Appendiceal mucinous cystadenoma is a neoplastic complication of IBD: case-control study of primary appendiceal neoplasms.

BACKGROUND: IBD is a risk factor for development of colorectal neoplasia. Although IBD frequently involves the appendix microscopically, it is uncertain whether it also predisposes to appendiceal neoplasia. METHODS: We performed a retrospective case-control study of incidental appendiceal neoplasms in colectomy specimens of adults with and without IBD (cases and controls, respectively) based on surgical pathology records spanning 54 months. To minimize referral bias, patients were excluded if they had preoperative clinical evidence or a principal pathologic diagnosis of appendiceal disease. T

Retrospective analysis of the appendiceal neoplasms: sampling technique may influence neoplasm detection.

BACKGROUND AND AIMS: Appendiceal neoplasms are uncommon entities that are usually determined incidentally during the histopathological examination. Different techniques used for the macroscopic sampling of appendectomy material may affect the determinating neoplasms. MATERIALS AND METHODS: H&E-stained slides of 1280 cases who underwent appendectomy between 2013 and 2018 were reviewed retrospectively for histopathological features. RESULTS: Neoplasms were determined in 28 cases (3.09%); 1 lesion was observed in the proximal part of the appendix, 1 covering the entire length from proximal to d

High-grade appendiceal mucinous neoplasms in children: a case report.

BACKGROUND: Appendiceal Mucinous Neoplasms (AMNs) are a rare disease characterized by the accumulation of mucus within the vermiform appendix and are frequently misdiagnosed as appendicitis. Hence, it is crucial to consider AMNs because they have the potential to progress into peritoneal pseudomyxoma (PP), a clinical syndrome distinguished by mucus buildup in the peritoneum leading to progressive abdominal pathology. CASE REPORT: We present a case involving a 13-year-old male patient who was initially suspected of having acute purulent appendicitis prior to surgery, a formal laparoscopic appe

Correlation of molecular and morphological features of appendiceal epithelial neoplasms.

AIMS: The aims of this study were to identify the genetic features of appendiceal epithelial neoplasms and correlate the genetic features with morphology. METHODS AND RESULTS: We analysed the genetic features of a series of 47 appendiceal epithelial neoplasms of various morphologies by using targeted next-generation sequencing of 11 genes commonly mutated in gastrointestinal neoplasms. Seven of nine serrated polyps harboured BRAF mutations, which are rare in other types of appendiceal tumours. Most cases of low-grade appendiceal mucinous neoplasms (LAMNs) exhibited GNAS and KRAS mutations. LA

Clinicopathological spectrums and prognosis of primary appendiceal adenocarcinoma, goblet cell adenocarcinoma, and low-grade appendiceal mucinous neoplasms.

Primary appendiceal adenocarcinoma (APCA), goblet cell adenocarcinoma (GCA), and low/high-grade appendiceal mucinous neoplasms (LAMN/HAMN) are distinct entities with overlapping clinical presentation and histomorphology, leading to diagnostic challenges. We retrospectively reviewed our archived cases between 2010 and 2018 for diagnosis reappraisal and comparative analysis using updated terminology and modern parameters. A total of 87 cases (22 APCA, 40 GCA, and 25 LAMN pT≥3) were included. The entire cohort had 49 women and 38 men with a median age of 59.9 (range 26-88) years. There were no st

Dysplastic intestinal-type metaplasia of appendiceal endometriosis: a mimic of low grade appendiceal mucinous neoplasm.

UNLABELLED: We report an example of dysplastic intestinal-type metaplasia involving endometriosis of the appendix in a 45 year old woman. One other example of this phenomenon has been reported. As it occurs within the muscular wall of the appendix, confusion with low grade appendiceal mucinous neoplasm (LAMN) may occur. Evidence supporting the metaplastic nature of the intestinal epithelium is offered. As the initial pathological diagnosis was of invasive cancer with perforation of the appendix treatment consisted of peritonectomy and hyperthermic intraperitoneal chemotherapy (HIPEC). VIRTUAL

Clinicopathologic and Molecular Characteristics of High-Grade Appendiceal Mucinous Neoplasms.

BACKGROUND: Although 2016 consensus guidelines defined high-grade appendiceal mucinous neoplasms (HAMNs) as a distinct histologic entity, their clinical course and optimal management remain poorly characterized. Comparative data with low-grade appendiceal mucinous neoplasms (LAMNs) are limited. This study aims to detail the clinical management and outcomes of patients with HAMN and those with LAMN. METHODS: This retrospective cohort study was conducted at a large academic cancer center. An internal database was queried to identify patients with LAMN or HAMN who underwent appendectomy between

Examination of Appendiceal Neoplasms-A Retrospective, Single-Centre, Cohort Study.

BACKGROUND: Appendiceal neoplasms are rare, but recent data indicate a rising trend, particularly in patients under the age of 50. These tumours are often diagnosed incidentally during histopathological examination. This study has aimed to examine the incidence and histological subtypes of appendiceal neoplasms in a Southern Hungarian population. METHODS: Our study included neoplastic appendix specimens processed at the University of Szeged between 2014 and 2023. RESULTS: Neoplasms were identified in 71 cases from 3640 appendectomies (1.9%). Benign lesions were present in 37% of cases (n = 2

Risk stratification of surveillance for low-grade appendiceal mucinous neoplasms.

