The genetics of cardiac arrhythmias.

Cardiac rhythm problems result in high levels of morbidity and mortality, with sudden arrhythmic death claiming approximately 300,000 lives in the United States each year. Investigations into the genetic contributions to rhythm and conduction disorders have found genes or loci associated with primary rhythm/conduction disorders such as familial atrial fibrillation and atrio-ventricular block, underscoring the importance of collecting a thorough family history. Combinations of single or multiple genes and environmental risk factors may place only certain family members at risk. Some cardiac mus

[Stress, depression and cardiac arrhythmias].

A relationship between behavioural factors and cardiac arrhythmogenesis in humans has been described. Three sets of conditions contribute to the occurrence of arrhythmias: myocardial electrical instability, most often due to coronary artery disease; an acute triggering event, frequently related to mental stress; and a chronic, pervasive, and intense psychological state, often including depression and hopelessness. The autonomic nervous system plays an important role in the occurrence of cardiac arrhythmias and it is well documented that mood alterations as mental stress and depression influenc

Catheter ablation for cardiac arrhythmias.

The clinical introduction of catheter ablation in 1981 revolutionized the treatment of cardiac arrhythmias. Implementation of radiofrequency as an alternative energy source, with the advantages of higher selectivity and less collateral damage, provided an expansion of catheter ablation therapy. Today the majority of arrhythmias can potentially be cured with catheter ablation therapy. The safety and efficacy of catheter ablation for treatment of AV nodal reentrant tachycardia, accessory pathway arrhythmias, focal atrial tachycardia, atrial flutter and idiopathic ventricular tachycardia, is well

Cardiac arrhythmias in cardiothoracic surgery.

Most patients with cardiopulmonary disease are predisposed to develop perioperative arrhythmias with the individual patient risk depending upon the type of operative procedure performed, the risk profile of the patient, and the complexity of the post-operative course. There are several management options that may tend to prevent perioperative arrhythmias that should be considered in certain patient subsets. Most important of these is the use of beta-blocker therapy before and after operation in patients with coronary risks factors undergoing non-cardiac thoracic procedures and in patients havi

Postoperative Cardiac Arrhythmias in Children.

The cardiac rhythm can be very labile in the early postoperative period, necessitating a high degree of vigilance. Bradycardia may be due to sinus node dysfunction or varying degrees of atrioventricular block, both of which are usually due to surgical trauma. Temporary pacing, using implanted temporary pacing wires, should be readily available. Most often, bradycardias are transient. Patients who fail to recover an adequate sinus rhythm or atrioventricular conduction within 7 to 10 days should be treated with a pacemaker. Supraventricular tachycardia, ventricular tachycardia, and junctional ec

Primary cardiac arrhythmias in children.

OBJECTIVES: Primary cardiac arrhythmias are much less common in children than adults. This study was performed to identify the characteristics of primary arrhythmias in pediatric patients in the Emergency Department (ED). METHODS: Retrospective review of ED visits of patients <18 years of age presenting between January 1991 and November 1996 with an Emergency Department diagnosis of primary cardiac arrhythmia. Secondary cardiac arrhythmias, ie, sinus tachycardia resulting from a fever, were excluded. Clinically significant arrhythmias were defined as those capable of altering cardiovascular s

Adenosine in treating cardiac arrhythmias.

1. Adenosine is an endogenous nucleoside which causes a brief blockade of the AV nodal conduction pathway following intravenous administration. 2. Such a brief AV block can be used therapeutically for reliable termination of AV nodal re-entry tachycardia and WPW re-entry tachycardia. It can also be used for demasking atrial activity in rapid suspected supraventricular tachycardia with a broad QRS complex or a Delta wave, not present during sinus rhythm with normal AV node conduction, indicating the presence of a hidden WPW syndrome. 3. Side effects after adenosine application are frequent, but

Cardiac pacing and atrial arrhythmias.

