A multi-method intervention to reduce no-shows in an urban residency clinic.

BACKGROUND AND OBJECTIVES: Missed appointments can create financial, capacity, and continuity issues in primary care. An urban family medicine residency teaching clinic with a large culturally diverse population of low-income patients struggled for decades with a persistent no-show rate of 15%--17% despite multiple attempts to remind patients or otherwise address the problem. This study sought to measure the effects of a multi-method approach to decreasing the overall clinic no-show rate over time. METHODS: A team of clinicians and staff undertook a systematic review of the literature to iden

Understanding women's decisions to seek an endometriosis diagnosis: A Health Belief Model approach.

Early diagnosis of endometriosis is crucial, yet limited literature exists on factors influencing women's decisions to seek diagnosis. This study explores the role of symptoms, health beliefs, and social influences in this decision-making process. A cross-sectional online survey was completed by 388 women (median age = 22 years; interquartile range = 10; range = 18-50). Data included sociodemographic characteristics, medical information, and Health Belief Model constructs. Multivariable regression analyses were conducted to identify predictors of the intention to schedule an appointment and of

Outreach engagement efforts: are they worth the effort?

The present study was conducted to evaluate the efficacy of an outreach strategy designed to engage substance-abusing individuals who did not keep their initial treatment appointment. After missing their first scheduled appointment, telephone contact was attempted for 267 consecutive prospective patients as part of an attempt to engage them in treatment. Results indicated that this telephone contact was useful at prompting these individuals to both reschedule and arrive for intake. Our concern that persons who failed to keep initial appointments might be less motivated for treatment was not su

Online Scheduling and the Digital Front Door to Unequal Care.

Online appointment platforms are often viewed as tools of convenience, but they may reproduce and potentially amplify inequities rooted in underlying insurance and reimbursement structures. In this commentary, we highlight implications from a study using simulated profiles of patients with statutory health insurance (SHI) and private health insurance (PHI) to conduct an internet-based audit of online appointments. Not only was the time to the first available appointment longer for SHI than PHI patients, but also the PHI profile had access to more listings with appointments, and listings offeri

Patient navigation to improve diabetes outpatient care at a safety-net hospital: a retrospective cohort study.

BACKGROUND: Recent emphasis on value based care and population management, such as Accountable Care Organizations in the United States, promote patient navigation to improve the quality of care and reduce costs. Evidence supporting the efficacy of patient navigation for chronic disease care is limited. The objective of this study was to evaluate the effect of a patient navigation program on medical and administrative outcomes among patients with diabetes in an urban, safety-net hospital clinic setting. METHODS: A retrospective cohort study with pre- and post-intervention periods was conducted

A Pilot Randomized Controlled Trial to Assess the Impact of Motivational Interviewing on Initiating Behavioral Therapy for Migraine.

BACKGROUND: Relaxation, biofeedback, and cognitive behavioral therapy are evidence-based behavioral therapies for migraine. Despite such efficacy, research shows that only about half of patients initiate behavioral therapy recommended by their headache specialists. OBJECTIVE: Motivational interviewing (MI) is a widely used method to help patients explore and overcome ambivalence to enact positive life changes. We tested the hypothesis that telephone-based MI would improve initiation, scheduling, and attending behavioral therapy for migraine. METHODS: Single-blind randomized controlled trial

Effect of text message reminders to improve paediatric immunisation rates: a randomised controlled quality improvement project.

Previous studies have demonstrated that text message reminders can improve pediatric vaccination rates, including low income & diverse settings such as those served by federally qualified health centers. In this study, we aimed to improve compliance with routine childhood immunizations via a text message intervention in a network of urban, federally qualified health centers at a large academic medical center. We targeted parents or guardians of children aged 0-2 years who were overdue or due within 14 days for at least one routine childhood immunization without a scheduled appointment. In Roun
