Forty-five years later: the challenge of optimizing assertive community treatment.

PURPOSE OF REVIEW: Assertive Community Treatment is an established evidenced based practice that provides intensive community treatment for individuals with severe mental illness with recurrent hospitalizations and/or homelessness. Emerging evidence indicates limitations in its implementation in terms of to the original ACT model and its current relevance. RECENT FINDINGS: Findings from recent studies (2018-2020) reveals challenges with implementation centered around basic implementation activities, such as changes in the psychosocial context of individuals with SMI, clinicians' abilities to

Duration of device-detected subclinical atrial fibrillation and occurrence of stroke in ASSERT.

BACKGROUND: ASSERT demonstrated that subclinical atrial fibrillation (SCAF) is common in pacemaker patients without prior AF and is associated with increased risk of ischemic stroke or systemic embolism. SCAF episodes vary in duration and little is known about the incidence of different durations of SCAF, or their prognosis. METHODS AND RESULTS: ASSERT followed 2580 patients receiving a pacemaker or ICD, aged >65 years with hypertension, without prior AF. The effect of SCAF duration on subsequent risk of ischemic stroke or embolism was evaluated with time-dependent covariate Cox models. Patie

Clinical Factors Associated with Successful Discharge from Assertive Community Treatment.

We sought to explore clinical factors associated with successful transition from Assertive Community Treatment to less intensive clinical services. Mixed-method observational follow up study of veterans discharged from three VA-affiliated ACT teams to less intensive clinical services. Of the 240 veterans in ACT, 9% (n = 21) were discharged during the study period. Among the 11 of 21 discharged veterans who enrolled in the follow up study, reason for discharge, designated by the veteran's primary clinician at the time of discharge, predicted outcomes (p = 0.02) at 9 months, with "disengagement"

Recovery-oriented service provision and clinical outcomes in assertive community treatment.

OBJECTIVE: While the term "recovery" is routinely referenced in clinical services and health policy, few studies have examined the relationship between recovery-oriented service provision and client outcomes. The present study was designed to examine the relationship between recovery-orientation of service provision for persons with severe mental illnesses and outcomes in Assertive Community Treatment (ACT). METHODS: Client, family, staff, and manager ratings of service recovery-orientation and outcomes across a range of service utilization and community functioning indicators were examined a

Characteristics of teams, staff and patients: associations with outcomes of patients in assertive outreach.

BACKGROUND: Little is known about what characteristics of teams, staff and patients are associated with a favourable outcome of severe mental illness managed by assertive outreach. AIMS: To identify predictors of voluntary and compulsory admissions in routine assertive outreach services in the UK. METHOD: Nine features of team organisation and policy, five variables assessing staff satisfaction and burn-out and eleven patient characteristics taken from the baseline data of the Pan-London Assertive Outreach Study were tested as predictors of voluntary and compulsory admissions within a 9-mont

Perceived stigma and community integration among clients of assertive community treatment.

Facilitating the integration of persons with psychiatric disabilities is an important goal of community programs. In addition to limitations such as inadequate housing, poverty, and unemployment, a common barrier to community adjustment identified by people with psychiatric disabilities is their perception of being stigmatized. This study examined the relationships between perceived stigma and community integration in 95 clients of assertive community treatment (ACT) teams, using sequential multiple regression procedures. Findings indicate that despite their physical presence in the community

Sexual assertiveness mediates the effect of social interaction anxiety on sexual victimization risk among college women.

Sexual victimization is prevalent among college women and is associated with adverse psychological consequences. Social anxiety, particularly related to interpersonal interaction, may increase risk of sexual victimization among college women by decreasing sexual assertiveness and decreasing the likelihood of using assertive resistance techniques. This study examined social interaction anxiety as a risk factor for sexual victimization. College women (n=672) completed online measures of social interaction anxiety, sexual assertiveness, and sexual victimization experiences. Social interaction anx

Sexual revictimization during women's first year of college: self-blame and sexual refusal assertiveness as possible mechanisms.

Although sexual victimization during adolescence increases risk for later revictimization, mechanisms for increased risk among new college students have not been identified. Female undergraduates (N = 87) were assessed at the start and end of their first academic year. Those who reported initial sexual victimization at Time 1 were more likely than other women to report later college victimization at Time 2. Path analyses showed that self-blame and decreased sexual refusal assertiveness (SRA) explained this effect. Specifically, initial victimization was associated with increased self-blame; in

Antibiotics and Surgical Site Infection in Expander-Based Breast Reconstruction Trial (ASSERT).

BACKGROUND: The use of prophylactic antibiotics following postmastectomy tissue expander breast reconstruction (TE-BR) varies widely. The Centers for Disease Control and Prevention (CDC) recommends a single preoperative antibiotic dose for clean and clean-contaminated procedures. This multi-institutional, prospective, randomized controlled trial (RCT) examined whether the CDC-recommended single preoperative dose (SPD) of antibiotics is not inferior to an additional week of postoperative (WPO) prophylactic antibiotics in preventing surgical site infection (SSI) in immediate TE-BR following mast

Evaluating the consistency of SMARCB1 variant classification and assertions of genotype-phenotype relationships in ClinVar.