Pacemaker patients frequently have concomitant atrial tachyarrhythmias, most importantly atrial fibrillation, because numerous underlying electrical or structural diseases can potentiate both conditions. DDD pacing (or atrial) appears superior to VVI in limiting the occurrence of atrial fibrillation and its associated negative sequela. Sick sinus syndrome, a prior history of atrial fibrillation, and advanced age predispose patients with DDD devices to postimplant atrial fibrillation, yet the majority of these high-risk patients are manageable in DDD throughout their follow-up. DDD patients at

[Postoperative cardiac arrhythmias (author's transl)].

13 male patients suffering from arteriosclerotic heart disease and/or arterial hypertension were monitored continuously before and after vascular surgical procedures using an arrhythmia computer. Heart rate, paroxysmal supraventricular tachycardias, ventricular extrasystoles, ventricular tachycardias, ventricular fibrillation and prematurity index (QnQe/QTn) were recorded numerically. Ventricular arrhythmias were detected as follows preoperatively in 12 patients, after operation in all patients, paired ventricular extrasystoles or episodes of ventricular tachycardia were found in 5 cases befor

Cardiac arrhythmias in acute stroke.

Cardiac arrhythmias were more frequent (p less than 0.001) in 312 stroke patients admitted to an intensive care stroke unit, than in 92 patients admitted to the unit and subsequently found not to have strokes. This significant difference remained when a stroke subgroup and the non-stroke group were matched for age, sex and duration of stay in the unit (P less than 0.005). Hypertension and hypertensive cardiac disease were more common in the stroke than in the non-stroke patients (P less than 0.001). Ectopic beats and atrial fibrillation, as well as other arrhythmias, were most frequent in pati

Cardiac Arrhythmias and Sleep Disorders.

Sleep disordered breathing (SDB), periodic limb movements of sleep, insomnia, sleep curtailment, and sleep architectural changes are associated with cardiac arrhythmias in community-based and clinical cohort studies. Findings appear to be strongest in magnitude and most consistent for SDB and atrial fibrillation (AF). The mechanisms most consistently identified in animal experimental models include autonomic nervous system fluctuations and intrathoracic pressure alterations, the latter resulting in direct mechanical effects on the thin-walled atria. Other studies point toward intermittent hypo

Human Genetics of Cardiac Arrhythmias.

Inherited forms of cardiac arrhythmias mostly are rare diseases (prevalence <1:2000) and considered to be either "primary electrical heart disorders" due to the absence of structural heart abnormalities or "cardiac ion channel disorders" due to the myocellular structures involved. Precise knowledge of the electrocardiographic features of these diseases and their genetic classification will enable early disease recognition and prevention of cardiac events including sudden cardiac death. The genetic background of these diseases is complex and heterogeneous. In addition to the predominant "private

Genome Editing and Cardiac Arrhythmias.

This article reviews progress in the field of cardiac genome editing, in particular, its potential utility in treating cardiac arrhythmias. First, we discuss genome editing methods by which DNA can be disrupted, inserted, deleted, or corrected in cardiomyocytes. Second, we provide an overview of in vivo genome editing in preclinical models of heritable and acquired arrhythmias. Third, we discuss recent advancements in cardiac gene transfer, including delivery methods, gene expression optimization, and potential adverse effects associated with therapeutic somatic genome editing. While genome ed

Infanticide vs. inherited cardiac arrhythmias.

AIMS: In 2003, an Australian woman was convicted by a jury of smothering and killing her four children over a 10-year period. Each child died suddenly and unexpectedly during a sleep period, at ages ranging from 19 days to 18 months. In 2019 we were asked to investigate if a genetic cause could explain the children's deaths as part of an inquiry into the mother's convictions. METHODS AND RESULTS: Whole genomes or exomes of the mother and her four children were sequenced. Functional analysis of a novel CALM2 variant was performed by measuring Ca2+-binding affinity, interaction with calcium cha
