Radiocarpal and intercarpal arthroscopy.

The development of arthroscopic wrist surgery has helped to understand the anatomy and pathology of the wrist joint. One of the most important functions of the arthroscopic inspection in the radiocarpal and mediocarpal joints is the evaluation of the wrist stability. State of the art in the hand surgery today includes a combination of arthroscopic and arthroscopic assisted operations. The arthroscopy of the wrist offers significant information independent from the standard radiological diagnostic tools. TFCC lesions and scapholunate dissociation remain to be the most common causes for arthrosc

[Tibiotalar arthrodesis under arthroscopy].

PURPOSE OF THE STUDY: The authors describe an arthroscopic technique of tibio-talar arthrodesis. They discuss the advantages, indications and results of this technique. MATERIAL: Seven arthroscopic tibio-talar arthrodeses (A.T.T.A.) were performed between may 1992 and september 1993 for post traumatic arthritis (five following bimalleolar fractures, two following talar fractures). METHOD: Traction by means of a transcalcaneus pin was applied to obtain articular distraction. Arthroscopic debridement was performed using two standard anterolateral and anteromedial portals. The arthrodesis was s

Advances in elbow arthroscopy.

Arthroscopy of the elbow is a very precise and demanding technique. Because of the proximity of neurovascular structures to the recommended portals, a thorough knowledge of regional and intraarticular anatomy is essential. The high degree of congruency of the articular surfaces and the relatively small capsular volume make sound arthroscopic skills a necessity. With proper precautions, the technique is safe and affords the opportunity to obtain valuable diagnostic information and to perform selected procedures without the morbidity of an arthrotomy.

Complications of shoulder arthroscopy.

Shoulder arthroscopy has become a popular therapeutic and diagnostic procedure during the past decade. As with all new procedures, complications can occur. The highest complication rate (3.2%) was noted with staple capsulorrhaphy, and recurrence is more likely than with open repairs. Subacromial space procedures are safe, with a very low complication rate of less than 1%; short-term results are favorable. Proper patient selection and attention to technical considerations will decrease the incidence of complications. Careful positioning of the patient and the extremity lessens the potential of

Complications of ankle arthroscopy.

Of over 4500 ankle arthroscopies recently surveyed by the Arthroscopy Association of North America, only one complication (an infection) was reported. Nerve injury, vascular injury, or reflex sympathetic dystrophy were not reported. The ankle's anatomy is complex and there are many structures that can be damaged by imprecise portal placement. A review of our experience with 53 consecutive ankle arthroscopies found nine complications for a rate of 17%. There were three permanent dorsal sensory nerve injuries, two synovial fistulas, three wound infections, and one reflex sympathetic dystrophy. A

Progress in ankle arthroscopy.

Ankle arthroscopy is rapidly gaining in popularity as an important diagnostic and therapeutic procedure. Indications for this technique include pain, swelling, stiffness, instability, hemarthrosis, and locking of the ankle. The anteromedial, anterolateral, and posterolateral portals are most commonly used. The use of a short, 30 degrees oblique arthroscope, with an ankle holder and distractor, permits visualization of the entire joint. Operative treatment is facilitated by small joint shavers, burrs, knives, and baskets. Intraarticular problems such as chondromalacia, osteophytes, loose bodies

Arthroscopy and patellar chondropathy.

Arthroscopy has proved to be a reliable method to determine acute and chronic retropatellar damage to cartilaginous tissue. The discrepancy between clinical and radiological findings, subjective disorders, and the actual morphological changes, seem to make arthroscopy desirable in analysing an objective and documentable initial situation. Long-term examinations will have to show how reliable prognostic statements are, and how justifiable the operative or conservative measures indicated by arthroscopic examination can be.

Interphalangeal arthroscopy of the toes.

Interphalangeal (IP) joint pathology of the toes has gained little attention of the orthopedic surgeon. However, similar intra-articular pathologies e.g. osteoarthritis, synovitis, chondral lesion, arthrofibrosis and instability can occur as in other joints. Moreover, many corrective procedures of toe deformity involved excisional arthroplasty or arthrodesis of the IP joint although it is not the site of primary pathology. Classically, the operations of the IP joint were performed in an open manner. We described the technique of interphalangeal arthroscopy and its application to treat various

[Arthroscopy of the metacarpophalangeal joints].

