Outcomes of symptomatic penetrating aortic ulcer and intramural hematoma in the endovascular era.

BACKGROUND: Although endovascular technology has resulted in a paradigm shift in treatment, medical management remains the standard of care for penetrating aortic ulcer (PAU) and intramural hematoma (IMH). This study aimed to detail the short- and long-term outcomes of symptomatic PAU/IMH. METHODS: Institutional data on symptomatic PAU/IMH were gathered (2005-2020). The primary outcome was the composite of recurrent symptoms, radiographic progression, intervention, rupture, and death from related or unknown cause. Factors associated with the primary outcome were determined using a Fine-Gray m

Early and late outcomes of acute type A aortic dissection with intramural hematoma.

INTRODUCTION: Controversy remains regarding management of acute type A dissection with intramural hematoma (IMH). Our purpose was to analyze our experience and report outcomes after repair of acute type A aortic dissection with IMH. METHODS: We analyzed all patients from a single center who underwent open repair for acute type A aortic dissection with IMH. RESULTS: Between 2000 and 2013, we performed 418 repairs for acute type A aortic dissection. These were divided into 2 groups of patients: 64 patients (15%) with type A IMH and 354 patients (85%) with typical dissection. Those with IMH wer

Type A intramural hematoma in the setting of acute type B aortic dissection.

Type A intramural hematoma (IMH) is an uncommon entity, the pathophysiology of which is thought to be related to a contained hemorrhage within the medial layer of the aorta as a result of either rupture of the vasa vasorum or an atherosclerotic plaque. We present a case of type A IMH in the setting of acute type B aortic dissection with suspicion for malperfusion syndrome and discuss the treatment algorithm of this uncommon entity.

Intramural posttraumatic hematoma of the ascending aorta in a patient with a double aortic arch.

We report a case of posttraumatic intramural hematoma (IMH) of the ascending aorta in a patient with a double aortic arch and coronary artery disease. This unusual and asymptomatic finding came to our observation because the patient presented an acute myocardial infarction and underwent coronary angiography. The diagnosis was performed by magnetic resonance imaging (MRI) which defined well the anatomy of aorta and showed the characteristics of the hematoma. The data were useful in planning the surgical repair.

Conservative management of Type B aortic intramural hematoma guided by imaging surveillance.

Aortic intramural hematoma is a life-threatening condition within the spectrum of acute aortic syndromes and is often misdiagnosed. We present the case of a 76-year-old man who attended the Emergency Department with mild, atypical chest pain. Initial assessment and laboratory tests were reassuring, and a musculoskeletal cause was considered; however, high blood pressure and diagnostic uncertainty prompted computed tomography angiography. Imaging revealed type B aortic intramural hematoma. The patient was managed conservatively with strict control of blood pressure and heart rate and close imag…

Type A aortic intramural hematoma: clinical features and outcomes in Chinese patients.

BACKGROUND: The purpose of this study was to describe the clinical characteristics and clinical outcomes for Chinese patients with type A intramural hematoma (IMH). METHODS AND RESULTS: We studied 90 patients with Stanford type A acute aortic syndrome who presented to our institution from 1998 to 2005 and evaluated the presentation, management, and clinical outcomes of acute IMH by comparing these patients with those diagnosed with classical aortic dissection (AD). A total of 34 patients had IMH and they tended to be older (69.7 ± 12.4 versus 60.5 ± 16.2 years; p=0.006). The development of pe

Clinical characteristics and outcomes in patients with acute type A aortic intramural hematoma.

BACKGROUND: Although type A acute aortic dissection (AAD) including classic double-channel aorta and intramural hematoma (IMH) is a life-threatening condition, the prognostic impact and predictors of IMH remain to be established. The present study evaluated the prevalence, baseline characteristics, and outcomes of IMH as compared with classic non-thrombosed type A AAD. METHODS: This multicenter registry in Japan retrospectively included 703 patients with type A AAD. IMH was defined as a crescentic or circular area along the ascending aortic wall without contrast enhancement on computed tomogr

Impact of thoracic endovascular aortic repair timing on aortic remodeling in acute type B aortic intramural hematoma.

OBJECTIVE: Thoracic endovascular aortic repair (TEVAR) is increasingly utilized in the management of acute type B aortic intramural hematoma (TBIMH). Optimal timing for intervention has not been described. The aim of this study was to evaluate TEVAR timing on postoperative aortic remodeling. METHODS: A retrospective chart review was performed on patients who underwent TEVAR for TBIMH from January 2008 to September 2018. Imaging was reviewed pre- and postoperatively. Primary data points included true lumen diameter (TLD) and total aortic diameter (TAD) at the site of maximal pathology. Primary…

Operative and hemostatic differences between acute type A intramural hematoma and aortic dissection.

