Isotope localization of infarcts in aphasia.

Radionucleide localization of infarcts producing aphasia was undertaken in 65 patients with a scan-test and onset-test interval of one months or less. The scans were traced on anatomical templates without knowledge of the aphasias. A phasics were classified by their test scores according to taxonomic criteria, independently from localization. Scans belonging to each clinically distinct group were overlapped "blindly". The results showed distinct areas for Broca's conduction and Wernicke's aphasics along the parasylvian axis of the lateral templates. Lesions of global aphasics covered all these

Transcortical sensory aphasia: revisited and revised.

Transcortical sensory aphasia (TSA) is characterized by impaired auditory comprehension with intact repetition and fluent speech. We induced TSA transiently by electrical interference during routine cortical function mapping in six adult seizure patients. For each patient, TSA was associated with multiple posterior cortical sites, including the posterior superior and middle temporal gyri, in classical Wernicke's area. A number of TSA sites were immediately adjacent to sites where Wernicke's aphasia was elicited in the same patients. Phonological decoding of speech sounds was assessed by audito

[Crossed aphasia in a dextral patient--a case of sensory aphasia due to a right hemispheric lesion].

An extremely rare case of sensory aphasia due to a right hemispheric lesion in a 61-year-old, right-handed male is reported. He manifested jargon aphasia, constructional apraxia and ideational apraxia after an operation for right temporo-parietal cerebral hemorrhage. Standard language test of aphasia (SLTA) administered 6 months after the onset revealed disturbance of verbal comprehension and repetition, fluent jargon with many paraphasias and paragraphias. The brain CT, MRI and IMP-SPECT disclosed right temporo-parietal and angular lesions. There are two other reported cases of right posterio

An atypical presentation of Wernicke-Korsakoff encephalopathy mimicking a left hemispheric stroke: case report.

BACKGROUND: Wernicke-Korsakoff encephalopathy is a metabolic disease caused by vitamin B1 deficiency that predominantly affects alcoholic patients. Its clinical picture is characterized mainly by altered mental status with memory deficits, ophthalmoparesis, and ataxia, although other clinical manifestations may also be present. The current case presents certain clinical difficulties regarding the diagnosis when confronting an atypical presentation of a classical disease in an acute setting when a decision to administer an intravenous thrombolytic agent needs to be made. CASE PRESENTATION: Thi

Impaired semantic control in the logopenic variant of primary progressive aphasia.

We investigated semantic cognition in the logopenic variant of primary progressive aphasia, including (i) the status of verbal and non-verbal semantic performance; and (ii) whether the semantic deficit reflects impaired semantic control. Our a priori hypothesis that individuals with logopenic variant of primary progressive aphasia would exhibit semantic control impairments was motivated by the anatomical overlap between the temporoparietal atrophy typically associated with logopenic variant of primary progressive aphasia and lesions associated with post-stroke semantic aphasia and Wernicke's a

[Conduction aphasia and phonemic disorder].

Conduction aphasia is usually described as a repetition impairment. Semiology or pathophysiology cannot be explained with this definition. We report a single case particularly demonstrative. The patient showed spontaneous speech, denomination, repetition and reading impairments. Main errors were phonemic paraphasia. No arthric disorder nor comprehension impairment was observed. Damage of supramarginalis gyrus and Wernicke's area was found. A cognitive analysis suggested that the phonological buffer and the working memory were impaired. Implication for rehabilitation, which included segmentatio

A critical appraisal of neuropsychological correlates of Japanese ideogram (kanji) and phonogram (kana) reading.

Owing to the Japanese language's unique writing system, which consists of phonograms and ideograms, reading impairments of Japanese brain-damaged patients have attracted the interest of many researchers. Past case reports as well as some widely accepted handbooks and textbooks have concluded that a specific aphasia type or lesion site is associated with a particular impairment pattern of phonograms and ideograms in reading. However, the methodology and analytical procedures in previous studies were inadequate for reliable generalizations to be made. First, the test materials were unspecified o

Relating selective brain damage to impairments with voicing contrasts.

Research is reviewed concerning the performance of several neurological groups on the perception and production of voicing contrasts in speech. Patients with cerebellar damage, Parkinson's disease, specific language impairment, Broca's aphasia, apraxia, and Wernicke's aphasia have been reported to be impaired in the perception and articulation of voicing. The types of deficits manifested by these neurologically impaired groups in creating and discriminating voicing contrasts are discussed and the respective contributions of separate neural areas are identified. A model is presented specifying

Apraxia and aphasia: the functional-anatomical basis for their dissociation.

We studied the functional and anatomical relationship between aphasia and apraxia in 177 patients with CT evidence of left hemisphere stroke. In six severe aphasics, praxis was spared; these cases were analyzed in detail. One patient had a small temporal lesion with severe Wernicke's aphasia. Large frontoparietal lesions were found in the others. The sparing of praxis suggested bilateral representation of visuokinesthetic motor patterns, and functionally active right parietofrontal connections. Some of these patients had uncommon patterns of skull asymmetries that may have been related to bila

Praxis and language: the extent and variety of apraxia in aphasia.

