Aortic valve rupture has been previously reported in association with blunt chest trauma as well as spontaneously in the setting of abnormal valves. We present a case of a patient who required emergent aortic valve replacement following rupture of his aortic valve during an isometric muscle contraction.

Intra-aortic balloon pump is used frequently to support a failing myocardium in cardiac patients. Due to the invasive nature of this device, usage is accompanied by consistent risk of complications. Balloon rupture, although it occurs rarely, may lead to entrapment if diagnosis delays. Ascending aortic injury is seen in only 5% of patients who survive long enough to undergo imaging. Emergent aortography is commonly used to confirm and define suspected thoracic rupture. We describe a case of ascending aortic rupture at the root of the aorta following blunt trauma diagnosed with CT imaging.

Immediate surgical treatment of traumatic aneurysms of the aorta is in our point of view in most cases problematic, also because of the combination with life threatening injuries of other organ systems. In our own patient-collection seven patients out of 44 with traumatic transsection were immediately operated. Six patients died in tabula, three of them due to uncontrollable hemorrhage. Hitherto the size of abdominal aortic aneurysms (AAA) has been considered the most important factor in determining the risk of rupture. For this reason most interest has been devoted to physical, echographic and tomographic analyses of the shape of AAA. However, it is known that rupture can also occur in small AAA. Other factors must be considered to have an important role in the natural history of aneurysms. The aim of this study was to characterise the mechanical stress in the wall of an AAA due to pressure in the presence of atherosclerosis, intraluminal thrombus and anatomical restraints.

A case of aortic valve rupture as a result of blunt trauma by motorcycle accident is presented. A previously healthy 18-year-old male developed severe congestive heart failure 46 days after the initial trauma. There were crescentic tears in the right and non-coronary cusps parallel to and along the whole length of the cusps. The valve was excised and replaced with a 21 mm St. Jude Medical prosthesis. A case of late rupture of an abdominal aortic aneurysm that had been successfully treated by surgically induced thrombosis is presented. After emergency surgery consisting of ligation of the neck of the aneurysm, the patient survived and is doing well. The literature on surgically induced thrombosis of abdominal aortic aneurysm is reviewed. It is stressed that complete thrombosis of aortic aneurysm induced by distal ligation does not preclude rupture, and if patients present with symptoms of expansion or rupture, they should undergo emergency surgery.

We have described a case of malignant schwannoma (MS) originating from the posterior mediastinum with subsequent pathologic rupture of the thoracic aorta. The patient presented with acute retrosternal pain radiating to the back, and she died on the day of admission. Necropsy revealed a large hemothorax and a tumor mass involving the thoracic aorta. The histologic features of the lesion fulfilled the criteria for delineated MS. We found no similar cases of thoracic MS with consequent aorta rupture.

The principal radiological indication for thoracic aortography following blunt chest trauma has been a widened mediastinum on chest roentgenogram. Transcatheter aortic-valve implantation (TAVI) is a reliable alternative to surgical aortic-valve replacement in inoperable or high-risk patients. However, this evolving therapy is associated with a wide range of potential complications- some specific to TAVI, some often fatal. Prevention, early recognition, and taking essential precautions will significantly improve results. The case of an 85-year-old woman with balloon rupture during valvuloplasty in TAVI, a very rare and potentially fatal complication, is presented.
