[Arthropathy as a late result of trauma (author's transl)].

On the basis of 5 cases the syndrome of post traumatic arthropathy is described. Like its other forms it shows radiographyically a high degree of osteolysis, fragmentation and para-articular calcification of soft parts. The picture is similar to that of syringomyelia and tabes. If trauma occurs in childhood there will also be developmental retardation. One has to assume that trauma may be responsible for these isolated arthropathies at its site. Since the further course corresponds to that of a neurogenous arthropathie it is likely that in these cases there is an underlying disturbance of the

Atypical joint involvement in neuropathic arthropathy secondary to syringomyelia: A case report.

Neuropathic arthropathy (NA) is a chronic, progressive, destructive, aseptic inflammatory joint disorder resulting from sensory neurological deficit. We report a new case of NA with simultaneous involvement of the shoulder, elbow, and hands secondary to syringomyelia. Due to the rarity of this condition and its unusual location. The patient presented with painless deformation of both hands. Clinical examination revealed amyotrophy in both hands associated with hypoesthesia and bilateral areflexia of the upper limbs. The plain radiographs showed bone destruction and reconstruction in the metaca

Reconstruction of the Charcot Spine with a Vascularized Pedicle: A Report of 3 Cases.

CASE: We present 3 patients with Charcot spinal arthropathy (CSA) as a result of previous spine trauma treated initially with spinal instrumentation. All developed CSA in a delayed presentation years after the original injury and surgery. All were reconstructed with anterior and posterior surgery. In each case, the anterior reconstruction was supplemented with a vascularized pedicle graft. CONCLUSION: All 3 patients went on to fusion after previously demonstrated bone loss after a vascularized pedicle was used.

Bone Metabolism in Charcot.

Bone metabolism in the healthy, young adult is identified as a relatively stable process. Normal bone turnover is a dynamic state, which is conferred through intracellular signaling and complex cellular pathways. It has been well described in the literature that Charcot neuro-osteoarthropathy is a disease state, which is marked by intense bone turnover leading to structural collapse and dissolution of skeletal features of the foot and ankle. Within the last two decades, extensive interest has been placed in characterizing the metabolic pathogenesis of Charcot bone metabolism. Despite this work

High rate of complications after corrective midfoot/subtalar arthrodesis and Achilles tendon lengthening in Charcot arthropathy type Sanders 2 and 3.

PURPOSE: Corrective midfoot resection arthrodesis is the standard treatment of Charcot arthropathy type Sanders 2 and 3 with severe dislocation. In order to critically evaluate the effect of surgical correction, a retrospective analysis of our patient cohort was performed. Hereby, special emphasis was set on the analysis of the pre- and post-operative equinus position of the hindfoot. METHODS: Retrospectively, all patients (n = 82) after midfoot resection arthrodesis in Charcot type Sanders 2 or 3 were included. Complications were recorded, and the mean complication-free interval was calculat

Syphilis with Charcot arthropathy: A case report.

Syphilis is a chronic systemic infectious disease involving multiple organs. Involvement of skeletal system by syphilis including osteochondritis, osteoarthritis, periostitis, and osteitis. Charcot arthropathy is an unusual concomitant disease of syphilis, and the incidence had decreased significantly these decades. The present authors report a case of syphilis presenting with Charcot arthropathy of bilateral ankle joints.

Surgical Treatment Options for the Diabetic Charcot Midfoot Deformity.

Management of diabetic Charcot midfoot deformity is one of the most demanding aspects of foot and ankle surgery. Its treatment should aim at reducing the rate of complications, including foot and ankle amputations or limb loss. Attempting reconstruction at Eichenholtz stages I and II carries the risk of infection and loss of fixation. It is advisable to limit surgical reconstruction to Eichenholtz stage III in the absence of any evidence of infection or vascular insufficiency. Achilles lengthening or gastrocnemius-soleus release is an essential initial step in surgery. Addressing the medial fo

[Neurogenic osteoarthropathy of the knee associated with spina bifida: a diagnosis not to be missed].

Neurogenic arthropathy is a severe complication of chronic sensitive deficits that occurred commonly in diabetic neuropathies. It is a destructive and painless osteoarthritis associated with a loss of the deep sensitivity and a defect of protective reactions against chronic articular microtraumatisms. We report a 55-year-old woman with neuroarthropathy of the knee resulting from a spina bifida. Bisphosphonate use is an effective but non-consensual treatment.

Diabetic foot problems in India: an overview and potential simple approaches in a developing country.

India has the highest number of people with diabetes in the world. Diabetic foot care is one of the most ignored aspects of diabetes care in India. Due to social, religious, and economic compulsions, many people walk barefoot. Poverty and illiteracy lead to usage of inappropriate foot wear and late presentation of foot lesions. Many nonphysicians are interfering in the treatment of diseases, including diabetes. Patients also try home remedies before visiting their physicians. We believe that rational improvisation is the key to success when working with diabetic foot patients in developing cou

Charcot of the calcaneus.

Charcot of the calcaneus, although not as prolific as midfoot deformation, still results in significant morbidity. Current treatment centers on methods proven effective for other joints in the foot. Most neuropathic conditions of the calcaneus can be managed reasonably nonoperatively. In cases of severe deformity or ulceration, surgical management may be the more conservative approach. The surgical principles of proper soft tissue balancing and handling are critical. As the diabetic population continues to increase, the incidence of Charcot of the calcaneus will concomitantly increase. Further

Late spinal dislocation after treatment of spinal arteriovenous malformation. A case of Charcot spinal arthropathy.

Neuropathic arthropathy of the spine is a destructive condition of the spine which is secondary to a loss of the protective proprioceptive reflexes. In the majority of cases, it occurs in patients who have suffered from traumatic medullary lesions and is responsible for destruction of the vertebral bodies and considerable spinal deformity. We report a case of neurogenic lumbar arthropathy in a patient with a spinal arteriovenous malformation. This vascular lesion caused considerable disturbances of proprioception. The course was favorable with regard to the deformity after correction and fusio

Charcot Spinal Arthropathy-Induced Progression From Upper to Lower Motor Neuron Bowel Syndrome.

Charcot spinal arthropathy is a progressively degenerative joint disorder of the vertebrae. Historically, it was a common consequence of tertiary syphilis. Currently, it is a rare complication of spinal cord injury (SCI). We present the case of a 28-year-old patient with paraplegia who developed progressive, neurogenic bowel dysfunction due to Charcot spinal arthropathy. Our patient had upper motor neuron bowel syndrome secondary to SCI which advanced to lower motor neuron bowel syndrome. Charcot spinal arthropathy should be considered as a possible cause for symptom progression in SCI patient

Contralateral foot temperature monitoring during Charcot immobilisation: A systematic review.

AIMS: Contralateral temperature difference (CTD) is a frequently used marker of healing in Charcot neuro-osteoarthropathy (CN). We aimed to determine whether there is a consistent approach to CTD measurement during healing and the decision-making process around cessation of immobilisation. MATERIALS AND METHODS: Medline, Scopus, and Web of Science were searched until February 2022 for peer-reviewed studies using keywords, including (('arthropathy' OR 'osteoarthropathy' OR 'osteopathy' OR 'neuroarthropathy') AND 'Charcot' AND ('temperature')), which returned 789 results excluding duplicates.
