlCl3, we investigated the anti-irritating effects of aloe extract and taurine in vitro and in vivo. In an in vitro experiment, reconstructed human epidermis model, EpiDerm, was tested with AlCl3 in the presence or absence of taurine and aloe extract. In

Axillary Contact Dermatitis: An Update on Potential Allergens and Management.

The differential diagnosis of dermatoses in the axillae is broad. Contact dermatitis-both irritant and allergic-represents common etiologies. Axillary contact dermatitis can develop following exposure to a variety of irritants and/or allergens. Frequently implicated sources include deodorants, antiperspirants, detergents, soaps, and clothing. Fragrance, a ubiquitous ingredient within these products, as well as metals and dyes, are common causes of contact dermatitis. Clinical assessment, bedside diagnostic techniques, histopathology, and patch testing can aid in the diagnosis and help inform m

Iontophoresis for palmar and plantar hyperhidrosis.

Iontophoresis is a safe, efficacious, and cost-effective primary treatment of palmar and plantar hyperhidrosis. Decades of clinical experience and research show significant reduction in palmoplantar excessive sweating with minimal side effects. To get the best results from iontophoresis, health care professionals need to provide education on the mechanism of action and benefits, evidence of its use, and creation of a future patient-specific plan of care for continued treatments at home or in the physician's office. Iontophoresis may be combined with other hyperhidrosis treatments, such as topi

Aluminium and the human breast.

The human population is exposed to aluminium (Al) from diet, antacids and vaccine adjuvants, but frequent application of Al-based salts to the underarm as antiperspirant adds a high additional exposure directly to the local area of the human breast. Coincidentally the upper outer quadrant of the breast is where there is also a disproportionately high incidence of breast cysts and breast cancer. Al has been measured in human breast tissues/fluids at higher levels than in blood, and experimental evidence suggests that at physiologically relevant concentrations, Al can adversely impact on human b

Topical therapies in hyperhidrosis care.

Primary focal hyperhidrosis affects 3% of the US population; about the same number as psoriasis. More than half of these patients have primary focal axillary hyperhidrosis: sweating that is beyond what is anticipated or necessary for thermoregulation. Most topical therapies are based on aluminum salts, which work by a chemical reaction that forms plugs in the eccrine sweat ducts. Topical anticholinergics may also be used. Instruction on proper methods and timing of antiperspirants enhances effect and may be effective alone or in combination with other treatments in patients with hyperhidrosis.

[Management of hyperhidrosis].

Hyperhidrosis continues to be undertreated in our view, despite its propensity to considerably impair quality of life. We shall break down therapeutic approaches to hyperhidrosis into several steps: (a) determine the physiological causes of excess sweating; (b) establish the type of hyperhidrosis involved and screen for causes of secondary hyperhidrosis before diagnosing essential hyperhidrosis; (c) evaluate the severity of the hyperhidrosis by means of a validated scale (HDSS score), Minor's starch-iodine test or gravimetric analysis; (d) select one of the medical therapies currently availabl

Hyperhidrosis and bromhidrosis -- a guide to assessment and management.

BACKGROUND: Hyperhidrosis and bromhidrosis are two common conditions, which are troublesome for patients and carry a significant psychosocial burden. OBJECTIVE: This article details an approach to the assessment and management of hyperhidrosis and bromhidrosis, and outlines current treatment options. DISCUSSION: Hyperhidrosis can be either generalised or focal. Generalised hyperhidrosis may be primary and idiopathic or secondary to systemic disease. Treatment may require oral anticholinergic agents. Focal hyperhidrosis is usually primary and responds to topical measures. Specialist referral

Bisabolol.

Bisabolol is an active plant extract isolated from German chamomile and thought to have antiinflammatory and skin-soothing properties. It is a common additive in many products, including moisturizing creams and ointments, lotions, cleansers, sunscreens, antiperspirants, and makeup products. Contact dermatitis from bisabolol has been reported in Europe and is purported to occur in the United States. Patch testing with bisabolol-containing products or bisabolol may be useful in the work-up of patients with presumptive allergic contact dermatitis or potentially worsening atopic dermatitis. Patien

Aluminum-based salts: an unorthodox but simple way to treat molluscum contagiosum and cutaneous warts.

Molluscum contagiosum and human papillomavirus (HPV) are very common cutaneous infections with worldwide distribution, yet treatment options remain limited. Traditional therapies are invasive and have a high rate of recurrence, and newer topical agents available are costly. Here I discuss evidence supporting the hypothesis that the aluminum-based salts, utilized in the antiperspirant industry since the early 1900s, could have a therapeutic effect on skin lesions due to molluscum contagiosum, HPV and possibly other viruses. Clinical trials in this area are needed.

Aluminum: The 2022 American Contact Dermatitis Society Allergen of the Year.

Aluminum recently was selected as the 2022 Allergen of the Year by the American Contact Dermatitis Society. Aluminum contact allergy, which most often is related to its use as an adjuvant in select vaccines and allergen-specific immunotherapies, tends to present with pruritic subcutaneous nodules at the injection site. Allergy to aluminum-containing antiperspirants manifests as axillary vault dermatitis. In this article, we highlight the growing recognition of aluminum contact allergy, particularly in the pediatric population, focusing on distinct presentations of aluminum allergic contact der

Concentration of aluminium in breast cyst fluids collected from women affected by gross cystic breast disease.

