[3.0 T MRI with a high resolution protocol for the study of benign disease of the anus and rectum. Part one: High resolution protocol for 3.0 T MRI, anatomic review, benign tumors, and congenital or acquired alterations of the sphincter complex].

Benign anorectal disease comprises a broad group of processes with very diverse origins; these processes may be congenital or acquired as well as inflammatory or tumor related. However, benign anorectal disease has received less attention in the scientific literature than malignant disease. We present an image-based review of the most common benign diseases of the anus and rectum. In this first part, we review the anatomy of the region and provide a brief description of the peculiarities of the high resolution protocol that we use with 3.0 T MRI. We go on to describe the most common benign ano

Multizonal intraepithelial neoplasia of the lower genital tract and anus in women: terminology for defining the disease, an introduction by the British Society for Colposcopy and Cervical Pathology (BSCCP), International Anal Neoplasia Society (IANS), European Federation for Colposcopy (EFC) and British Society for the Study of Vulval Disease (BSSVD) scientific committees.

Multizonal anogenital intraepithelial neoplasia (MZIN) is an uncommon chronic pre-malignant condition. In the United Kingdom (UK) and elsewhere MZIN is managed by a variety of clinical specialities with differing strategies, resulting in a lack of standardisation in diagnosis and treatment which ultimately disadvantages those affected. Screening for anogenital precancerous conditions is sporadic rather than nationalised in the UK and elsewhere in Europe, with the exception of the cervix. To address this lack of standardisation, the BSCCP brought together a panel of stakeholders from aligned ex

[Transperineal sonography in anorectal disorders].

AIM: Magnetic resonance imaging, computed tomography, endorectal and endoanal sonography are used for imaging of inflammatory and neoplastic conditions of the lower rectum, the anus and the perineum. These methods, however, have their limitations regarding accuracy, cost-effectiveness, and availability in the acute setting. Pain may be a limiting factor when introducing a probe into the anus. Percutaneous transperineal sonography is an acknowledged method for diagnosing anorectal malformations in children and infants and for diagnosis of prostatic disease. Until today, only limited reports reg

Posterior Sagittal Anorectoplasty for Acquired Imperforate Anus Complicated by Anorectal Necrosis: A Case Report.

Anorectal necrosis is an uncommon lethal disease in children, characterized by necrosis of the mucosa of the anus and rectum. The difference between anorectal necrosis and Fournier's gangrene is that anorectal necrosis does not affect the genital organs. The treatment for anorectal necrosis includes debridement of the anus, colostomy, and the use of broad-spectrum antibiotics. However, anorectal necrosis may lead to anal stricture, anal malfunction, or even acquired atresia of the anus. There is no consensus on the treatment for acquired imperforate anus. Herein, we report a case of a four-mon

Anorectal malformation with didelphys uterus: Extremely rare anomaly and successful neoanal sphincter reconstruction with gracilis muscle flap.

Anorectal malformation or imperforate anus is a congenital anomaly of rectum and anus. Mullerian duct anomalies are abnormal development of uterus, cervix, and vagina. Imperforate anus with double uterus is extremely rare and cannot explain by normal embryologic development. Moreover, guideline in treatment is inconclusive. We report an extremely rare case of a young adult female who presented with recurrent pelvic inflammatory disease caused by rectovaginal fistula in congenital imperforate anus and didelphys uterus, and successfully neoanal reconstruction with gracilis muscle flap. Ultrasonography of imperforate anus in neonate: an approach correlated with current surgical concepts.

Neonates with an imperforate anus pose a challenge to those responsible for diagnosing and treating the congenital abnormality. Early assessment and accurate diagnosis of the type of imperforate anus are essential for determining the appropriate surgical procedure. Transperineal ultrasonography (US) can be used to identify the internal fistula and to define the type of imperforate anus. Thus, US represents an useful noninvasive imaging modality for assisting in the diagnosis and appropriate management of this disease. We will describe this imaging technique and present different types of illus

Clinical features of skin lesions in rabbit syphilis: a retrospective study of 63 cases (1999-2003).

Skin lesions in rabbit syphilis are usually diagnostic, but it is occasionally difficult to differentiate these lesions from those of other skin diseases. Skin lesions in 63 cases of rabbit syphilis were analyzed for early and accurate diagnosis. Lesions were found most frequently around the nose (55 cases) followed by the genitalia (22), lips (20), eyelids (12), and anus (10). Sneezing was observed in 33% of cases with nasal lesions. In cases of maternally acquired infection, lesions could be initially found mainly on the face. Rabbits should be examined carefully not only for facial lesions,

[An Adult Case of Transperineal Repair of Congenital Rectourethral Fistula Using Gracilis Muscle Flap Interposition].

