Left Atrial Function in Patients with Titin Cardiomyopathy.

BACKGROUND: Truncating variants in titin (TTNtv) are the most prevalent genetic etiology of dilated cardiomyopathy (DCM). Although TTNtv has been associated with atrial fibrillation, it remains unknown whether and how left atrial (LA) function differs between patients with DCM with and without TTNtv. We aimed to determine and compare LA function in patients with DCM with and without TTNtv and to evaluate whether and how left ventricular (LV) function affects the LA using computational modeling. METHODS AND RESULTS: Patients with DCM from the Maastricht DCM registry that underwent genetic test

Left atrial appendage function assessment and thrombus identification.

BACKGROUND: The diagnosis of thrombus in the left atrium in patients with persistent atrial fibrillation (AF) and may be inconsistent because of variability in thrombus morphology. In some cases it is challenging and requires unusual approach. New Doppler-derived methods might be helpful to identify such thrombi. We evaluated quantitative differences in mechanical function of the left atrial appendage (LAA) basal segments using tissue Doppler imaging (TDI)and speckle tracking echocardiography (STE) in patients with non-valvular AF with and without LAA thrombus and compared them with SR patient

Ventricular function in thyrotoxicosis complicated by atrial fibrillation.

The radionuclide gated ventriculogram was used to study the association between thyrotoxic atrial fibrillation and underlying ischaemic heart disease. Comparison between ten patients with atrial fibrillation induced by thyrotoxicosis which reverted to sinus rhythm after return to a euthyroid state, and ten age-matched controls with treated hyperthyroidism who never developed atrial fibrillation, showed no significant difference in the incidence of underlying ischaemic heart disease. This study suggests that further investigation for underlying coronary arterial disease is not indicated in simp

Left atrial booster function in valvular heart disease.

This study was designed to assess atrial booster pump action in valvular heart disease and to dissect booster pump from reservoir-conduit functions. In five patients with aortic stenosis and six with mitral stenosis, sequential atrioventricular (A-V) pacing was instituted during the course of diagnostic cardiac catheterization. Continuous recording of valvular gradient allowed estimation of flow for each cardiac cycle by transposition of the Gorlin formula. Left ventricular ejection time and left ventricular stroke work in aortic stenosis or left ventricular mean systolic pressure in mitral st

Atrial function is altered in lone paroxysmal atrial fibrillation in male endurance veteran athletes.

AIMS: Intensive endurance sport practice is associated with an increased risk of atrial fibrillation (AF) in male veteran athletes. Paroxysmal AF (PAF) is the very beginning step of this disease. The description of atrial remodelling occurring at this early stage might enable to depict predictive factors of AF in veteran athletes in order to give them personalized recommendation according to their sport practice. METHODS AND RESULTS: Twenty-seven male endurance veteran athletes with documented PAF were retrospectively enrolled and compared with 30 control endurance athletes without documented

Estimation of atrial fibrillation burden as a function of monitoring time without atrial fibrillation.

BACKGROUND: Atrial fibrillation (AF) burden is associated with adverse outcomes; in some patients, very low AF burden may not be actionable. OBJECTIVE: This study aimed to model AF burden as a function of monitoring time without AF. METHODS: We included 11,639 US patients (median age 62 years [interquartile range 46‒73], 40.6% male) without permanent/persistent AF, monitored for ≥14 days in 2021 with the PocketECG device (MEDICALgorithmics). The overall 14-day AF burden was modeled as a function of monitoring duration without AF episodes lasting ≥30 seconds to <6 minutes, ≥6 minutes to 1 hou

Integrative Evaluation of Atrial Function and Electromechanical Coupling as Predictors of Postoperative Atrial Fibrillation.

Background and Objectives: Postoperative atrial fibrillation (POAF) remains one of the most frequent complications after cardiac surgery, increasing the risk of morbidity, prolonged hospitalization, and adverse long-term outcomes. Although several clinical and echocardiographic factors have been associated with POAF, the integrated contribution of atrial conduction delay, biatrial mechanics, and atrioventricular coupling to arrhythmogenesis remains unclear. Materials and Methods: This retrospective study included 131 adult patients undergoing coronary artery bypass grafting and/or aortic valve

Arnebiae Radix prevents atrial fibrillation in rats by ameliorating atrial remodeling and cardiac function.

