Benefits of lethal pandemics: direct impact of contagious diseases on public administration in Hungary (1867-1914).

The reconciliation of 1867 between Austria and Hungary brought great changes to Hungarian public administration: the way towards the building up of a modern public administration had been opened. Although there was a functioning public health system and a related legislation from the late 18th century, major issues - such as balanced geographical distribution of medical personnel, fair access to medical services even in the poorer regions of the country, and the effective protection against some contagious diseases - were not resolved for decades. During the reform work of public administratio

Gout in immigrant groups: a cohort study in Sweden.

Our aim was to study the association between country of birth and incidence of gout in different immigrant groups in Sweden. The study population included the whole population of Sweden. Gout was defined as having at least one registered diagnosis in the National Patient Register. The association between incidence of gout and country of birth was assessed by Cox regression, with hazard ratios (HRs) and 95% confidence intervals (95% CI), using Swedish-born individuals as referents. All models were conducted in both men and women, and the full model was adjusted for age, place of residence in Sw

Lungworms and gastrointestinal parasites of domestic cats: a European perspective.

With the exception of Aelurostrongylus abstrusus, feline lungworms have been poorly studied. Information on their distribution is patchy and mostly limited to case reports. In this study, the occurrence of feline lungworms and co-infecting gastrointestinal parasites has been investigated in 12 European countries (i.e. Austria, Belgium, Bulgaria, France, Greece, Hungary, Italy, Portugal, Romania, Spain, Switzerland and the United Kingdom). An average of 10 domestic cats, with regular outdoor access, was sampled each month for 12months, and freshly passed faeces were collected. Stools were proce

Semmelweis and the aetiology of puerperal sepsis 160 years on: an historical review.

It is generally accepted that Professor Ignaz Semmelweis was the first to identify the mode of transmission of puerperal sepsis. However no appropriate statistical analysis of Semmelweis's data supporting his theory has been reported. Mean annual percent maternal mortality rates for the Allgemeines Krankenhaus and Dublin Maternity Hospitals (1784-1858) were analysed. The introduction of pathological anatomy at the Allgemeines Krankenhaus in 1823 was associated with increased mortality. After 1840 maternal mortality was higher in Clinic 1 which was staffed by male obstetricians and medical stud

EQ-5D in Central and Eastern Europe: 2000-2015.

OBJECTIVE: Cost per quality-adjusted life year data are required for reimbursement decisions in many Central and Eastern European (CEE) countries. EQ-5D is by far the most commonly used instrument to generate utility values in CEE. This study aims to systematically review the literature on EQ-5D from eight CEE countries. METHODS: An electronic database search was performed up to 1 July 2015 to identify original EQ-5D studies from the countries of interest. We analysed the use of EQ-5D with respect to clinical areas, methodological rigor, population norms and value sets. RESULTS: We identifie

[Viral encephalitis after tick bite].

TBE is caused by a Flavivirus and is endemic in certain areas of Central and Eastern Europe, such as Austria, the south of Germany, Hungary, Czechoslovakia, Poland, the former Yugoslavia and SNG and certain areas of Sweden and Finland. Recent developments in the Eastern European countries have made them popular with Danish travellers, who are thus exposed to infection. Until now TBE has been a very rare disease in Denmark. The clinical course is biphasic being preceded by an incubation period of ten days. After the initial phase with uncharacteristic symptoms, a period af about five days follo

Protocol for a prospective cohort study on the use of clinical nutrition and assessment of long-term clinical and functional outcomes in critically ill adult patients.

BACKGROUND AND AIMS: Limited data are available on the impact of clinical nutrition over the course of critical illness and post-discharge outcomes. The present study aims to characterize the use of nutrition support in patients admitted to European intensive care units (ICUs), and its impact on clinical outcomes. Here we present the procedures of data collection and evaluation. METHODS: Around 100 medical, surgical, or trauma ICUs in 11 countries (Austria, Belgium, Czech Republic, Germany, France, Hungary, Italy, Poland, Spain, Sweden, United Kingdom) participate in the study. [E. M. Jellinek's silenced and silencing transgenerational story].

Jellinek is a kind of archetypal character for future generations in the field of addiction studies. His implosion in the arena of alcoholism around the age of 50 was an unexpected challenge to medical science. We know very little about his own role models giving an intellectual and moral compass to his pragmatic creativity. More than 30 years has passed since Jellinek's death when an American sociologist Ron Roizen started unearthing his silent story. Roizen discerned that there are a lot of unsaid and muted issues in his personal Hungarian past. Our paper, based on the authors' research in H

Prevalence and resistance of commensal Staphylococcus aureus, including meticillin-resistant S aureus, in nine European countries: a cross-sectional study.