With the advancements in arthroscopic technique, arthroscopy has become feasible in most human joints, even those as small as the finger joints. The metacarpophalangeal joints are very well suited for arthroscopy and arthroscopic therapy. Good results have been reported on arthroscopic synovectomy of the metacarpophalangeal joints in rheumatoid arthritis. Osteochondral lesions in degenerative arthritis, loose bodies and foreign bodies can well be treated. Arthroscopic arthrolysis for capsular contracture and treatment of post-traumatic lesions have been performed successfully. The arthroscopic

[Complications of hip arthroscopies].

Surgical complications of hip arthroscopies are rare in the hands of experienced hip arthroscopists. However, when performed by beginners and in more demanding situations such as marginal distraction of the head and socket and technically advanced procedures, the risk increases. This report describes possible complications which may happen during positioning and traction, portal placement, and diagnostic and therapeutic procedures. Possible causes of soft tissue lesions of the portal area, perineum and foot, intra-articular lesions of the labrum and cartilage, direct and traction-related indir

[Arthroscopy of the hip joint].

PURPOSE OF THE STUDY: Arthroscopic examination of joints has recently gained wide application. Due to hip joint shape and a difficult approach to it, hip arthroscopy has long remained outside the attention and abilities of arthroscopists. The authors present their first experience with operative hip arthroscopy that offers new options for the treatment of intra-articular pathology of the hip joint. MATERIAL: In the years 2001-2003, 24 hip arthroscopies were performed. The following pathological conditions were diagnosed and treated: loose bodies, chondral lesions of the femoral head and aceta

[Complication of arthroscopy of the knee].

Arthroscopy is an elementary surgical procedure in the management of the knee. Due to its minimal invasiveness the complications are not frequent. However, because of continuously increasing number of arthroscopies being performed worldwide (circa 1,000,000 per year), the total number of complications is significant. More frequent complications accompany particularly more complex and difficult procedures. The percentage of complications after arthroscopy oscilates about 2%. The most common complications are: intraarticular damage, neurological injury, vascular injury, compartment syndrome, ins

[Arthroscopy of the elbow joint].

A series of 31 patients who had elbow arthroscopy has been reviewed. Good results have been achieved after removal of loose bodies or post-traumatic hematoma. A danger of injury to periarticular structures due to the use of arthroscopic instruments, especially with pathologically decreased joint volume has been pointed out.

Arthroscopy of the shoulder joint.

Arthroscopy is without doubt the most comprehensive procedure for shoulder lesions, providing even better inspection of the interior of the joint than conventional open procedures. In 174 diagnostic arthroscopies, it proved more reliable than the computed tomography scan, ultrasound, or arthrography. The results of ultrasonography proved disappointing despite the enthusiastic reports on this technique, probably due to imperfections of equipment and lack of expertise rather than any inherent defect in the method. Arthrography seems to have a high incidence of false results. Though an invasive m

Arthroscopy of the knee in hemophilia.

Arthroscopic synovectomy for chronic hemophilic arthropathy has been performed since June of 1980, giving a 4 to 8 year follow-up of eight knees. The procedure was very successful in decreasing the incidence of hemarthroses. Only one patient had a recurrence of bleeding with synovitis. Joint flexion and extension returned to the preoperative status or improved in five knees. Only one patient lost significant motion. This loss occurred secondary to postoperative complications. Although arthroscopic synovectomy proved to be effective in synovial removal and decreasing the incidence of recurrent

Arthroscopy of the knee in children.

The complaint of knee pain is frequent in children. The difficulty of obtaining an accurate history from the younger child adds to the already-difficult problem of establishing an accurate diagnosis from clinical examination and routine radiographs in this group. This report is a prospective study of 32 children less than 18 years of age who underwent arthroscopy either because their symptoms failed to resolve with conservative treatment or because knee surgery was planned. The findings suggested some important differences between these children over 13 years of age and those age 13 years or y

[Complications of knee arthroscopy].

BACKGROUND: Today, knee arthroscopy is one of the standard interventions performed by an orthopaedist and usually has a low potential for complications. PURPOSE: The surgeon should still be aware of possible problems, and be able to control and explain them to the patient in detail before surgery. MATERIALS AND METHODS: The possible relevant peri-, intra- and postoperative complications of knee arthroscopy are discussed. RESULTS: Evaluation of the patient's medical history and comorbidities is crucial to successful treatment, in addition to a correct diagnosis and indications with the assis