A man in his 50s was referred to our hospital after recurrent severe urinary tract infection. He had undergone anoplasty for anorectal malformation during early infancy. He noticed urinary leakage from the anus for a long time. Under diagnosis of congenital rectourethral fistula, we performed fistula closure. The fistula was transsected via transperineal incision and each stump was closed. A gracilis muscle flap approximately 30 cm long was harvested from the left thigh, brought into the deepest part between the separated rectum and urethra through a subcutaneous tunnel and fixed there. The ur

Eczema of the anus is symptomatic of different diseases, and pure symptomatic treatment should therefore not be carried out. After dermatological and proctological examinations have been performed, the correct diagnosis can be made and the therapy determined, which will then be successful.

Bilateral Transposition Flap for Postoperative Anal Stenosis after Reconstruction for Paget Disease.

One of the complications of anal surgery or disease is anal stenosis. To release the tension of the anus, a tension-releasing incision in the perianal skin and various anoplasty procedures are usually considered. The aim of this article is to describe a straightforward technique with local flaps for severe anal stenosis after anal reconstruction. A 57-year-old man presented to the clinic with diverticulitis secondary to severe anal stenosis, and reported difficulty with defecation after perianal skin resection around the anus and surgery to create a V-Y advancement flap for perianal primary Pa

Vulvar and Anal Intraepithelial Neoplasia: Terminology, Diagnosis, and Ancillary Studies.

Currently, it is recognized that there is an HPV-related and an HPV-independent pathway to developing squamous cell carcinomas (SCC) in the anus and vulva. The majority of precursor lesions and SCC in the anus and vulva are high-risk HPV-associated, with HPV16 the most common type. Given the morphologic overlap and biological equivalence of HPV-related preinvasive squamous lesions of the lower anogenital tract, a unified, 2-tiered histopathologic nomenclature is now recommended. In contrast, mutations in the TP53 gene have been associated with HPV-independent vulvar and anal SCC. A precursor l

Multimodality Imaging Review of Anorectal and Perirectal Diseases With Histological, Endoscopic, and Operative Correlation, Part I: Anatomy and Neoplasms.

A broad spectrum of pathology affects the rectum, anus, and perineum and understanding of its relevant anatomy is important in accurate reporting, particularly in rectal cancer. In this pictorial essay, correlative imaging, endoscopic, pathologic, and operative images are presented to illustrate normal anorectal anatomy and neoplastic conditions that affect the anus and rectum. A particular case-based focus is given to rectal adenocarcinoma with pelvic MR and surgical histopathology. Additionally, carcinoid tumor, gastrointestinal stromal tumor, condylomata acuminata, squamous cell carcinoma,

Review of Various Perianal Lesions on CT and MRI.

The perianal area consists of complex and diverse anatomical structures. Various conditions can affect this area, including inflammatory diseases, such as perianal abscesses, fistulas, and condyloma acuminata, and malignant tumors, such as mucinous adenocarcinoma, squamous cell carcinoma of the rectum, and squamous cell carcinoma of the anus. Due to the small size of the perianal area, lesions may be easily overlooked. However, with advancements in imaging technology, perianal lesions can now be identified using CT and MRI, aiding in the planning of treatment strategies. This pictorial essay a

Associations of intestinal diseases with anal diseases: a Mendelian randomization study.

Although observational clinical studies have established an association between Intestinal Diseases (IDS) and Anal Diseases (ADS), the causal relationship is still not fully understood due to the limitations of observational studies. Genome-wide association study (GWAS) statistical data for IDS and ADS were obtained from publicly available databases. To assess the causal effects of IDS on ADS, we conducted Mendelian randomization analysis. The inverse variance weighted method indicated that Inflammatory bowel disease (IBD) had a significant causal relationship with three kinds of ADS: Anorecta

New operative technique for Hirschsprung's disease.

A new technique for definitive surgery in Hirschsprung's disease is described. After transection of the rectum at 2 cm above the peritoneal reflection, the distal rectum is incised laterally on both sides in a longitudinal direction half way to the anus. The mucosal lining of the anterior rectum is partially removed. The posterior portion of the rectum is divided into two flaps by sagittal incision up to the mucocutaneous junction of the anus, which are then reflected and everted through the anus. The proximal colon is then pulled through, exteriorizing 5 cm of ganglionic colon. The exterioriz

Pathophysiologic correlates of thromboembolism in nonvalvular atrial fibrillation: I. Reduced flow velocity in the left atrial appendage (The Stroke Prevention in Atrial Fibrillation [SPAF-III] study).

Stroke associated with atrial fibrillation (AF) is mainly due to embolism of thrombus formed during stasis of blood in the left atrial appendage (LAA). Pathophysiologic correlates of appendage flow velocity as assessed by transesophageal echocardiography (TEE) in patients with AF have not been defined. To evaluate the hypothesis that reduced velocity is associated with spontaneous echocardiographic contrast and thrombus in the LAA and with clinical embolic events, we measured LAA flow velocity by TEE in 721 patients with nonvalvular AF entering the Stroke Prevention in Atrial Fibrillation (SPA