This quantitative study of apraxia in aphasics establishes the extent and characteristics of apraxia in various aphasic groups, the differences between various categories of praxis, and the correlation between praxis and language parameters. Broca's aphasics have more severe apraxia than Wernicke's aphasics even though their comprehension is significantly better. Apraxia, however, correlates best with comprehension deficit and secondly with the severity of aphasia. Among the nonverbal parameters, drawing shows the highest correlations. Our study confirms the prominence of facial apraxia in Bro

Lesion correlates of patholinguistic profiles in chronic aphasia: comparisons of syndrome-, modality- and symptom-level assessment.

One way to investigate the neuronal underpinnings of language competence is to correlate patholinguistic profiles of aphasic patients to corresponding lesion sites. Constituting the beginnings of aphasiology and neurolinguistics over a century ago, this approach has been revived and refined in the past decade by statistical approaches mapping continuous variables (providing metrics that are not simply categorical) on voxel-wise lesion information (voxel-based lesion-symptom mapping). Here we investigate whether and how voxel-based lesion-symptom mapping allows us to delineate specific lesion p

Clinicoanatomical correlation in stroke related aphasia.

CONTEXT: With advances in neuroimaging, traditional views regarding the clinicoanatomic correlation in stroke patients with aphasia are being challenged and it has been observed that lesions at a given cortical or subcortical site may manifest with different aphasia profiles. AIMS: To study as to whether there is a strict clinicoanatomical correlation between the type of aphasia and lesion site in patients with first ever stroke. SETTINGS AND DESIGN: Observational study, based in a tertiary care center. MATERIALS AND METHODS: Stroke patient's ≥18 years of age were screened and those with fi

Auditory agnosia with anosognosia.

A 66-year-old right-handed female medical doctor suffered two consecutive cardioembolic strokes, initially affecting the right frontal lobe and the right insula, followed by a lesion in the left temporal lobe. The patient presented with distinctive phenomenology of general auditory agnosia with anosognosia for the deficit. She did not understand verbal requests and her answers to oral questions were fluent but unrelated to the topic. However, she was able to correctly answer written questions, name objects, and fluently describe their purpose, which is characteristic for verbal auditory agnosi

[A suddenly nonsensical speaking man].

A 61-year-old man presented to the emergency department with acute fluent but nonsensical speech and intact comprehension. Imaging showed a contrast-enhancing lesion near Wernicke's area, suspect for an intracranial neoplasm, without signs of ischemia or hemorrhage. Suspecting focal epilepsy with conduction aphasia, symptoms resolved after anti-convulsive therapy.

[A case of fully recovered non-herpetic subacute encephalitis along with aphasia].

We report the case of a 39-year-old female with non-herpetic subacute encephalitis. Aphasia occured after a slight fever, and severe epilepsy followed 3 months later. Wernicke's aphasia was noted at the recovery stage, but the patient showed almost complete recovery. Head MRI revealed a lesion from the left parietal to the left posterior temporal lobe. Improvement in the left hemisphere noted on the encephalograms was consistent with that of aphasia. While amnesia was not found, aphasia, ideomotor apraxia and limb-kinetic apraxia were noted. There was no involvement of herpes simplex types 1 a

Clustering Aphasic Speech: A Comparative Study of Feature Extraction Techniques for Fluent and Non-Fluent Categories.

Aphasia assessment plays a crucial role in rehabilitating people with aphasia (PWA), including classifying healthy individuals, identifying subtypes of aphasia, and assessing severity. This study explores unsupervised clustering methods in aphasic speech data, comparing five feature extraction approaches and four clustering algorithms. Hierarchical Density-Based Spatial Clustering of Applications with Noise (HDBSCAN), Agglomerative clustering, K-means, and Gaussian Mixture Models. The focus is on distinguishing between fluent and non-fluent aphasia and subtypes such as Broca's, Wernicke's, and

Effectiveness of Abstract Semantic Associative Network Training (AbSANT) in improving lexical retrieval in Mandarin-speaking aphasia: A multiple-baseline single-case study.

PURPOSE: Semantic Associative Network Training (AbSANT) is evidence-based in alphabetic languages, but its applicability to Mandarin-speaking individuals with aphasia remains unclear. Since Mandarin compound words often share morphemes across abstract and concrete concepts, the morphological structure of Mandarin may uniquely enhance semantic generalisation. METHOD: Using a multiple-baseline single-case design, we evaluated treatment outcomes via effect sizes for two participants with anomic aphasia) and Wernicke's aphasia. RESULT: Both participants significantly improved in retrieving abstr

Language and visuospatial impairment in a case of crossed aphasia.

Crossed aphasia in dextrals (CAD) constitutes an interesting model for understanding the lateralisation and interaction of language with other cognitive functions in the brain. The authors present a study of a right-handed patient with Wernicke's aphasia following a right hemisphere cerebrovascular accident, who also had impairment in visuospatial skills. Although the patient presented a remarkable improvement in language symptoms on longitudinal follow-up, the combination of linguistic, visuospatial, and attentional impairments ultimately resulted in a persisting inability to perform complex
