Immediate closure of traumatic upper arm and forearm injuries with the latissimus dorsi island myocutaneous pedicle flap.

This paper reports the authors' experience with latissimus dorsi island pedicle flaps in the acute treatment of massive arm injuries. Seven patients with upper arm injuries and four patients with forearm injuries were treated with latissimus dorsi pedicle flaps. All cases involved massive soft-tissue loss and open fractures. Primary healing of wounds occurred without complications in 10 of 11 patients; the eleventh developed a wound infection. There were no instances of flap loss or vascular complications. This report compares and discusses surgical management options and details the importanc

The effect of eccentric arm cycling on muscle damage and injury-related biomarkers.

PURPOSE: There is a scarcity of information regarding the effect of upper-body eccentric exercise on biomarkers of muscle damage. This study sought to investigate the effect of eccentric arm cycling on muscle damage [exercise-induced muscle damage (EIMD)]. METHOD: Ten subjects performed a 15 min eccentric arm cycling protocol (cadence 49 ± 7 rpm, power absorbed 248 ± 34 W). Maximal voluntary contraction (MVC) of the elbow flexors was evaluated at rest and at 5 min, 24 h, and 48 h post-exercise. In addition, blood samples were drawn at rest and thereafter at 30 min, 24 h, and 48 h intervals af

Risk Factors for Moral Injury Among Canadian Armed Forces Personnel.

OBJECTIVES: The traumatic nature of high-risk military deployment events, such as combat, is well-recognized. However, whether other service-related events and demographic factors increase the risk of moral injury (MI), which is defined by consequences of highly stressful and morally-laden experiences, is poorly understood. Therefore, the objective of this study was to examine determinants of MI in Canadian Armed Forces (CAF) personnel. METHODS: Data were obtained from the 2018 Canadian Armed Forces Members and Veterans Mental Health Follow-up Survey (CAFVMHS; unweighted n = 2,941). To identi

Fireworks injuries, active component, U.S. Armed Forces, 2008-2017.

Although fireworks shows are considered patriotic and festive, the use of consumer fireworks also can be dangerous. There were 302 records of fireworks injuries during the 10-year surveillance period (2.4 cases per 100,000 person- years [p-yrs]). During 2008-2017, the lowest crude annual incidence of fireworks injury was reported in 2010 (1.5 per 100,000 p-yrs) and the highest was reported in 2017 (3.4 per 100,000 p-yrs). Compared with their respective counterparts, overall incidence of fireworks injury was higher among males, non-Hispanic whites, Army members, those in an enlisted rank, and t

Update: Heat injuries, active component, U.S. Armed Forces, 2014.

The incidence rate of heat stroke among active component service members in 2014 was slightly higher than in 2013 but similar to the rates in 2011 and 2012. Incidence rates of heat stroke were higher among males, those younger than 20 years of age, Asian/Pacific Islanders, Marine Corps and Army members, and service members in combat-specific occupations, compared to their respective counterparts. Fewer service members were treated for "other heat injuries" in 2014 (n=1,683) than in any other year of the 5-year surveillance period. In addition, there were fewer reportable medical events for "ot

Systemic reperfusion injury during arm replantation requiring intraoperative amputation.

This case describes the attempted replantation of an arm following its traumatic amputation. After a five-and-a-half hour ischaemic time, perfusion of the arm was re-established. However over the ensuing hours, the patient developed hyperkalaemia, acidaemia and severe hypotension requiting high dose inotropic support. Ultimately re-amputation was necessary to treat the systemic effects of the ischaemia reperfusion syndrome.

A new approach to categorization of ocular injury among U.S. Armed Forces.

This report describes a new approach to categorizing ocular injury using Military Health System data, the application of an algorithm to a dataset, and the verification of the results using an audit of clinical data. Based on health care encounter data, an algorithm was developed to systematically document the occurrence of specific complications and medical procedures in the 12 months following initial ocular injuries. The injuries were classified into 1 of 2 groups: "uncomplicated injury" with no complications or medical procedures and "complicated injury" with complications and/or medical p

Double floating arm injury in a child: a case report and review of the literature.

The combination of ipsilateral humeral fractures at three different levels namely proximal, shaft and supracondyle has been rarely defined in the literature. We present a case report on such a complex injury in a 10-year-old child after falling down from the second floor of his house while playing. To the best of our knowledge, no such case report exists in the English literature. We define it as double floating arm injury. Firstly, shaft of humerus was open reduced and fixed with 4.5 mm narrow dynamic compression plate. Then closed reduction and pinning of the supracondylar humerus under an i

Disseminated Nocardia asteroides infection in an immunocompetent woman following an arm injury.

A case of disseminated infection with Nocardia asteroides in a 55-year-old immunocompetent woman after mild trauma to the arm is reported. Secondary dissemination was identified in the skin, right kidney, liver, peritoneal cavity, lungs and thigh. The patient was successfully treated with surgical drainage and a 9-week course of antibiotics including cefotaxime, amikacin, chloramphenicol, trimethoprim/sulfamethoxazole (TMP/SMX) and doxycycline. The administration of TMP/SMX in combination with doxycycline was clinically beneficial despite in vitro resistance.

Computer-assisted evaluation of hand and arm function after thermal injury.

Comprehensive care of the burned upper extremity requires accurate and complete evaluation of function, including two-point discrimination, active and passive range of motion, and grip strength. These evaluations, when performed serially during a course of therapy, are time-consuming and manpower-intensive. We tested the utility and accuracy of a commercially available computer-assisted impairment evaluation system when used to automate and standardize measurement of upper-extremity function. The function of 80 upper extremities was evaluated with both the conventional and the computer-assiste

Application of a Modified Pedicled Adipofascial Lateral Arm Flap in the Prevention and Treatment of Radial Nerve Injuries.

The fasciocutaneous lateral arm flap is a workhorse flap in upper extremity reconstruction. However, its adipofascial variant is not widely used. The technique can be used in various clinical scenarios. The adipofascial flap can be transposed to circumferentially wrap the radial nerve with a pliable, vascularized fat and fascial envelope, mimicking the natural fatty environment of peripheral nerves. This technique has the advantage of providing a scar tissue barrier, a barrier to hardware irritation and a milieu for vascular regeneration of the nerve. Suggested applications include nerve cover