BACKGROUND: Although acute type A intramural hematoma and classic aortic dissection often present with similar clinical features and are managed with similar operative strategies, their perioperative risk profiles may differ. Whether these differences translate into distinct surgical outcomes remains incompletely understood. METHODS: This retrospective single-center study included 351 patients who underwent open surgical repair for acute type A intramural hematoma or aortic dissection between February 2018 and December 2022, with follow-up through November 2023. Baseline characteristics, oper

Predictors of Intervention in Acute Type B Aortic Penetrating Ulcer and Intramural Hematoma.

We aimed to investigate predictors of intervention of acute type B aortic penetrating ulcer (PAU) and intramural hematoma (IMH). We conducted a retrospective chart review of all patients admitted for acute type B PAU or IMH in a tertiary referral hospital. Indications to intervention were "complicated" (rupture, impending rupture, malperfusion) or "high risk for unfavorable outcome" (refractory hypertension and/or pain despite best medical treatment, morphologic aortic evolution, transition to a new aortic syndrome, or increase in IMH/PAU depth >5 mm) during the acute/subacute phase. The prima

Case Series: Pulmonary Artery Intramural Hematoma in Stanford Type A Acute Aortic Dissection.

Main Teaching Point: Diagnosing acute ascending aortic dissection in patients with equivocal radiologic data may rely on associated findings such as pulmonary artery intramural hematoma. The immediate diagnosis of aortic dissection is paramount in its management. Its diagnosis may be challenging on computed tomography when the intimal flap, pathognomonic of dissection, is not readily visualized. Pulmonary artery intramural hematoma may arise from rupture of the posterior wall of the ascending aorta into the common aortopulmonary adventitia as a result of acute dissection. The clinical signific

Pancreaticoduodenal artery aneurysm associated with coeliac artery occlusion from an aortic intramural hematoma.

Pancreaticoduodenal artery aneurysms are a rare type of visceral artery aneurysm, whose rupture is associated with high mortality. These aneurysms are of particular interest because local haemodynamic change caused by coeliac artery obstruction plays an important role in their development. However, the pathophysiological mechanism of coeliac artery obstruction is not completely understood. Pressure from the median arcuate ligament is most frequently reported cause. Although it is well-known that stenosis or occlusion of the visceral vessels may be caused by aortic syndrome, reports of pancreat

Takayasu's arteritis mimicking aortic intramural hematoma in a female patient with chest pain.

We report the case of a young female patient who presented with chest pain, having a background of systemic complaints and a history of essential thrombocytosis. Transesophageal echocardiography (TEE) was suggestive of an intramural hematoma of the distal ascending aorta and transverse arch, confirming computed tomography findings. However, surgery revealed only diffuse aortic wall thickening without hematoma or dissection, despite an epiaortic ultrasonogram corroborating the TEE findings. Histopathology demonstrated inflammation involving all layers of the aortic wall, with intimal fibrosis,

Broken-crescent sign at CT indicates impending aortic rupture in patients with acute aortic intramural hematoma.

BACKGROUND: This retrospective study evaluated the computed tomography (CT) features and clinical implications of a novel broken-crescent sign in patients with acute aortic intramural hematoma (IMH). METHODS: Out of 104 patients with aortic IMH encountered in our institution between 2003 and 2018, nine patients exhibited a positive broken-crescent sign, which was defined as a focal defect within the hyper-attenuating crescentic IMH on unenhanced CT, corresponding to a smooth out-bulging of the aortic lumen on enhanced study. The clinical findings, CT features, and outcomes of these nine patie…

Thoracic Endovascular Aortic Repair for Type B Aortic Dissection Associated with Retrograde Type A Intramural Hematoma.

PURPOSE: To evaluate feasibility and efficacy of thoracic endovascular aortic repair (TEVAR) for type B aortic dissection (TBAD) associated with retrograde type A intramural hematoma (IMH). MATERIALS AND METHODS: From April 2013 to January 2017, 15 consecutive patients with TBAD associated with retrograde type A IMH who underwent TEVAR were reviewed retrospectively. There was no cardiac tamponade, aortic regurgitation, involvement of coronary artery, or sign of cerebral ischemia in these patients. Enhanced CT was used in 4 patients to diagnose malperfusion of abdominal visceral arteries or lo

Acute ascending aortic intramural hematoma as a complication of the endovascular repair of a Type B aortic dissection.

Endovascular aortic graft repair (EVAR) for patients with Type B aortic dissection is a less invasive surgical procedure (compared to traditional open surgical repair) that is associated with less morbidity and shortened recovery times. However, there are notable complications for the patients undergoing EVAR. We report a patient who was brought to our hospital with a Type B dissection and underwent a thoracic EVAR but suffered iatrogenic aortic injury resulting in cardiac tamponade. This case study highlights the importance of intraoperative transesophageal echocardiography to facilitate earl