Pathogenic SMARCB1 variants are associated with multiple Mendelian syndromes: Schwannomatosis (SM), Rhabdoid Tumour Predisposition Syndrome (RTPS1), and/or Coffin-Siris Syndrome (CSS). Although some data suggests genotype/phenotype relationships based on mutation type and location, this is not used consistently potentially due to inconsistent or absent phenotypic data. We used ClinVar, the largest public platform of variant classifications, to evaluate variant-specific data from clinical laboratories and cited publications to assess reporting consistency and genotype-phenotype relationships. Seeking alternatives, asserting choices: Dr Mahendralal Sarkar's life in medicine and science.

This article examines the medical life of a native physician who embraced homoeopathy despite being trained in the dominant pathy i.e. allopathy at the prestigious Calcutta Medical College in the 1860s. Mahendralal Sarkar, the physician in focus, was one of the first famous converts to homoeopathy. His seeking of an alternative option created space and legitimacy for homoeopathy. Through the journal Calcutta Journal of Medicine that Sarkar founded, he, with great ability, asserted and defended his right as a physician to decide in favour of homoeopathy. Sarkar gradually established himself as

Effects of an assertive community program in patients with severe mental disorders and impact on their families.

AIMS: Assertive community treatment (ACT) is known to have a positive impact on the number and length of inpatient stays. However, recent studies have shown little or no effect of such programs in European settings. This paper aims to describe the impact of a newly implemented ACT program on patients and their families' burden. Predictive factors have also been examined. METHOD: Fifty-five patients characterized by heavy use of psychiatric care, numerous hospitalizations, or failure to link with outpatient psychiatric care and their relatives were followed. Data were gathered on patients befo

[Study of the impact of an assertive community program on the families of patients with severe mental disorders].

OBJECTIVES: Assertive Community Treatment (ACT) is known to have a positive impact on the number and length of inpatient stays. Yet, research is needed in order to help understand how ACT programs may ease off families' burden, e.g. in terms of economic expenditures. Indeed, many families with siblings suffering from chronic mental illness, who disengaged from psychiatric services, report needs related to ACT. This paper aims to describe the impact of a new ACT program in Geneva on patients and their families' burden. METHODS: Out of 91 patients consecutively treated by the ACT program for at

Behavioral and bodily self concept changes after assertive training. A pilot study.

The study aims at finding correlations between the development of assertiveness through assertive training and the modification of the bodily self concept (or body image). Eleven social phobic patients (6 women, 5 men, mean age 31.5) are treated in two groups and measured by Rathus's assertiveness schedule, and Osgood's semantic differential (key concepts: my self, my body, my voice, my gestures, my look; control concept: a table). At post test and follow-up (between 6 and 9 months), an ANOVA shows a significant positive change in assertiveness (p 0.001), correlated with modifications in bodil

Implementation of Sterilization and Disinfection Protocols for Potentiometric Microneedle Sensors: A Requirement for a Safety Assertion.

Microneedle (MN) sensors represent a promising technology for disease surveillance. For their application in long-term animal studies and preclinical research, they must exhibit both analytical reliability and biosafety, including the elimination of microorganisms capable of inducing infection. Nonetheless, MNs' compatibility with conventional sterilization and disinfection methods remains largely unexplored. Accordingly, we introduce the first systematic evaluation of the compatibility of potentiometric MN sensors with sterilization and high-level disinfection protocols widely available in la

The East tennessee assertive adolescent family treatment program: a three-year evaluation.

The Assertive Adolescent Family Treatment Program targeted adolescents defined as age 12 to 17 years and transition age youth (age 18-24) who were served by the Helen Ross McNabb Center Regional Mental Health System. Most of the participating individuals met the Diagnostic and Statistical Manual of Mental Disorders(IV-TR) criteria for substance abuse or substance dependence and co-occurring mental health disorder. Significant findings in the study included a decrease in illegal drug use, decrease in alcohol consumption, decrease in criminal activity, decrease in violent behaviors, and an incre

Staff and consumer perspectives on defining treatment success and failure in assertive community treatment.

OBJECTIVE: Although assertive community treatment (ACT) has been consistently recognized as effective, there has been little research as to what constitutes success in ACT. The purpose of this study was to understand how ACT consumers and staff define treatment success and failure and to examine whether definitions varied between staff and consumers. METHODS: Investigators conducted semistructured interviews with 25 staff and 23 consumers from four ACT teams. RESULTS: Across perspectives, success and failure were most clearly related to consumer factors. Other themes included having basic ne

Cost-effectiveness of assertive community treatment for homeless persons with severe mental illness.

BACKGROUND: Homelessness is a major public health problem among persons with severe mental illness (SMI). Cost-effective programmes that address this problem are needed. AIMS: To evaluate the cost-effectiveness of an assertive community treatment (ACT) programme for these persons in Baltimore, Maryland. METHODS: A total of 152 homeless persons with SMI were randomly allocated to either ACT or usual services. Direct treatment costs and effectiveness, represented by days of stable housing, were assessed. RESULTS: Compared with usual care, ACT costs were significantly lower for mental health i