BACKGROUND: Low-grade appendiceal mucinous neoplasms (LAMNs) can progress to pseudomyxoma peritonei (PMP). The incidence and risk factors for recurrence are unclear, and there is a lack of consensus on the need and duration of surveillance imaging. METHODS: Patients at the Mayo Clinic in Rochester, Minnesota, with a histologic diagnosis of LAMNs confined to the appendix and limited involvement of the right lower quadrant from 1992 to 2023 were included. Associations between recurrence and risk factors were assessed using Kaplan-Meier curves and Cox proportional hazards regression. RESULTS: A

Optimal Surgical Approaches for Incidentally Discovered Ruptured Mucinous Appendiceal Neoplasms.

Mucinous neoplasms of the appendix, including low-grade appendiceal mucinous neoplasms (LAMNs), are rare but significant due to their potential for peritoneal dissemination. These tumors are often discovered incidentally during imaging or surgery for unrelated conditions. The presence of mucinous material in the peritoneum raises concern for pseudomyxoma peritonei, necessitating careful intraoperative and postoperative management. A 52-year-old male presented with testicular pain, leading to a diagnosis of seminoma. Staging computerized tomography (CT) revealed a dilated appendix with a surrou

The Frontiers of Appendiceal Controversies: Mucinous Neoplasms and Pseudomyxoma Peritonei.

Appendiceal mucinous neoplasms show a range of morphologic features and biological risk. At one end of the spectrum, high-grade adenocarcinomas are cytologically malignant with infiltrative invasion, lymph node metastases, and behavior similar to that of extra-appendiceal mucinous adenocarcinomas. At the other end, mucinous neoplasms confined to the mucosa are uniformly benign. Some cases lying between these extremes have potential risk to metastasize within the abdomen despite a lack of malignant histologic features. They show "diverticulum-like," pushing invasion of mostly low-grade epitheli

Necessity of posttreatment surveillance for low-grade appendiceal mucinous neoplasms.

BACKGROUND AND OBJECTIVES: Low-grade appendiceal mucinous neoplasms (LAMNs) are generally treated by surgical resection, but posttreatment surveillance protocols are not well-established. The objectives of this study were to characterize posttreatment surveillance and determine the risk of recurrence following surgical resection of LAMN. METHODS: Patients who underwent surgical resection of localized LAMNs in an 11-hospital regional healthcare system from 2000 to 2019 were identified. Posttreatment surveillance regimens were characterized, and rates of disease recurrence were evaluated. RESU

Prophylactic hyperthermic intraperitoneal chemotherapy in patients with epithelial appendiceal neoplasms.

Background Prophylactic hyperthermic intraperitoneal chemotherapy (HIPEC) is a promising approach for preventing peritoneal carcinomatosis in high-risk patients. We report our initial experience with prophylactic HIPEC in a series of patients with appendiceal neoplasms. Methods We retrospectively reviewed our prospectively maintained database to identify patients who underwent HIPEC in the absence of peritoneal disease. Patients with previously documented peritoneal surface disease were excluded. Data regarding clinical, operative and pathological features were analysed. Results Out of 322 HIP

High-grade appendiceal mucinous neoplasm: case report.

High-grade appendiceal mucinous neoplasm is an extremely rare gastrointestinal tumour with nonspecific clinical manifestations that complicate preoperative diagnosis. In women, it may mimic gynecological diseases, particularly ovarian tumours, leading to diagnostic delays and inappropriate surgical approaches. We report the case of a 68-year-old woman from the San Martín region of Peru who presented with abdominal pain, progressive distension, and abnormal vaginal bleeding. Laboratory tests showed elevated carcinoembryonic antigen levels, while transvaginal ultrasound revealed a right adnexal

An Uncommon Confluence: Splenic Lymphangioma and Appendiceal Mucinous Neoplasm.

Splenic lymphangiomas, per se, are exceedingly rare, benign congenital malformations of lymphatic vessels, typically diagnosed in childhood or incidentally in adulthood. Appendiceal mucinous neoplasms (AMNs) are also infrequent, often asymptomatic, and usually discovered incidentally. To date, the co-occurrence of these two distinct primary lesions in the same patient has not been previously reported. We present the case of an 83-year-old Turkish female who underwent emergent abdominal surgery for an acute abdomen secondary to a strangulated left diaphragmatic herniation. Multiple perforations

Surgical management of appendiceal mucinous neoplasm: Is appendectomy sufficient?

BACKGROUND AND OBJECTIVES: Due to the rarity of appendiceal mucinous neoplasms (AMNs), there are few established treatment guidelines. The clinical course varies from incidental detection to progressive spread with pseudomyxoma peritonei (PMP). This study investigated the extent of resection on the prognosis and outcomes of AMNs. METHODS: This multicenter retrospective study evaluated patients with AMN who underwent surgery between 4/2006 to 9/2017. Primary endpoints included overall survival (OS) and disease-specific survival (DSS). Secondary endpoints included PMP incidence and treatment wi
