A retrospective analysis of drug therapy in patients with rheumatoid arthritis.

A retrospective analysis of the pharmacotherapy used in 100 patients during their illness was performed. The method was that of a standardized interview based on a questionnaire. Analysis of the data showed that salicylates still dominate among the first-line antirheumatic drugs, but are gradually being replaced by newer antirheumatic agents. A large number of patients received drugs belonging to the group of pyrazolidine compounds. Basic therapy primarily consisted of chrysotherapy; therapy with antimalarials had to be discontinued due to side effects in a high percentage of cases. An interes

Review of medications used in juvenile rheumatoid arthritis.

Juvenile rheumatoid arthritis is a chronic condition. The goal of therapy is to control pain, preserve joint range of motion and function, minimize systemic complications, and assist in normal growth and development. Recent advances in understanding the pathophysiology of arthritis have expanded the treatment of this chronic condition. Many medications including nonsteroidal anti-inflammatory agents, disease-modifying antirheumatic drugs, biologic agents, and cytotoxic agents are available for treating juvenile rheumatoid arthritis. Emergency medicine physicians should be familiar with the dif

Hematological Manifestations among Patients with Rheumatic Diseases.

BACKGROUND: Rheumatic diseases have many hematological manifestations. Blood dyscrasias and other hematological abnormalities are sometimes the first sign of rheumatic disease. In addition, novel antirheumatic biological agents may cause cytopenias. SUMMARY: The aim of this review was to discuss cytopenias caused by systemic lupus erythematosus and antirheumatic drugs, Felty's syndrome in rheumatoid arthritis, and autoimmune hemolytic anemia, thrombosis, and thrombotic microangiopathies related to rheumatological conditions such as catastrophic antiphospholipid syndrome and scleroderma renal

Management of asymptomatic coccidioidomycosis in patients with rheumatic diseases.

Coccidioidomycosis is an endemic fungal infection common in the southwestern United States. Some rheumatology clinics periodically screen patients with coccidioidal serology, resulting in the identification of patients who are serologically positive but without clinical symptoms. The management of such patients is unclear. A retrospective study was conducted between 2007 and 2015 at two arthritis centers in Tucson, Arizona. The asymptomatic patients were identified who were receiving disease-modifying antirheumatic agents and had a positive coccidioidal serology. Serological testing including

Immune Checkpoint Inhibitor Related Rheumatological Complications: Cooperation between Rheumatologists and Oncologists.

In cancer, immune checkpoint inhibitors (ICIs) improve patient survival but may lead to severe immune-related adverse events (irAEs). Rheumatic irAEs are a distinct entity that are much more common in a real-life than in clinical trial reports due to their unspecific symptoms and them being a rare cause of hospitalization. This review focuses on an interdisciplinary approach to the management of rheumatic irAEs, including cooperation between oncologists, rheumatologists, and immunologists. We discuss the immunological background of rheumatic irAEs, as well as their unique clinical characterist

A case of rheumatoid arthritis complicated by demyelination in both cerebral cortex and spinal cord during etanercept therapy.

Tumor necrosis factor (TNF) antagonists, including etanercept, have been approved for the treatment of rheumatoid arthritis (RA). These agents are not free of adverse events like other antirheumatic agents. Several important adverse events in CNS lesions have been reported. In this paper, we report on one patient with RA that had complications from a demyelinating disorder during TNF-blockade therapy using etanercept at 24 months after initial administration. A 66-year-old Japanese woman was diagnosed with RA in 1959. She received various disease-modifying antirheumatic drugs (DMARDs), but all

ISHLT consensus document on lung transplantation in patients with connective tissue disease: Part III: Pharmacology, medical and surgical management of post-transplant extrapulmonary conditions statements.

Patients with connective tissues disease (CTD) are often on immunomodulatory agents before lung transplantation (LTx). Till now, there's no consensus on the safety of using these agents perioperative and post-transplant. The International Society for Heart and Lung Transplantation-supported consensus document on LTx in patients with CTD addresses the risk and contraindications of perioperative and post-transplant management of the biologic disease-modifying antirheumatic drugs (bDMARD), kinase inhibitor DMARD, and biologic agents used for LTx candidates with underlying CTD, and the recommendat

Use of healthcare administrative claims data in observational studies of antirheumatic drug effects on pregnancy outcomes: A scoping review.

The safety of antirheumatic drugs in pregnancy and their impact on maternal and neonatal outcomes are understudied. Despite pregnant individuals being excluded from clinical trials, their continued use of medications raises the importance of addressing knowledge gaps regarding safety and impact on outcomes. A scoping review was conducted following JBI methodology and PRISMA reporting guidelines to describe how antirheumatic drugs and associated adverse pregnancy outcomes have been investigated in observational studies using claims data. Electronic databases (MEDLINE (Ovid), Embase (Ovid), and

Atherogenic lipid profiles and its management in patients with rheumatoid arthritis.