Gross cystic breast disease (GCBD) is the most common benign breast disorder, but the molecular basis of cyst formation remains to be identified. If the use of aluminium-based antiperspirant salts is involved in the etiology of gross breast cyst formation, it might be expected that aluminium would be at elevated levels in human breast cyst fluid (BCF). Aluminium was measured by ICP-MS in 48 samples of BCF, 30 samples of human blood serum and 45 samples of human breast milk at different stages of lactation (colostrum, intermediate, mature). The median level of aluminium in apocrine type I BCF (

Isoniazid induced palmoplantar hyperhidrosis.

Secondary focal hyperhidrosis is usually due to peripheral or central neuronal defects and is a complex dysfunction of the parasympathetic and sympathetic nervous system. Palmoplantar hyperhidrosis has been described with various types of polyneuropathies such as diabetic but has not previously been described with isoniazid. We report a 15-year-old boy, being followed for neurotuberculosis, who presented with burning sensation and hyperhidrosis of both palms and soles five months after starting antitubercular therapy. With oral pyridoxine supplementation, the paraesthesia and hyperhidrosis red

Studies on the mechanism of topical anhidrosis due to polyvalent cations.

Synopsis Although eccrine sweat glands are present in the foot pads of many mammals, and are abundant in the general body skin of many of the higher primates, it is only in man that these glands have a thermoregulatory function and are cosmetically significant. The expansion in sales of various antiperspirant products containing complexes of aluminium and zirconium has resulted in many studies, whose aim is to elucidate the mode of action of these substances. Sweat production in the rat foot pad in vivo has been studied, and its modification following topical application of various polyvalent

Granular Parakeratosis: A Case Report.

Granular parakeratosis is an uncommon acquired keratinization disorder that occurs in the armpit, groin, and other parts of the body. It may be related to stimulation by detergents and antiperspirants. This article reports a case of granular parakeratosis in the groin. The patient was a young man with no predisposing factors. The clinical manifestations included symmetrical bilateral inguinal erythema, dryness, and a small amount of bran-like desquamation. After histopathological examination, the final diagnosis was granular parakeratosis, which was cured by topical application of glucocortico

Efficacy of paroxetine in primary palmoplantar hyperhidrosis occurring with juvenile myoclonic epilepsy.

Primary palmoplantar hyperhidrosis (PPH) is believed to be a dysfunction of emotional sweating, with an estimated prevalence of around 3%. Several treatment options including topical antiperspirants, tap water iontophoresis, botulinum toxin injections, oral anticholinergics, and tricyclic antidepressants are available, each with a significant adverse event profile. For the first time, we report a PPH patient with comorbid juvenile myoclonic epilepsy (JME) treated successfully with a combination of paroxetine and divalproex sodium. Paroxetine resulted in improvement in PPH, possibly through its

Topical botulinum toxin to treat hyperhidrosis? No sweat!

Palmar, plantar and axillary hyperhidrosis, though benign, may be burdensome and occupationally restrictive, even hazardous. Treatment modalities range from topical antiperspirants, iontophoresis, systemic medications such as anticholinergics and benzodiazepines and injections of botulinum toxin, to thoracic sympathectomy. Intradermal injections of botulinum toxin (BTX), though effective, are painful especially when multiple injections are required. Iontophoretic administration of BTX has been described, the BTX entering the eccrine sweat glands via the sweat pores and through the sweat ducts.

Cutaneous Reactions to Aluminum.

: Cutaneous exposure to aluminum may occur via contact with metal items, medications, and personal care products. Despite the widespread use of aluminum, allergic contact dermatitis is relatively rare. Sensitization is often incidentally identified during patch testing with aluminum-based chambers. This article presents several cases along with a literature review summarizing prevalence and clinical manifestations of cutaneous reactions to aluminum, recommendations for patch testing, sources of aluminum, and reproducibility of aluminum allergy over time.

Primary focal hyperhidrosis: scope of the problem.

Focal hyperhidrosis (HH) can cause debilitating reductions in the physical and emotional quality of life (QOL) of patients, which can result in numerous restrictions of a patient's personal and professional lifestyle and activities. A variety of treatment options are available for primary focal HH, including topical and oral agents, tap water iontophoresis (TWI), botulinum toxin type A (BTX-A), and surgery. Studies evaluating BTX-A (Botox) treatment for palmar, plantar, and facial HH reveal that BTX-A provides effective treatment of primary focal HH, with a reasonable duration of effect, and h

Current and emerging therapeutic modalities for hyperhidrosis, part 1: conservative and noninvasive treatments.

Approximately 1% to 3% of the US population has hyperhidrosis (HH). HH can be an incapacitating medical condition because it not only hinders patient quality of life but also causes the secondary effect of excess cutaneous sweat. There is a broad spectrum of treatment modalities including topical and systemic therapies, iontophoresis, localized neuroinhibitory injections, and surgical interventions. This article reviews HH and the conservative treatments for the condition.