ETHNOPHARMACOLOGICAL RELEVANCE: Arnebiae Radix, a common herbal medicine in China, is often utilized to treat blood-heat syndrome and has been reported to exert an effect on the heart. AIM OF THE STUDY: The combination of acetylcholine (Ach) and CaCl2 has been widely used to induce atrial fibrillation (AF) in animals. However, whether Arnebiae Radix displays any preventive action on Ach-CaCl2 induced AF in rats remains uncertain. In our study, we attempted to investigate the protective effects of Arnebiae Radix on Ach-CaCl2 induced AF compared to amiodarone, which was employed as the positive

Atrial size and transport function after the Maze III procedure for paroxysmal atrial fibrillation.

BACKGROUND: The Maze procedure is a curative surgical therapy for atrial fibrillation, restoring sinus rhythm and improving quality of life. Because the procedure results in tissue scarring, the atrial transport function is most likely affected. METHODS: Seventeen patients with paroxysmal atrial fibrillation underwent the Maze III procedure without any concomitant valve operation. Atrial size and transport function were measured before and at 2, 6, and 24 months after operations with two-dimensional echocardiography and pulsed-wave Doppler. RESULTS: Fifteen patients (88%) had signs of left a

Clinical and echocardiographic features influencing recovery of atrial function after cardioversion of atrial fibrillation.

Atrial mechanical dysfunction after cardioversion for atrial fibrillation has been widely evaluated in recent years. Nevertheless, the influence of many clinical and echocardiographic parameters is not yet understood. The aim of the present study was to evaluate the influence of clinical and echocardiographic parameters on the return of effective atrial contraction. A total of 109 patients were evaluated: 41 patients had spontaneous recovery of sinus rhythm and 68 patients were randomly treated using either direct-current (DC) shock or intravenous procainamide. Elective cardioversion was accom

Functional tricuspid regurgitation and efficacy of electrical cardioversion in patients with atrial fibrillation and atrial functional mitral regurgitation.

INTRODUCTION: Atrial functional mitral regurgitation (afMR) is common in patients with atrial fibrillation (AF). The presence of functional tricuspid regurgitation (fTR), which arises as a secondary effect of afMR, has the potential to impact the effectiveness of procedures aimed at restoring normal heart rhythm. In this study, we sought to evaluate the efficacy of electrical cardioversion (CVE) in AF regarding the presence and degree of fTR in patients with afMR. MATERIAL AND METHODS: Retrospective analysis included 521 patients with persistent AF on optimal medical therapy undergoing CVE. 1

Trans-esophageal Echocardiographic Assessment of Left Atrial and Left Atrial Appendage Function in Atrial Fibrillation and Rheumatic Heart Disease.

INTRODUCTION: Rheumatic heart disease (RHD) is one of the most typical causes of atrial fibrillation in developing countries like India. The left atrial and left atrial appendage structure and function are deranged in atrial fibrillation and are a major source of thromboembolism. The goal of this study was to assess the left atrial and left atrial appendage function by transesophageal echocardiography in patients with atrial fibrillation and their comparison in patients with or without RHD. METHODS: A total of 172 consecutive patients with atrial fibrillation with or without RHD were subjecte

Atrial Functional Tricuspid Regurgitation as a Valvular Manifestation of Right Atrial Failure.

Functional tricuspid regurgitation has traditionally been attributed to right ventricular remodeling secondary to pulmonary hypertension or left-sided heart disease. However, contemporary imaging studies increasingly identify patients with significant tricuspid regurgitation despite preserved right ventricular geometry, characterized instead by marked right atrial (RA) enlargement and tricuspid annular dilation, most commonly in the setting of atrial fibrillation. This phenotype, termed atrial functional tricuspid regurgitation (AF-TR), is typically described as regurgitation caused by isolate