Cardiovascular morbidity and mortality are enhanced in rheumatoid arthritis, which might be due to an increased prevalence of cardiovascular risk factors such as dyslipidemia. The dyslipidemia observed in RA appears to be dependent on disease activity, ie, a higher disease activity is associated with lower total cholesterol levels and even more depressed high density lipoprotein levels, leading to a higher (ie, unfavorable) atherogenic index. It appears that this dyslipidemia is already present long before the clinical onset of rheumatoid arthritis. Antirheumatic drug treatment with disease mo

Clinical Consensus Statement: Perioperative Management of Prothrombotics, Antiplatelets, Anticoagulants, DMARDs and Biologics in Elective Podiatric Surgery.

INTRODUCTION: A substantial body of literature exists on the perioperative management of pharmacological agents specifically prothrombotic agents, antiplatelets, anticoagulants and disease-modifying antirheumatic drugs (DMARDs) aimed at mitigating risks associated with both the medications themselves and the comorbidities they treat. However, a clear and consistent consensus on best practice is either lacking or poorly defined especially in foot surgery. To address this gap, a working group of podiatric surgeons convened to develop a Clinical Consensus Statement (CCS) for the perioperative man

Biological Therapies in Inflammatory Myopathies.

Idiopathic inflammatory myopathies (IIM) are a rare group of disorders that feature progressive immune-mediated skeletal muscle destruction along with skin, lung, and joint involvement. Management of IIMs necessitates glucocorticoid therapy followed by conventional steroid-sparing agents to control disease activity. In the settings of refractory myositis or life-threatening manifestations, e.g. lung involvement or oropharyngeal dysphagia, second-line therapies are needed to minimize disease burden, avoid end-organ damage and steroid toxicity, and decrease mortality. These therapies may include

Current immunotherapy in rheumatoid arthritis.

Rheumatoid arthritis is a common autoimmune disease primarily manifesting as chronic synovitis, subsequently leading to a change in joint integrity. Progressive disability and systemic complications are strongly associated with a decreased quality of life. To maintain function and health in patients with rheumatoid arthritis, early, aggressive and guided immunosuppressive therapy is required to induce clinical remission. Antirheumatic drugs are capable of controlling synovial inflammation and are therefore named 'disease-modifying antirheumatic drugs' (DMARDs). This article aims to bridge the

Functional Components from Nature-Derived Drugs for the Treatment of Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a chronic systemic disorder characterized by persistent synovitis and systemic inflammation. Currently, the widely used drugs for the treatment of RA are disease-modifying antirheumatic drugs, biological agents and glucocorticoids. But their clinical use has been limited because of their adverse effects with a high frequency and high cost of treatment. It is essential to find novel candidate agents. Traditional Chinese medicine (TCM) has been used for RA treatment for a long period of time. In recent years, significant amounts of studies have shown that some TCMs a

[Anti-interleukin 17 is a new possible treatment of patients with psoriatic arthritis].

Psoriatic arthritis (PsA) is a chronic inflammatory disease associated with comorbidity and decreased quality of life. The current treatment of PsA is non-steroid anti-inflammatory drugs or in more severe cases disease modifying antirheumatic drugs such as methotrexate or leflunomide, but these agents are often inadequately effective. Biologic tumour necrosis factor (TNF)-inhibitors are effective in the treatment of PsA. However, many patients lose efficacy or have an insufficient response to TNF-inhibitors. Biologic agents targeting interleukin-17A have shown encouraging clinical results.

Management of osteoporosis in rheumatoid arthritis patients.

INTRODUCTION: In rheumatoid arthritis (RA) patients, the risk of both vertebral and non-vertebral fractures is roughly doubled, which is for an important part caused by inflammation-mediated amplification of bone loss and by immobilization. New treatments have become available in the last two decades to treat both RA and osteoporosis. AREAS COVERED: Epidemiology and assessment of osteoporosis and fracture risk (including the influence of RA disease activity and bone-influencing medications such as glucocorticoids), the importance of vertebral fracture assessment in addition to bone density me

The therapeutic dilemma triangle in rheumatoid arthritis-related respiratory disease.

Rheumatoid arthritis (RA) is increasingly well controlled with conventional disease-modifying antirheumatic drugs (DMARDs) including methotrexate, as well as the widespread use of biologic agents and Janus kinase (JAK) inhibitors, which have improved joint outcomes and the overall prognosis. Nevertheless, respiratory complications remain a major determinant of clinical outcomes and often fundamentally shape therapeutic strategies in routine practice. Respiratory manifestations of RA are diverse and include interstitial lung disease (ILD), airway disease (e.g., bronchiectasis and bronchiolitis)

[Cardiovascular disease in rheumatic diseases].

The representatives of immunoinflammatory diseases are rheumatic ones, such as primarily rheumatoid arthritis, juvenile idiopathic arthritis, spondyloarthritis, psoriatic arthritis, systemic lupus erythematosus, and other systemic connective diseases, which are characterized by a high risk for untimely death. The high risk of untimely death in these diseases has been found to be associated with the severity of an immunoinflammatory process that gives rise to severe irreversible damage to vital organs and systems and with the development of a wide spectrum of comorbidities (infections, intersti