BACKGROUND: Information about the prevalence of Staphylococcus aureus resistance to antimicrobial drugs has mainly been obtained from invasive strains, although the commensal microbiota is thought to be an important reservoir of resistance. We aimed to compare the prevalence of nasal S aureus carriage and antibiotic resistance, including meticillin-resistant S aureus (MRSA), in healthy patients across nine European countries. METHODS: In this cross-sectional study, nasal swabs were obtained from 32,206 patients recruited by family doctors participating in existing nationwide family doctor net

Human Dirofilaria repens infections diagnosed in Slovakia in the last 10 years (2007-2017).

Human dirofilariosis is currently considered to be an emerging zoonosis of Central Europe with climate change and globalization playing a key role in its spreading. Additionally, the occurrence of Dirofilaria spp. is strongly subject to a certain number of microfilaremic dogs and the presence of the mosquito vectors. Both conditions are fulfilled in Slovakia. The first human case of autochthonous dirofilariosis in Slovakia was diagnosed in 2007, 2 years after the infection had been observed in the Slovak dog population. Since then, a total of 12 human cases caused by Dirofilaria repens have be

Oesophagitis in Italian and Hungarian asymptomatic subjects.

It is known that chronic inflammatory phenomena of the gastric mucosa fairly often occur in asymptomatic subjects. Starting from this fact, the authors decided to investigate possible non-symptomatic oesophageal inflammatory lesions referable to oesophagitis and any other digestive and/or systemic pathology. Furthermore it was also intended to ascertain the possible existence of geographical differences. 108 subjects, 65 Italians and 43 Hungarians, were studied. The endoscopic examination revealed congestion of the lower third of the oesophageal tract in 5 Italians out of 65, whereas all the H

Skin and Disease in Early Modern Medicine: Jan Jessen's De cute, et cutaneis affectibus (1601).

This article examines skin and disease in early modern medicine through the writings of the little-known Bohemian physician Jan Jessen (1566-1621). In 1601, Jessen published De cute, et cutaneis affectibus, a set of twenty-one theses dedicated to the question of whether skin disease existed. In considering Jessen and his relationship to a broader world of writing, this article makes three arguments. First, it suggests that, contrary to existing historiography, the question of skin disease was a common sixteenth-century concern. Second, it posits a professional channel for this concern, which a

Alexander Gordon, puerperal sepsis, and modern theories of infection control--Semmelweis in perspective.

Ignaz Semmelweis, a Hungarian doctor who practised in 19th century Vienna, is widely believed to be the father of modern infection control. He earned this accolade when he showed that puerperal sepsis was contagious and that it could be prevented with adequate hand hygiene. In fact, such ideas had circulated in the medical world for at least a century before Semmelweis' work. Moreover, it is well documented that Alexander Gordon, an obstetrician working in Aberdeen, UK, was the first to prove the contagious nature of puerperal sepsis. He also advocated the need for good hygiene for its prevent

Nationwide Survey of Klebsiella Pneumoniae Strains Producing CTX-M Extended-spectrum β-lactamases in Croatia.

Extended-spectrum β-lactamases (ESBL) producing bacteria have been increasingly reported in both hospital and community patients. Production of ESBLs is the major mechanism of resistance to oxymino-cephalosporins and aztreonam in Gram-negative bacteria. Recently a new family of ESBLs with predominant activity against cefotaxime (CTX-M β-lactamases) has been reported. Over 80 CTX-M enzymes have been described so far, which can be grouped into five main subgroups according to amino acid sequence identity (CTX-M-1, CTX-M-2, CTX-M-8, CTX-M-9 and CTX-M-25). In some countries, CTX-M β-lactamases are

Equine multinodular pulmonary fibrosis (EMPF): Five case reports.

Equine multinodular pulmonary fibrosis (EMPF), a progressive fibrosing interstitial lung disease has been associated with gammaherpesviruses. This case series describes five horses with EMPF. Three of the horses (two in Hungary, one in the Czech Republic) were diagnosed with EMPF ante mortem. They presented with typical clinical signs of EMPF including dyspnoea and weight loss. Arterial blood gas analysis revealed hypoxaemia. Blood work showed signs of inflammation like neutrophilia and hyperfibrinogenaemia.
