At the current state of the art a comparison of gold salts and other therapeutic modalities in terms of their risk-benefit ratios clearly shows chrysotherapy to have a place in the treatment of active rheumatoid arthritis. Factors supporting the use of gold salts as base-line drugs for active rheumatoid arthritis include the extensive experience accumulated with chrysotherapy as a basic antirheumatoid treatment and the potential of gold salts to halt joint destruction. Three children with juvenile rheumatoid arthritis were given a single oral dose of approximately 0.1 mg/kg of auranofin. Blood concentrations up to 49 ng/ml were detectable at 24 h (earliest sample) and remained detectable for at least 2 weeks (final sample). Disappearance of measurable gold from both the blood and serum appeared to be linear through time. None of the patients experienced significant laboratory abnormalities nor adverse reactions. Gold levels attained in these children were somewhat lower than those seen in adults administered an equivalent dose.

We report a case of gold pulmonary toxicity in a patient with adult-onset Still's disease with dyspnea on exertion and a normal chest radiograph. Withdrawal of gold therapy resulted in complete resolution of pulmonary toxicity in our patient without the need for additional steroid therapy.

Benefits and risks of gold therapy in patients with juvenile chronic arthritis (JCA) were reviewed. An improvement of laboratory and clinical parameters was observed in eight out of the 16 children of the sample (10 boys and 6 girls). The side effects were noticed in 5 patients, but only in one of these children gold therapy had to be stopped due to alopecia areata. Corticosteroid dose was significantly reduced in 7 out of the 11 patients. It is concluded that gold salts may be effectively applied as primary therapy in children with JCA, with no more risk than in adults.

Auranofin (AF), an oral gold salt, was given for 9-18 months to 3 patients with pemphigus vulgaris. There was a slight improvement with significant oral pain relief in 2, and a complete remission in 1. Two patients including the one with the remission, subsequently flared up under AF therapy. Although AF is said to be better tolerated than parenteral gold this limited series suggests that it does not seem to have a superior therapeutic effect in PV.

A 63-year-old woman with seropositive rheumatoid arthritis was successfully treated with auranofin for 12 months. Heavy proteinuria then developed and the drug was stopped. However, the proteinuria apparently became worse. This was later recognised to be a spurious effect due to the introduction of tolmetin. Renal biopsy showed a type I membranous glomerulonephropathy.

Based on our own observations in 40 patients and a review of the literature the clinical and histological aspects of dermatitis caused by gold therapy are discussed. There is a broad spectrum of cutaneous side effects following gold therapy. In most of the patients a nonspecific dermatitis can be observed. The skin lesions may also mimic lichen planus, pityriasis rosea or various forms of eczema, obscuring early diagnosis. Histologically four types of reactions can be differentiated including a dermatitis type, vasculitis type, a lichenoid and an urticarial type.

Oxygen toxicity and antioxidant deficiencies contribute to the development of bronchopulmonary dysplasia.
